Dataset for carbon footprints of transforaminal lumbar interbody fusions performed under spinal or general anesthesia.

The datasets presented here quantify and compare the relative carbon footprints emitted by general versus spinal anesthesia in patients undergoing single-level transforaminal lumbar interbody fusions (TLIFs). Data were retrospectively collected from electronic medical records of 100 consecutive patients who underwent a single-level TLIF from a single neurosurgeon at a U.S. academic center. 50 patients were under general anesthesia, and another 50 patients were under spinal anesthesia. Clinic and operative notes were used to extract demographic and surgical information, whereas anesthesia recor

Intracranial rebleeding post spinal anesthesia in pregnant patient with undiagnosed chronic subdural hematoma: case report.

Intracranial hematoma after spinal anesthesia is a rare complication. It generally presents with posture-dependent headache that becomes persistent. We describe the case of patient submitted to spinal anesthesia for cesarean section who presented a non-posture-dependent headache, resistant to clinical treatment, that progressively worsened and with symptoms of intracranial hypertension. The patient had a history of head trauma without symptoms. The CT-scan revealed a chronic bilateral parietal hematoma with a recent bleeding component, treated surgically. We concluded that spinal puncture led

Hemodynamic effects of low-dose bupivacaine spinal anesthesia for cesarean section: A randomized controlled trial.

BACKGROUND: Spinal anesthesia is the most common technique for cesarean section. The conventional local anesthetic dose has been decreasing over time to 8-12.5 mg of bupivacaine. Lower doses of bupivacaine may be associated with reduced incidence of hypotension and other complications. This low dose also may be associated with improved maternal cardiac index (CI). We hypothesized that low dose spinal anesthesia using 4.5 mg bupivacaine would result in improved maternal CI when compared with conventional dose (9 mg) intrathecal bupivacaine. METHODS: This randomized controlled trial included al

Catastrophic thoracolumbar spinal massive hematoma triggered by intraspinal anesthesia puncture: A CARE-compliant case report.

RATIONALE: Intraspinal anesthesia, the most common anesthesia type of orthopedic operation, is regarded as safe and simple. Despite of the rare incidence, puncture related complication of intraspinal anesthesia is catastrophic for spinal cord. Here we present an intradural hematoma case triggered by improper anesthesia puncture. The principal reason of this tragedy was rooted in the neglect of spine deformities diagnosis before anesthesia. To the best of our knowledge, there is no specific case report focusing on the intradural hematoma triggered by improper anesthesia puncture. PATIENT CONCE

Prophylactic granisetron for post-spinal anesthesia shivering in cesarean section: A randomized controlled clinical study.

BACKGROUND: The serotonergic system is known to be involved in control of post-anesthetic shivering. Our hypothesis was that prophylactic granisetrone (serotonin antagonist) might reduce incidence of post-spinal anesthesia shivering in cesarean section. METHODS: Parturient scheduled for elective cesarean delivery under spinal anesthesia were allocated to receive 0.9% saline (Group I, n = 71), 1 mg granisetron (Group II, n = 69), or 0.7 mg granisetron (Group III, n = 72) before the spinal block. Assessment parameters included; hemodynamics, tympanic membrane temperature, neonatal Apgar score,

Effect of Posture on Post Lumbar Puncture Headache after Spinal Anesthesia: A Prospective Randomized Study.

Background Headache after lumbar puncture has long been attributed to early mobilization, and hence prophylactic bed rest had been standard protocol to prevent spinal headache after lumbar puncture. However, trend has been changing towards early mobilization to no need of bed rest at all after lumbar puncture. Objective To study the influence of posture in the incidence of post lumbar puncture headache in patients undergoing spinal anesthesia. Method In a prospective randomized study, patients undergoing various surgical procedures under spinal anesthesia from February 2013 to January 2014 wer

Duration of spinal anesthesia with bupivacaine in chronic opium abusers undergoing lower extremity orthopedic surgery.

BACKGROUND: It has been demonstrated that chronic opium abusers have lower thresholds for pain. Spinal anesthesia is a common procedure in anesthesia, which is performed through administration of drugs (usually local anesthetics) in the intrathecal space, to produce temporary pain relief. The aim of this study was to determine whether chronic opium abuse could have any possible effect on the duration of spinal block by bupivacaine. METHODS: In a case-control study, 50 opium abusers and 50 nonabusers undergoing lower extremity orthopedic operations were selected from the patients admitted in T

Divergence paresis without positional headache: an unusual presentation of cerebrospinal fluid hypovolemia after spinal anesthesia.

We report a rare complication of spinal anesthesia-divergence paresis-which is characterized by an acquired horizontal diplopia at distance without evidence for abducens palsy. A 64-yr-old man underwent prostatectomy under spinal anesthesia with 2.5 mL of dibucaine hydrochloride 0.3% injected through a 20-gauge cutting-tip spinal needle. Seventeen days after the operation, the patient noticed horizontal diplopia for distant objects. Although cranial magnetic resonance imaging demonstrated diffuse pachymeningeal gadolinium enhancement and subdural effusion, characteristic findings of cerebrospi

Colloid vs. crystalloid preloading to prevent maternal hypotension during spinal anesthesia for elective cesarean section.

