Aortic occlusion requiring surgery in a patient with autoimmune hemolytic anemia and severe anemia: a case report.

INTRODUCTION: Aortoiliac occlusive disease (AIOD) often requires urgent revascularization to prevent chronic limb-threatening ischemia (CLTI). The coexistence of autoimmune hemolytic anemia (AIHA) complicates surgical planning due to increased transfusion risks and unstable hemoglobin levels. Understanding the interplay between AIHA and AIOD is crucial, as delayed intervention can lead to irreversible ischemic damage. PRESENTATION OF CASE: A 72-year-old man presented with leg pain and bilateral lower-limb weakness. His medical history included type 2 diabetes mellitus and suspected AIHA. Phys…

Investigation of hemorheological parameters at the diagnosis and follow up of children with iron deficiency anemia and mixed anemia.

OBJECTIVE: We aimed to investigate the effects of iron deficiency anemia (IDA) and vitamin B12 deficiency coexisting with IDA which is called as mixed anemia (MA) on hemorheological parameters, to compare them with each other and healthy controls, and to assess the changes in hemorheological parameters after treatment. MATERIALS AND METHODS: 32 IDA patients (mean age:6.3 ± 5.3 years), 30 MA patients (mean age:7.2 ± 5.4 years), and 31 healthy controls (mean age:7.1 ± 5.2 years) were enrolled. Erythrocyte deformability and aggregation were determined by an ectacytometer, plasma and whole blood …

[Anemia and growth in Congolese adolescents with homozygous sickle cell anemia at Central University Hospital of Brazzaville].

The rate of hemoglobin of 71 homozygous sickle cell disease patients from 10 to 18 years was studied, as well as the mean cell volume for 61 of them. Their height, weight and bone age were compared. The rate of hemoglobin in steady state is 6.5 g/dl +/- 2.3 for girls against 7.9 g/dl +/- 1.5 for boys. There's no significant difference between the rate of hemoglobin on sexual maturation state for girls. But the boys from 14 to 18 years old with signs of sexual maturation had higher rate of hemoglobin than those had not (8.5 g/dl +/- 1.3 against 6.3 g/dl +/- 1.5). Eighteen per cent of patients w

Non-immune causes of fetal anemia.

Non-immune fetal anemia is a major cause of perinatal morbidity and mortality, affecting 3-5 per 10,000 pregnancies. It results from a range of conditions, including genetic hemoglobinopathies, congenital infections, fetomaternal hemorrhage, and twin-related complications. Advances in ultrasound-based screening, particularly middle cerebral artery peak systolic velocity measurement, have improved early detection, with a sensitivity exceeding 90% for detecting moderate to severe anemia in high-risk population. Intrauterine transfusion remains the primary treatment, with survival rates exceeding…

Iron Deficiency Anemia in Asthmatic Children.

This letter responds to the article "The Effect of Iron Deficiency Anemia on Emergency Department Admission in Asthmatic Children" by Selmanoglu et al. highlighting important limitations in their findings. The authors' retrospective study identified a significant association between iron deficiency anemia (IDA) and increased emergency department admissions for asthma exacerbations in asthmatic children. However, the letter emphasizes the need to consider potential confounding factors such as socioeconomic status and nutritional intake, which may influence both anemia and asthma severity. Addit…

[Practice-oriented rational assessment of anemia].

Anemia is frequently diagnosed in the daily clinical routine and is an indicator of a more or less severe underlying disorder. An understanding of the diverse and complex etiology is crucial for a practice-oriented diagnostic approach. In principle, there are many differential diagnoses. The diagnosis is further complicated by the fact that anemia is often multifactorial, especially in old age. A rational clarification of anemia takes epidemiological data and the individual medical history into account. Classification according to erythrocyte indices plays a central role. The most important ca…

[Current therapeutic strategies for aplastic anemia].

Aplastic anemia is a bone marrow failure syndrome characterized by bone marrow hypoplasia and pancytopenia, often involving a T cell-mediated autoimmune mechanism. Treatment modalities aimed at hematopoietic recovery include immunosuppressive therapy, thrombopoietin receptor agonists (TPO-RA), and allogeneic hematopoietic stem cell transplantation (allo-SCT). The combination of TPO-RA with conventional immunosuppressive therapy using anti-thymocyte globulin and cyclosporine has become a standard treatment option. In addition to improved outcomes with allo-SCT from HLA-matched sibling donors, f…

Iron Deficiency Anemia: Evaluation and Management.

Iron deficiency anemia is common worldwide. In adult patients without inflammation, a ferritin level of less than 45 ng/mL or ferritin level of 46 to 99 ng/mL plus a transferrin saturation of less than 20% is diagnostic of iron deficiency. In patients with inflammation, a ferritin level of less than 100 ng/mL is diagnostic. Risk factors for iron deficiency anemia include low socioeconomic status, female sex, age younger than 5 years, and chronic inflammation. Underlying causes should be investigated. Recurrent blood loss is responsible for 94% of cases. In younger patients with a plausible cau…

Severe Pernicious Anemia: A Case Report.

