BACKGROUND AND OBJECTIVES: Iatrogenic cerebral amyloid angiopathy (iCAA), likely caused by transmission of amyloid-beta by medical procedures, is a rare but distinct cerebral amyloid angiopathy (CAA) subtype, with rising numbers of patients identified globally. However, given its relatively recent recognition, data on long-term outcomes and prognosis are scarce. Therefore, we aimed to investigate the clinical disease progression in patients diagnosed with iCAA.
METHODS: After a systematic literature search for larger case series of iCAA (n ≥ 5), we included patients from 4 European countries …

Hemoperfusion, a therapeutic technique in which blood is circulated through extracorporeal adsorbents to remove toxins, faces the challenge of simultaneously enhancing the adsorption performance and hemocompatibility of hemoperfusion materials (HPMs) through a facile and eco-friendly surface engineering strategy. Herein, a one-step amyloid-mediated surface modification method is proposed to encapsulate HPMs by immersing them in a phase-transitioned bovine serum albumin (PTB) aqueous solution. The resultant amyloid-like PTB nanocoating modified activated carbon exhibited a nearly four-fold incr…

BACKGROUND: Inflammatory cerebral amyloid angiopathy (ICAA) is an atypical presentation of cerebral amyloid angiopathy (CAA), caused by an autoimmune reaction against cerebral beta-amyloid deposits. ICAA can be divided into two different entities: cerebral amyloid angiopathy-related inflammation (CAA-RI) and amyloid beta-related angiitis (ABRA).
CASE PRESENTATION: We report a histologically confirmed case of ABRA in which initial radiological findings were relatively subtle, resulting in a substantial diagnostic challenge. In this context, supportive cerebrospinal fluid findings, including el…

We report on how low-frequency Raman measurements can be used as a facile tool to investigate the supramolecular structure of amyloid fibrils. We investigate the low-frequency Raman spectra (<500 cm-1) of six different amyloid fibrils exhibiting parallel β-sheet structures prepared from amyloid-β1-40, amylin, amyloid-β25-35, and amylin20-29 peptides. We propose band assignments using a combination of semi-empirical tight-binding calculations and insights gleaned from previously published studies on model polypeptides in β-sheet conformations. We discuss how low-frequency Raman modes can be use…

Amyloid-related imaging abnormalities (ARIA) are parenchymal edema/effusion and hemorrhage that occur as side effects of immunotherapies targeting beta-amyloid for patients with Alzheimer's disease (AD). The mechanism of ARIA remains unclear but is thought to be due to loss of vascular integrity secondary to overwhelming perivascular clearance pathways, worsening of cerebral amyloid angiopathy (CAA), removal of amyloid from the vessel wall, and initiation of a local immune response. Further imaging and neuropathological studies may help further inform the mechanism of ARIA and its relationship…

Interactions between amyloid proteins represent the cornerstone of various pathogenic pathways, including prion conversion and co-development of distinct kinds of systemic amyloidosis. Various experimental methodologies provide insights into the effects of such cross-interactions on amyloid self-assembly, which range from acceleration to complete inhibition. Here, we present a comprehensive review of experimental methods most commonly used to study amyloid cross-interactions both in vitro and in vivo, such as fluorescence-based techniques, high-resolution imaging, and spectroscopic methods. Al…

Transthyretin amyloid cardiomyopathy is a progressive and fatal cardiomyopathy caused by the deposition of misfolded transthyretin in the form of amyloid fibrils in the myocardium. The advent of various highly efficacious transthyretin amyloid-specific disease-modifying therapies has sparked a growing interest in identifying the clinical indicators of disease progression that will be crucial in guiding treatment decisions. Markers of disease progression include changes in commonly measured biomarkers such as the N-terminal pro-B-type natriuretic peptide and the estimated glomerular filtration …

Transthyretin amyloid cardiomyopathy (ATTR-CM) is a progressive form of restrictive cardiomyopathy caused by the misfolding and deposition of transthyretin protein. What was once a condition that had little by way of treatment other than liver transplantation has now become a manageable disease due to revolutionary novel therapies. This review outlines therapeutic strategies aimed at halting or reversing disease progression. Current approaches include transthyretin (TTR) stabilizers such as tafamidis and acoramidis, as well as TTR silencers such as vutisiran. In addition, novel therapies under…

Transthyretin amyloid cardiomyopathy (ATTR-CM) is a progressive and life-threatening condition characterized by the deposition of misfolded transthyretin (TTR) protein in the myocardium, leading to restrictive cardiomyopathy and heart failure. Given its high mortality rate and historically limited therapeutic options, there is a growing need for effective treatments. Current therapeutic strategies focus on reducing the production of TTR using RNA-targeted therapies and stabilizing the native TTR tetramer to prevent misfolding and aggregation. In recent years, emerging therapies-including monoc…

