HISTORY AND CLINICAL FINDINGS: A 29-year-old woman was found to have arterial hypertension (175/115 mm Hg). The 24-hour profile showed no diurnal cortisol variations with normal concentrations and 24-hour urinary cortisol was normal. 14 months later there was definite hypercortisolism with discrete Cushing signs and no amenorrhoea. She also had signs of depression.
INVESTIGATIONS: Routine laboratory tests were unremarkable. ACTH and dehydroepiandrosterone levels were reduced and there was marked hypercortisolism (600 micrograms/24h). Bone densitometry showed osteoporosis. The low- and high-do

1. In vitro short-term production of cortisol by dispersed tumour and non-tumourous adrenal cortical cells was measured with and without added angiotensin II (AII) or adrenocorticotrophin (ACTH) in adrenals removed from five patients with primary aldosteronism. 2. Aldosterone-producing adenomas (APA) were classified as angiotensin responsive (AII-R) or angiotensin unresponsive (AII-U) based on pre-operative behaviour in vivo. 3. Cortisol was produced by both tumour and cortex in vitro without stimulation, and significantly more cortisol was generated by the cortex. 4. Addition of AII significa

Primary aldosteronism (PA) is the most common form of endocrine hypertension, with a prevalence of 5-15 %. Unilateral PA accounts for approximately 30 % of PA and has been associated with excellent post-operative results. Introduction of CYP11B2 immunostain led to the histopathology of primary aldosteronism (HISTALDO) consensus and the classification into classical and non-classical histology. We retrospectively reviewed 62 consecutive adrenalectomy cases of PA from routine clinical practice, including 46 with PA-only and 16 with cortisol cosecretion (PA-C). We also examined 3 adrenals from no

A 47-year-old female presented with hypertension, hypokalaemia, low plasma renin, high plasma aldosterone and was found to have a left adrenal tumour 4 cm in diameter by computerized tomography. Detailed biochemical studies showed high plasma levels of 11-deoxycorticosterone and corticosterone in addition to aldosterone and 18-hydroxycorticosterone. Basal 11-deoxycorticosterone levels were particularly high. Corticosterone, 18-hydroxycorticosterone and aldosterone concentrations were abnormally sensitive to infusions of ACTH and angiotensin II. Plasma cortisol and assays for sex hormones were

A hypertensive woman in her mid-30s presented with flank pain probably secondary to a left adrenal mass. Biochemical evaluation, including serum cortisol, dehydroepiandrosterone and 24-hour urinary metanephrines, was negative, and the mass was diagnosed as a non-functioning adrenocortical tumour. Intraoperatively, tumour manipulation precipitated abrupt hypertensive surges with bradycardia, necessitating nitroglycerine and sodium nitroprusside infusion. Following adrenal vein ligation, profound hypotension ensued, requiring transient noradrenaline infusion to maintain adequate mean arterial pr

OBJECTIVE: The indication for laparoscopic partial adrenalectomy (LPA) in patients with primary aldosteronism due to aldosterone-producing adenoma (APA) remains controversial. This study aimed to determine the functional and surgical outcomes of LPA in this context.
METHODS: This is a systematic review and meta-analysis. MEDLINE and Embase were searched until May 2024. Biochemical and clinical outcomes were defined according to the PASO criteria.
RESULTS: A total of 3515 articles were initially identified, and eventually 20 studies (8 comparative and 12 single-arm) were included. The pooled

Adrenal lesions (ALs) are often detected in patients with multiple endocrine neoplasia type 1 (MEN1). However, they are not well described in MEN1, making their clinical management unclear. This study examined the prevalence and outcomes of ALs found in MEN1. We performed a retrospective chart review of patients diagnosed with MEN1 from 1990 to 2021. ALs were diagnosed using abdominal or thoracic imaging and classified as being unilateral or bilateral, having single or multiple nodules, and as having diffuse enlargement or not. Measurable nodular lesions were analyzed for their size and growth

BACKGROUND: Many adrenal incidentalomas do not undergo appropriate biochemical testing and complete imaging characterization to assess for hormone hypersecretion and malignancy. With the growing availability of clinical narratives in the electronic medical record, automated surveillance using advanced data analytic techniques may represent a promising method to improve management.
METHODS: A data provisioning process using a series of structured query language scripts was used to abstract all chest and abdominal computed tomography and magnetic resonance imaging reports from an academic healt

A system for discriminating between adrenal adenoma and hyperplasia based on the levels of aldosterone production, plasma renin concentration, severity of electrolyte disturbances, plasma aldosterone patterns during recumbency and after assuming erect posture, and 131I-19-iodocholesterol scan has been developed. Indicated for operation are patients with adenomas whose elevated blood pressure cannot be continuously controlled with usual doses of medication and patients with documented deterioration of target organ function. Adrenalectomy has been performed 83 times in 81 patients with a diagnos

Isolated ectopic secretion of corticotropin-releasing hormone (CRH) is an exceedingly rare cause of Cushing's syndrome (CS), accounting for fewer than 1% of cases. Ectopic CS is an uncommon but potentially life-threatening condition that often necessitates urgent diagnostic evaluation and treatment. Hormonal testing may suggest a pituitary origin, complicating the diagnostic process. To date, isolated ectopic CRH production has primarily been reported in cases of medullary thyroid carcinoma, pheochromocytoma, and various neuroendocrine tumors. We present the first documented case of adrenocort

Patients with intra-cerebral metastases often receive glucocorticoids, particularly in the presence of peri-lesional vasogenic cerebral oedema. We present a case of presumptive lung carcinoma with cerebral metastases where central diabetes insipidus was unmasked after glucocorticoid administration and correction of undiagnosed central hypocortisolism.

Hirsutism is a frequent anomaly of the female hair follicles, which produces psychological problems. The pathogenesis, clinical manifestations and metabolism of the androgens are discussed. A way of diagnosis which is practicable for clinical purpose as well as for practice is described. Finally two interesting cases are demonstrated.
