Nasal polyps - histopathologic spectrum.

Analysis of 345 polypoidal masses in nose and nasal sinuses with clinical diagnosis of nasal polyp, observed in 10 years, revealed 175 (50.7%) non- neoplastic lesions and 170 (49.3%) neoplasms. Among the non- neoplastic lesions, there were 110 cases (62.8%) of true nasal polyps including 74 cases (67.3%) of allergic polyps and 36 (32.7%) inflammatory ones. The next common non- neoplastic polyp was due to rhinosporidiosis (31.4%). Benign neoplastic lesions consisted mainly of haemangioma (45.7%), angiofibroma (23.2%), fibroma (6.2%), transitional cell papilloma, inverted papilloma, adenoma-3.9%…

Surgical therapy of internal carotid artery lesions of the skull base and temporal bone.

Five cases of carotid artery lesions at the skull base or temporal bone, treated surgically, are presented. These lesions include mycotic aneurysms and carotid stenosis secondary to angiofibroma and a glomus caroticum. One case demonstrated an anomalous carotid anatomic pattern with a persistent stapedial artery. A description is provided of the technique of subtotal petrosectomy with permanent anterior displacement of the facial nerve, and middle ear obliteration, thus achieving a safe exposure of the temporal course of the internal carotid artery. The cases indicate that cooperation between …

The effects of tying the neurovascular bundle during palatoplasty.

Controversy rages in the literature over the possibility of denervation and atrophy of the palate that may occur secondary to severance of the greater palatine neurovascular bundles during palatoplasty. A pilot study was undertaken to assess palatal bulk, mobility, sensation, and mucous gland population, as well as speech, in a group of cleft palate patients, some of whom had severance of the bundles during their palatal repair and some who did not. These same parameters were tested in a group of patients who had juvenile nasopharyngeal angiofibromata, in whom one bundle was tied and the other…

Folliculocystic and Collagen Hamartoma: A Subset of Fibrous Cephalic Plaque.

Tuberous sclerosis complex is known to cause a variety of cutaneous hamartomas, most commonly hypomelanotic macules, angiofibromas, shagreen patches, and fibrous cephalic plaques. In recent years, a new cutaneous hamartoma that bears physical and histological resemblance to fibrous cephalic plaque has been proposed called folliculocystic and collagen hamartoma. The primary difference between the two diagnoses is the histologic presence of infundibular cysts in the latter. However, some authors have called into question if the two diagnoses are truly distinct. In this case report, we present a …

Tuberous sclerosis: a rare cause of seizure in Nigeria.

BACKGROUND: Tuberous sclerosis is a rare genetic multisystem disorder that is typically apparent shortly after birth. Dermatologic manifestations may be the only clues to the diagnosis of the disorder, which is also marked by childhood seizures and mental retardation. The aim of this report is to present a twelve year old boy with tuberous sclerosis. METHODS: A review of the case records of a child with angiofibromas of the face and neck and the relevant literature. RESULTS: An eleven year old primary two pupil of Ijaw tribe, southern Nigeria, presented with recurrent afebrile, generalized t…

Tuberous sclerosis complex: the past and the future.

Renal lesions represent the second most significant cause of morbidity and mortality in patients with tuberous sclerosis complex (TSC). Recent advances in the understanding of the pathophysiology of TSC have led to the exploration of new potential therapeutic targets. Clinical trials with mammalian target of rapamycin (mTOR) inhibitors have demonstrated promising results for several indications, such as renal angiomyolipoma, subependymal giant cell astrocytoma, lymphangioleiomyomatosis and facial angiofibromas. Currently, there is a scarcity of natural history data and randomized, placebo-cont…

Oral cavity and pharynx.

Imaging of the oral cavity and pharynx often is required in three settings: assessment of an inflammatory mass in association with odontogenic, tonsillar, or pharyngeal infections; determination of the cause of a submucosal mass; and staging of squamous-cell carcinomas. Spread of infection from the oral cavity and pharynx can lead to abscesses in the masticatory space, the retropharyngeal compartment, and in a parapharyngeal location. Submucosal masses include congenital cysts (thyroglossal and dermoid), benign neoplasms (hemangioma, schwannomas, pleomorphic adenomas juvenile angiofibromas), i…

Massive angiomyofibroblastoma of the glans penis.

Male genital tract angiomyofibroblastoma (AMF) is a rare benign tumour, with a total of 34 cases reported in literature. We are presenting a case of AMF of the glans penis in a 68-year-old man who presented with a progressively increasing in size large lesion located on the tip of his penis. Following routine investigations, the lesion was surgically excised with no adjuvant treatment, the patient was followed-up for 5 years with no evidence of local, nodal or distant recurrence. As AMF of the glans penis is extremely rare, there is not enough literature to support management guide lines, but

Angiomyofibroblastoma-like tumor of the scrotum: a case report and review of literature.

Angiomyofibroblastoma-like tumor is a rare mesenchymal tumor of men. It commonly occurs during the fifth to eighth decades of life and mainly involves the inguinoscrotal region. It is derived from perivascular stem cells and has capacity of lipoid and myofibroblastic differentiation. Histopathologically this tumor in the male genitalia mimics female angiomyofibroblastoma but there are morphological and immunohistochemical differences between these lesions. We report a case of an angiomyofibroblastoma-like tumor that arose in the scrotal region in a 40-year-old man.

Subependymal giant-cell astrocytoma associated with tuberous sclerosis: case report.

Tuberous sclerosis is a neurocutaneous syndrome with a wide variety of clinical, pathologic, and radiologic manifestations. Intracranial phakomatosis has been reported to include subependymal nodules; cortical tubers, and subependymal giant-cell astrocytomas. Subependymal giant-cell astrocytomas are rare, benign brain tumors of unknown histogenesis which cause increased intracranial pressure, seizures, and focal neurologic signs. The only treatment of these tumors is total surgical removal, because they are not sensitive to irradiation and chemotherapy. We report a patient with tuberous sclero

Mesenchymal lesions of the vulva.

Mesenchymal lesions of the vulva include site-specific entities limited to the lower genital tract, as well as a range of non-site-specific tumors that are more common at extragenital sites. Site-specific lesions include fibroepithelial stromal polyp, cellular angiofibroma, angiomyofibroblastoma, and aggressive angiomyxoma. Non-site-specific tumors that may occur in the vulva include those of smooth muscle, skeletal muscle, vascular, neural, adipocytic, and uncertain differentiation. This review discusses both site-specific and non-site-specific vulvar mesenchymal lesions including non-neoplas…

[Tuberous sclerosis: diagnostic criteria and new treatment approaches].

With a prevalence of 1 in 6,000 births, tuberous sclerosis is a relatively frequent hamartoma and tumor syndrome inherited as an autosomal dominant trait, which manifests primarily on the skin and in the central nervous system. Decisive factors for morbidity and mortality and thus for the prognosis are the changes in the central nervous system in the form of cortical hamartomas. Treatment for many years consisted solely in using nonspecific symptomatic approaches; dermatological therapy comprised mainly laser or electroacoustic ablation of facial angiofibromas. New models of therapy hinder the…