Alzheimer's disease (AD) is pathologically characterized by deposition of amyloid plaques (comprising amyloid β [Aβ] protein) and neurofibrillary tangles (comprising tau protein), and neuronal death. Aβ monomers aggregate to form oligomers, protofibrils, and mature fibrils. Previously, the mature fibrils and plaques were implicated as contributors to neurotoxicity and neurodegeneration. However, a growing body of evidence proves stronger toxicity of oligomers and protofibrils. Among the many recent phase 3 clinical trials that have investigated the role of anti-Aβ antibodies in AD, some have s…

Recent efforts demonstrated the efficacy of identifying early-stage neuropathology of Alzheimer's disease (AD) through lumbar puncture cerebrospinal fluid assessment and positron emission tomography (PET) radiotracer imaging. These methods are effective yet are invasive, expensive, and not widely accessible. We extend and improve the multiscale structural mapping (MSSM) procedure to develop structural indicators of β-amyloid neuropathology in preclinical AD, by capturing both macrostructural and microstructural properties throughout the cerebral cortex using a structural MRI. We find that the …

Amyloid plaques are a neuropathologic hallmark of Alzheimer's disease (AD), which can be imaged through positron emission tomography (PET) technology using radiopharmaceuticals that selectively bind to the fibrillar aggregates of amyloid-β plaques (Amy-PET). Several radiotracers for amyloid PET have been validated (11C-Pittsburgh compound B and the 18F-labeled compounds such as 18F-florbetaben, 18F-florbetapir, and 18F-flutemetamol). Images can be interpreted by means of visual/qualitative, semiquantitative, and quantitative criteria. Here, we summarize the main differences between the availab…

Recent advances in clinical passive immunotherapy have provided compelling evidence that eliminating amyloid-β (Aβ) slows cognitive decline in Alzheimer's disease (AD). However, the modest benefits and side effects observed in clinical trials indicate that current immunotherapy therapy is not a panacea, highlighting the need for a deeper understanding of AD mechanisms and the significance of early intervention through optimized immunotherapy or immunoprevention. This review focuses on the centrality of Aβ pathology in AD and summarizes recent clinical progress in passive and active immunothera…

INTRODUCTION: In amyloid-positive individuals, disease-related biomarker heterogeneity is understudied.
METHODS: We used Subtype and Stage Inference (SuStaIn) to identify data-driven subtypes among cerebrospinal fluid (CSF) amyloid beta (1-42)-positive individuals from the Alzheimer's Disease Neuroimaging Initiative (ADNIGO/2 [n = 376]). Variables included: CSF phosphorylated tau (p-tau181), hippocampal and whole-brain volume, logical memory (LM), composite Trail Making Test score, and white matter hyperintensity (WMH) volumes. CSF amyloid-negative, apolipoprotein E ε4 non-carrier cognitively…

Proximity-induced pharmacology (PIP) for amyloid-related diseases is a cutting-edge approach to treating conditions such as Alzheimer's disease and other forms of dementia. By bringing small molecules close to amyloid-related proteins, these molecules can induce a plethora of effects that can break down pathogenic proteins and reduce the buildup of plaques. One of the most promising aspects of this drug discovery modality is that it can be used to target specific types of amyloid proteins, such as the beta-amyloid protein that is commonly associated with Alzheimer's disease. This level of spec…

Alzheimer's disease (AD) is a devastating neurodegenerative disease that affects ∼50 million people globally. The accumulation of amyloid-β (Aβ) plaques, a predominant pathological feature of AD, plays a crucial role in AD pathogenesis. In this respect, Aβ has been regarded as a highly promising therapeutic target for AD treatment. Polyoxometalates (POMs) are a novel class of metallodrugs being developed as modulators of Aβ aggregation, owing to their negative charge, polarity, and three-dimensional structure. Unlike traditional discrete inorganic complexes, POMs contain tens to hundreds of me…

Alzheimer's disease (AD) is a major cause of dementia debilitating the global ageing population. Current understanding of the AD pathophysiology implicates the aggregation of amyloid beta (Aβ) as causative to neurodegeneration, with tauopathies, apolipoprotein E and neuroinflammation considered as other major culprits. Curiously, vascular endothelial barrier dysfunction is strongly associated with Aβ deposition and 80-90% AD subjects also experience cerebral amyloid angiopathy. Here we show amyloid protein-induced endothelial leakiness (APEL) in human microvascular endothelial monolayers as we…
