Total spinal block after local anesthetic administration through the wrong access port of a spinal infusion pump.

We present a case reported on the SENSAR database. A patient with a spinal infusion pump was admitted for reservoir refill. On administration of 22ml of 0.75% bupivacaine the patient suffered a total spinal block with widespread loss strength and respiratory arrest. The patient required emergency orotracheal intubation, mechanical ventilation and admission to ICU, where extubation was achieved within two hours without incidences. At a later stage it was stated that the local anaesthetic had been administered via the access port for bolus or contrast administration instead of via the access to

Local anesthetic injection deep to the fascia iliaca at the level of the inguinal ligament: the pattern of distribution and effects on the obturator nerve.

STUDY OBJECTIVE: The femoral, lateral femoral cutaneous, and obturator nerves (ONs) can reportedly be blocked using a single-injection deep to the fascia iliaca (FI) at the level of the inguinal ligament. Two commonly used methods (the FI compartment and 3-in-1 blocks) have produced inconsistent results with respect to local anesthetic distribution and effect on the ON. To date, no study of either method has been performed using advanced imaging techniques to document both needle placement and local anesthetic distribution. We report the outcome of a series of 3-in-1 and FI blocks performed us

A prospective study on pain score with transperineal prostatic gold seed fiducial implantation under local anesthetic alone.

BACKGROUND: The purpose of this study was to monitor patient pain score with transperineal prostatic gold seed implantation in the absence of conscious sedation. METHODS: All patients who were scheduled for image-guided external beam radiation (IGRT) and referred for gold seed fiducials were eligible to participate. Gold seed implants were performed by two radiation oncologists between December 2007 and April 2008. Patients received only local and deep anesthetic. No patients had prophylactic IV cannulation for the procedure. Three gold seeds were inserted transperineally into the prostate. A

Ultrasound-guided evaluation of the local anesthetic spread parameters required for a rapid surgical popliteal sciatic nerve block.

BACKGROUND: The ideal spread of local anesthetic (LA) solution around the sciatic nerve during a popliteal block remains unclear. We tested the hypothesis that a circumferential spread of LA and/or intraneural injection could lead to rapid surgical block. METHODS: Patients (n = 100) scheduled for foot or ankle surgery underwent popliteal sciatic nerve block using nerve stimulation according to Borgeat's technique and injection of ropivacaine (0.5 mL/kg). Sensory and motor blockades were assessed on the tibial nerve (TN) and common peroneal nerve (CPN) at 5, 15, and 30 mins after completion of

Study to assess the laryngeal and pharyngeal spread of topical local anesthetic administered orally during general anesthesia in children.

BACKGROUND: Topical local anesthesia of the airway of anaesthetized children has many potential benefits. In our institution, lignocaine is topically instilled blindly into the back of the mouth with the expectation that it will come into contact with the larynx. The volume and method of application varies between clinicians. There is no published evidence to support the plausibility of this technique. AIM: To determine whether this technique of instillation results in the local anesthetic coming into contact with key laryngeal structures and whether this is influenced by volume or additional

Ultrasound-guided perineural catheter and local anesthetic infusion in the perioperative management of pediatric limb salvage: a case report.

Local anesthetic perineural infusion has emerged as an effective analgesic technique in pediatric patients, but it can also complement surgical perioperative management in complex cases that require optimal tissue perfusion such as limb salvage. We report the successful use of brachial plexus perineural infusion in the care of a child following near-amputation of her dominant hand. An ultrasound-guided infraclavicular brachial plexus catheter was placed in the recovery room after complex reconstructive surgery and a continuous infusion of 0.2% ropivacaine maintained for 24 days. The resultant

Acute toxicity of local anesthetic ropivacaine and mepivacaine during a combined lumbar plexus and sciatic block for hip surgery.

Hip fracture is a common pathology in elderly patients. Intercurrent diseases, mainly cardiac and respiratory, often result in significant morbidity and mortality. Anesthesia for hip fracture can be provided by general or regional techniques. The combination of a lumbar plexus and posterior sciatic nerve block represents an alternative to neuraxial technique of anaesthesia such as spinal anesthesia (4, 6). We report a case of acute toxicity resulting in the injection of local anesthetics Ropivacaine and Mepivacaine in elderly patient. An elderly woman was scheduled for surgical repair of a fra

Histologic response to intraligament injections using a computerized local anesthetic delivery system. A pilot study in mini-swine.

BACKGROUND: Intraligamentary (periodontal ligament) injection has been used to locally control pain with minimal anesthetic dose. The purpose of the present study was to determine the histologic effect of using a computer-controlled local anesthesia delivery system (CCLADS) for the administration of a periodontal ligament injection in the mini-swine model system. METHODS: Thirty-two sites in 3 mini-swine pigs with healthy periodontium were selected. Each site was notched on the mesial-lingual line angle of the tooth to allow a reference for needle placement and provide a histologic marker. An

Contrasting membrane localization and behavior of halogenated cyclobutanes that follow or violate the Meyer-Overton hypothesis of general anesthetic potency.

The membrane localization and properties of two halogenated cyclobutanes were examined using 2H and 19F NMR. The common predictors of potency indicate that these two compounds will have anesthetic activity; however, 1,2-dichlorohexafluorocyclobutane (c(CCIFCCIFCF2CF2)) is not an effective anesthetic, whereas 1-chloro-1,2,2-trifluorocyclobutane (c(CCIFCF2CH2CH2)) is an effective general anesthetic. Using 2H NMR, the effect of these compounds on the acyl chain packing in palmitoyl (d31) oleoylphosphatidylcholine membranes was examined. The addition of the anesthetic c(CCIFCF2CH2CH2) results in s

Comparison between bupivacaine liposome injectable suspension and ropivacaine as local anesthetics for transversus abdominis plane block in postoperative pain management after laparoscopic sleeve gastrectomy: a randomized controlled trial.

BACKGROUND: Postoperative pain management is critical for optimizing enhanced recovery after surgery (ERAS) in laparoscopic sleeve gastrectomy (LSG), where transversus abdominis plane block (TAPB) is a standard analgesic technique. Ropivacaine, the most commonly used local anesthetic for TAPB in LSG, has a short duration of action (4-6 h), which may be insufficient to control pain for up to 72 h and increase opioid dependence. Bupivacaine liposome injectable suspension (BLIS) provides extended analgesia (24-72 h) but has not been rigorously compared to ropivacaine in LSG-specific TAPB regardin

Evaluating the Efficacy of Dexmedetomidine as an Adjunct to Local Anesthetics in Adductor Canal Blocks for Adult Patients Undergoing Primary Total Knee Arthroplasty: A Systematic Review and Meta-Analysis.

Over 700,000 total knee arthroplasties (TKAs) are performed in the United States annually. Long-term functional recovery following TKA depends heavily on early ambulation and rehabilitation, which requires effective pain management. Adductor canal blocks (ACBs) offer targeted analgesia while preserving quadriceps strength. However, postoperative pain often outlasts single-shot peripheral nerve blocks. This analysis evaluated whether perineural dexmedetomidine added to local anesthetics increased the efficacy and duration of ACBs in patients undergoing TKA. This systematic review and meta-analy