Pernicious anemia is an infrequent condition and a recognized cause of vitamin B12 deficiency. It is an autoimmune disease that impairs the absorption of vitamin B12, leading to a significant reduction in its uptake. Clinical manifestations are often subtle and non-specific, yet potentially debilitating, which may delay diagnosis. However, with early recognition in primary care and appropriate replacement therapy, patients can recover their quality of life. Fatigue and asthenia are common complaints in primary care, highlighting the importance of careful clinical evaluation in the presence of …

[How I explore… anemia in children].

Anemia is a common problem in pediatrics. The most frequent cause is iron deficiency, but it can also be associated to a constitutional or acquired pathology of the bone marrow or red blood cells. We describe a practical approach for rapidly guiding the diagnosis and management of anemia in children. It is based on the history and clinical examination, mean corpuscular volume, ferritinemia, reticulocytosis and hemolytic profile.

Iron Deficiency Anemia: An Updated Review.

BACKGROUND: Worldwide, iron deficiency anemia is the most prevalent nutritional deficiency disorder and the leading cause of anemia in children, especially in developing countries. When present in early childhood, especially if severe and prolonged, iron deficiency anemia can result in neurodevelopmental and cognitive deficits, which may not always be fully reversible even following the correction of iron deficiency anemia. OBJECTIVE: This article aimed to familiarize physicians with the clinical manifestations, diagnosis, evaluation, prevention, and management of children with iron deficienc…

Polycythemia Vera Masked by Megaloblastic Anemia.

Polycythemia vera (PV) is a chronic myeloproliferative disorder characterized by uncontrolled red blood cell production. Megaloblastic anemia is caused by deficiency of cobalamin (vitamin B12) and/or folate (vitamin B9). While B12 deficiency may be caused by insufficient dietary intake or impairment of its utilization, its association with PV is described without exact knowledge of the physiopathology. We herein report the occurrence of megaloblastic anemia due to Vitamin B12 deficiency in an 85-year-old North African woman patient with PV. This case highlights this atypical presentation of PV…

Herpes Simplex Virus-Associated Aplastic Anemia.

Aplastic anemia is an uncommon condition defined by peripheral pancytopenia in the context of hypocellular bone marrow. In the majority of cases, it is idiopathic in origin. However, exposure to certain drugs and toxins, autoimmune processes, and viral infections have been linked to this entity. This is the case of a 56-year-old female with an acute presentation of fever, odynophagia, and dysphagia. Physical examination revealed multiple hemorrhagic ulcers affecting her oropharyngeal mucosa with regions of necrosis. Mucosal biopsy was compatible with the presence of local necrosis and keratini…

Fanconi Anemia: A Rare Genetic Disorder.

Fanconi anemia is a rare genetic disorder affecting various body systems. Congenital abnormalities, poor hematopoiesis, a higher incidence of acute myeloid leukemia, myelodysplastic syndrome, and malignancies are the hallmarks of this autosomal recessive condition. In certain instances, the clinical signs and highly diverse phenotypic presentation make a diagnosis challenging. In this case report, an eight-year-old boy presented with recurrent episodes of fever, generalized weakness and physical deformities. He had left thumb deformity, triangular face, short stature, and hyperpigmentation wit…

Does placental abruption cause neonatal anemia?

INTRODUCTION: Placental abruption can result in serious perinatal morbidity and mortality. However, it is not clear whether placental abruption could lead to neonatal anemia, as a direct relation has not been described yet. The objective of this study is to investigate whether there is a relation between occurrence of placental abruption and neonatal anemia. MATERIAL AND METHODS: All women with a clinical diagnoses of placental abruption between January 2016 and April 2021 in Amsterdam UMC, from both the VU University Medical Center and Amsterdam Medical Center, were included. Demographic dat…

[Somatic Mutations of Acquired Aplastic Anemia].

Aplastic anemia (AA) and myelodysplastic syndrome (MDS) are clonal diseases with hemopoietic stem cell (HSC) abnormalities, which are sometimes difficult to be distinguished from each other. The development of molecular detection techniques has facilitated our understanding of the molecular pathogenesis of the two diseases. This article reviewed the somatic mutation (SM) and cytogenetic changes of AA,and analyzed their molecular relationship with MDS and their roles in disease transformation. The most common somatic change in AA is the loss of PIGA and HLA alleles, which, along with trisomy 8 and de…

How I investigate acquired megaloblastic anemia.

In this review of megaloblastic anemia (MA), an overview of vitamin B12 and folate body requirements, biochemical pathways, and laboratory testing strategies will be provided. However, the focus of this review is the classic and unique features of MA in blood and bone marrow. Acquired MA is a benign disorder for many, but can be detrimental for some. The clinical presentation can vary considerably, and the spectrum of symptoms and signs is diverse and quite broad. Prompt recognition and therapy are critical to prevent potential irreversible damage and clinical sequelae, especially in patients …