BACKGROUND: Hypotension associated with spinal anesthesia for cesarean section is still a clinical problem. Colloid solutions seem preferable to crystalloid solutions for preloading. In most studies the overall rate of hypotension is reported. Few studies have, however, investigated the maternal and neonatal consequences of different levels of maternal hypotension. METHODS: In this randomized, double-blinded study 110 patients presenting for elective cesarean section received either 1000 ml acetated Ringer's solution or 1000 ml 3% dextran 60 solution immediately before spinal anesthesia. Ambulatory surgery patients may be discharged before voiding after short-acting spinal and epidural anesthesia.

BACKGROUND: Voiding before discharge is usually required after outpatient epidural or spinal anesthesia because of concern about bladder overdistention and dysfunction. Shorter duration spinal and epidural anesthesia may allow return of bladder function before overdistention occurs in low-risk patients (those younger than age 70, not having hernia, rectal, or urologic surgery, and without a history of voiding difficulty), and predischarge voiding may not be necessary. METHODS: After institutional review board approval and informed consent, 201 low-risk ambulatory patients were prospectively s

Over-the-needle versus microcatheter-through-needle technique for continuous spinal anesthesia: a preliminary study.

BACKGROUND AND OBJECTIVES: A new catheter-over-needle design (Spinocath, B. Braun) has been developed to minimize problems and complications of continuous spinal anesthesia with microcatheters, which include difficult catheter insertion, failure of insertion, breakage, inadequate anesthesia, postdural puncture headache, and, rarely, development of cauda equina syndrome. METHODS: A comparative evaluation of this new catheter (22-gauge Spinocath) to the microcatheter (28-gauge Portex) was performed to study the problems of insertion, complications, quality of anesthesia, and postoperative analg

[Micturition disorders following spinal anesthesia of different durations of action (lidocaine 2% versus bupivacaine 0.5%)].

Disturbances of micturition following spinal anaesthesia are considered to be rare and harmless side effects of this technique. For this reason, we set up a prospective study to investigate their incidence, characteristics and intensity. Our special interest was directed at the influence of the duration of action of local anaesthetics. METHODS. In a randomized, double-blind study, two groups, each consisting of 73 trauma surgical and orthopaedic patients, received isobaric spinal anaesthesia with either lidocaine 2% or bupivacaine 0.5%. From the 1st to the 3rd postoperative day, the patients w

Possible correlation between Mycobacterium abscessus subsp. massiliense spondylodiscitis and spinal anesthesia: Diagnostic and therapeutic challenges.

Spinal infections caused by Mycobacterium abscessus are rare and difficult to treat. We report the first case of M. abscessus subsp. massiliense spondylodiscitis temporally associated with spinal anesthesia. A 57-year-old man developed back pain after undergoing spinal anesthesia for prostate surgery. Six months later, decompressive laminectomy was performed; results for bacterial and fungal culture and molecular testing for Mycobacterium tuberculosis were negative. Two years later, a mycobacterial culture of material from a paravertebral abscess revealed the presence of M. abscessus, later id

Utility of real-time ultrasound-guided spinal anesthesia in severe ankylosing spondylitis - A case report.

BACKGROUND: Ankylosing spondylitis (AS) distorts the neuraxial anatomy and can complicate airway management. However, patients with severe AS are often considered unsuitable for spinal anesthesia, and attempts are typically avoided. We report a case of successful real-time ultrasound-guided spinal anesthesia in a patient with severe AS. CASE: A 73-year-old man with severe AS and restrictive lung impairment presented with a femoral neck fracture and was scheduled for bipolar hemiarthroplasty. Cervical flexion deformity and whole spine ankylosis predicted difficult airway and hindered spinal an

Cauda equina syndrome in an obese parturient following cesarean section under combined spinal-epidural anesthesia.

Cauda equine syndrome (CES) is the very rare complication especially after uneventful neuraxial anesthesia. A woman underwent a combined spinal-epidural anesthesia for cesarean section. Postoperatively 24 hours, she complained of paresthesia of the left leg, urinary retention, and saddle anesthesia. After checking magnetic resonance imaging (MRI), CES was suspicious and medical treatment was initiated. During the 18-month follow-up, though perineal hypoesthesia and paresthesia of the left leg were recovered, urinary and fecal incontinence remained. This case demonstrates that CES may occur eve

A Methodical Needle-Over-Needle Guidance (NNG) Approach for Spinal Anesthesia Using a "Super Introducer".

Post-dural puncture headache (PDPH) often occurs after lumbar puncture during spinal anesthesia, and younger patients tend to be more susceptible to this complication. In this case report, we present a needle-over-needle guidance (NNG) technique utilizing a larger gauge (20G) 3.5" regular spinal needle, referred to as the "Super Introducer," to guide a longer, smaller gauge (25G) 4.7" spinal needle toward the dura in the midline. The "Super Introducer," which is longer than the standard introducer needles found in spinal kits, allows for deeper engagement with the interspinous ligament and pro
