A case of ureteric injury postappendectomy presenting as ureterocutaneous fistula.

Ureteric injuries, although uncommon, are a potential complication in pelvic and gynaecological surgeries and can have serious implications. The risk is mainly related to the complexity of the surgical procedure and the presence of eventual periureteric pathology. Ureteric injury during appendectomy is rare. The ureter may be damaged by laceration, being crushed, or thermal or ischaemic injury. The retrocaecal appendix, due to its proximity to the ureter, may also get accidentally injured. The most effective measure to prevent iatrogenic injury is to have a sound knowledge of abdominal and pel

Siphoning diesel: a fatal mistake.

Diesel is commonly used as fuel for engines and is distilled from petroleum. Diesel has toxic potential and can affect multiple organs. Exposure can occur after ingestion, inhalation or through the dermal route. The practice of siphoning diesel using a rubber tubing and the mouth is common in rural communities. This can lead to accidental ingestion and aspiration. Here we report a case of a patient who accidentally ingested diesel during siphoning, which caused extensive erosion of the oral cavity and oesophagus leading to pneumomediastinum and severe chemical lung injury. The patient responde

Adolescent with penile fracture and complete urethral transection.

We report the first pediatric case of penile fracture with complete urethral transection caused by accidental fall. The patient presented atypically with the majority of swelling and ecchymosis occurring in the scrotal area rather than the penile shaft. The urethral injury occurred at the junction of the penile and bulbar urethra with significant urinary extravasation into the scrotum. The injury was repaired with plastic reanastomosis of the corpora cavernosa and urethra using a scrotal approach, rather than the commonly described degloving incision. The diagnosis and management of this unusu

[CLAVICLE FRACTURES IN CHILDREN--CIRCUMSTANCES AND CAUSES OF INJURY].

Clavicle fractures in children occur twice as often as in adults. During a child's growth period they account for 10-15% of all fractures sustained. The questions which should be asked are how these fractures are sustained and under which circumstances are the children injured. In the study 256 children with clavicle fractures treated during the period 2008-2013 were analyzed. The underlying cause and place of injuries were classified using the ICD-10 classification system, using environmental causes of injury. The circumstances were in each case accidental injury. Environmental causes were tr

Prophylactic Fibrin Glue Application for Immediate Management of Dural Puncture during Spinal Cord Stimulation Lead Placement: a Simple and Effective Technique.

BACKGROUND: Accidental dural puncture during epidural lead insertion for Spinal Cord Stimulation (SCS) is a recognized surgical complication that may lead to cerebrospinal fluid (CSF) leakage and subsequent postdural puncture headache (PDPH). The optimal technical approach to prevent CSF leakage remains controversial. This study aimed to evaluate a simple and efficient intraoperative technique for managing accidental dural puncture during SCS lead placement. MATERIALS AND METHODS: A retrospective review was conducted of the medical records and imaging studies of all patients who underwent SCS

Experimental modelling of imposed upper airway obstruction in infants and children.

The interpretation of injuries to children and infants poses a number of difficulties to any medical practitioner involved in their care or tasked with the investigation their death. This includes differentiating accidental from non-accidental trauma and the consideration of medical factors making a child more prone to injury. Non-fatal but life-threatening upper airway obstruction is unfortunately a well-recognized pattern of abuse which may precede a fatal episode. In this experimental study, we aimed to model theoretical digit marks to the head using infant and young child resuscitation dum

Identifying characteristics and outcomes that are associated with fall-related fatalities: multi-year retrospective summary of fall deaths in older adults from 2005-2012.

BACKGROUND: Fall-related deaths continue to be the leading cause of accidental deaths in the older adult (65+ year) population. However, many fall-related fatalities are unspecified and little is known about the fall characteristics and personal demographics at the time of the fall. Therefore, this report describes the characteristics, circumstances and injuries of falls that resulted in older adult deaths in one U.S. County and explores the variables associated with fatal injuries from falls. METHODS: This is a continued retrospective analysis of 841older adults whose underlying cause of dea

Laser eye injuries in military occupations.

INTRODUCTION: Lasers (light amplification by stimulated emission of radiation) play an important role in our world and their use is increasing. They are powerful tools for good, but can also cause tragedy, especially in an aviation environment. Information about injuries associated with lasers is limited. This study highlights several laser eye injuries in the U.S. military and discusses issues pertaining to them. METHODS: We gathered data from the U.S. Army Safety Center, the U.S. Army Center for Health Promotion and Preventive Medicine, and the Walter Reed Army Institute of Research. This p

Postmortem Artifacts Caused by Freshwater Crab Scavenging on Human Remains in Sri Lanka: A Case Series.

Artifacts from aquatic animal scavenging can disfigure human remains, making identification difficult, obscuring or mimicking antemortem injuries, and destroying evidence. Although freshwater crabs are common scavengers in tropical freshwater, literature describing their effects on human remains is limited. This case series describes postmortem scavenging by the freshwater crabs Oziotelphusa ceylonensis and Ceylonthelphusa rugosa in Sri Lanka, highlighting their forensic significance. Three adult males who died in separate incidents of accidental drowning were recovered from shallow freshwater

Characteristics of Child Deaths and Near-Deaths Resulting From Violence or Maltreatment.

U.S. children suffer higher rates of death resulting from violence than children in economically comparable countries. Prevention of violent injury relies on robust data regarding demographic and contributing factors. This retrospective study of records from the New Mexico Office of the Medical Investigator (OMI), the University of New Mexico Children's Hospital (UNMH), and the UNMH child protection team examined demographic and risk factors for child deaths and near-deaths resulting from violence or maltreatment (abuse or neglect) during the period August 1, 2019 - September 30, 2022. The OMI

Penetrating brain injury with a nail gun: technical considerations and the role of cerebral angiography. Illustrative case.

BACKGROUND: Nail gun injuries are rare forms of penetrating brain injuries that can be associated with significant morbidity and mortality. Management strategies differ greatly in previously reported cases. OBSERVATIONS: A male in his 40s presented with an accidental, work-related nail gun injury to the left parietal lobe with no focal neurological deficits. Noncontrast CT imaging of the head and vessel imaging including CT angiography and venography were performed that demonstrated the retained nail with trace surrounding intraparenchymal hemorrhage but no obvious vascular injury. An Unusual Case of Transorificial Abdomino-Thoracic Impalement Injury in a Child.

Impalement injuries are rare in the paediatric age group. Still rarer are injuries which traverse multiple body cavities. Such injuries require multispecialty management at a tertiary care centre. We describe a case of an accidental impalement injury in a 12-year-old boy after a fall from height. The rod was seen passing through the right lung in the paracardiac region, piercing the diaphragm and segment IV of the liver and then coursing anterior to the inferior vena cava in the midline. It was seen passing through the small bowel at multiple places and then coursing behind the bladder, enteri

Dermatoses misdiagnosed as deliberate injuries.

Examination of non-accidental injuries on a body should be done with caution, as often skin diseases can mimic deliberate injuries. Patients with active dermatoses or their residual/post-inflammatory spots seen during autopsy can generate false alarms or suspicion of burns, child abuse or battering, sexual assault or other medicolegal cases. The inability to distinguish between a mechanical injury and skin disease can cause unnecessary anxiety and can complicate medicolegal investigations. Many times, an elaborate history to find out the aetiology of the lesion is either unavailable or not rel…
