When Do Scanograms and Standing Ankle Radiographs Differ? A Comparative Analysis of Ankle Alignment Measurements.

Background: Lower extremity scanograms are widely used to evaluate lower limb alignment and are frequently used as a substitute for standing ankle radiographs in retrospective studies. However, their reliability for evaluating ankle osteoarthritis remains unclear. This study investigated radiographic discrepancies between lower extremity scanograms and standing ankle radiographs and identified the patient populations in whom scanograms may not be an appropriate alternative. Methods: This retrospective study included 90 ankles that underwent both lower extremity scanograms and standing ankle ra…

Development of Wearable Cyborg HAL Ankle Joint Unit to improve ankle joint function during gait.

Stroke, spinal cord injury, and cerebral palsy are central nervous system disorders that impair voluntary movement due to dysfunctions in the brain, nervous system, and muscles. Cybernics treatment of HAL is being promoted as a treatment for these diseases. Besides, patients with central nervous system disorders who have symptoms of equinus, drop foot, and spasticity experience difficulties improving their ankle joint function during gait because the ankle joint of HAL is a passive mechanism, which cannot reflect the wearer's intention. Thus, it is hypothesized that ankle joints can be improve…

Foot-Ankle Mechanical Transmission: Age Effects and the Relation to Ankle Push-Off During Walking.

Older adults walk with less push-off power than younger adults. Principally attributed to plantar flexor dysfunction, growing evidence implicates interactions between the foot and ankle as critical for generating effective push-off. Our purposes were to measure age effects on foot-ankle mechanical transmission (FAMT, ie, the ratio between metatarsal phalangeal extension and medial gastrocnemius fascicle length change), and its association with ankle push-off during walking. We hypothesized that (1) FAMT would be lesser in older adults and (2) lesser FAMT would positively correlate with slower …

Effects of chronic ankle instability after grade I ankle sprain on the post-traumatic osteoarthritis.

BACKGROUND: Untreated acute ankle sprains often result in chronic ankle instability (CAI) and can ultimately lead to the development of post-traumatic osteoarthritis (PTOA). At present, a typical animal model of ankle instability in mice is established by transecting the ligaments around the ankle joint. This study aimed to establish a grade I acute ankle sprain animal model by rapid stretching of peri-ankle joint ligaments. Furthermore, we tried to explore the pathophysiological mechanism of ankle osteoarthritis. METHODS: In all, 18 male C57BL/6 J mice (7 weeks) were randomly divided into th…

Functional tests as predictors of ankle instability six months after an acute lateral ankle sprain.

BACKGROUND: This study investigated whether functional tests performed in the acute-subacute phase after a lateral ankle sprain and demographic data are predictive of ongoing ankle instability. METHODS: Thirty-three subjects (mean age 23.6±3.6 years, 63.6% males) were tested within three weeks of injury using the Balance Error Scoring System (BESS) modified with a dual cognitive task, the single-leg drop landing task (SLDL) and the drop vertical jump task (DVJ). The Cumberland Ankle Instability Tool (CAIT) at six months and the occurrence of a new sprain during follow-up were used to define p…

Ankle plantar flexor muscle performance and patient reported outcomes in people following total ankle arthroplasty.

BACKGROUND: The purpose of this study was to prospectively evaluate ankle power generation during gait in people with total ankle arthroplasty, and examine the relationships between postoperative plantar flexor strength, ankle power, and patient outcomes. METHODS: Nineteen people with end-stage ankle arthritis who received a total ankle arthroplasty and 19 healthy matched controls participated in this case-control study. Patient reported outcomes included a region specific measure of foot function and a generic measure of physical function. Gait speed was recorded with the 6-min walk test. Is…

Ankle taping can reduce external ankle joint moments during drop landings on a tilted surface.

Ankle taping is commonly used to prevent ankle sprains. However, kinematic assessments investigating the biomechanical effects of ankle taping have provided inconclusive results. This study aimed to determine the effect of ankle taping on the external ankle joint moments during a drop landing on a tilted surface at 25°. Twenty-five participants performed landings on a tilted force platform that caused ankle inversion with and without ankle taping. Landing kinematics were captured using a motion capture system. External ankle inversion moment, the angular impulse due to the medio-lateral and ve…

Ankle Fusion Combined With Calcaneal Sliding Osteotomy for Severe Arthritic Ball and Socket Ankle Deformity.

BACKGROUND: Although a ball and socket ankle deformity is usually congenital and asymptomatic, abnormal inversion and eversion mobility can result in recurrent ankle sprain and osteoarthritis. This retrospective study was performed to evaluate the clinical and radiologic outcomes of ankle fusion combined with calcaneal sliding osteotomy for severe arthritic ball and socket ankle deformity. METHODS: Fourteen patients with severe arthritic ball and socket ankle deformity were followed for more than 3 years after operation. The clinical evaluation consisted of American Orthopaedic Foot & Ankle S…

Clinical assessment of ankle injury outcomes: case scenario using the foot and ankle ability measure.

UNLABELLED: PATIENT SCENARIO: A 20-y-old male Division 1 college basketball player sustained a grade 2 inversion ankle sprain during preseason that is preventing him from practicing and competing. CLINICAL OUTCOMES ASSESSMENT: The Foot and Ankle Ability Measure (FAAM) was administered to the injured athlete as an evaluative tool to provide the clinician with valuable subjective information on the patient's self-reported function. The FAAM consists of 2 subscales: the activities of daily living (ADL) subscale and the sports subscale. Together the 2 subscales contain 29 questions (21 questions o…

A new hip-knee-ankle-foot sling: kinematic comparison with a traditional ankle-foot orthosis.

In this study, we performed a kinematic analysis of a new, low-cost sling for the lower limb, compared to a common ankle-foot orthosis (AFO). Gait with no orthosis, with the AFO, and with the new sling was analyzed in one hemiplegic subject. Both the AFO and the sling reduced the mean angle and ROM (range of movement) of the ankle and the vertical displacement of the center of mass. The sling, but not the AFO, restored the normal sequence heel-strike, forefoot contact of the affected side. The sling, but not the AFO, reduced the affected limb stance and stride duration, increased stride length…

Validation of Foot and Ankle Ability Measure (FAAM) and the Foot and Ankle Outcome Score (FAOS) in individuals with chronic ankle instability: a cross-sectional observational study.

BACKGROUND: Ankle sprain is the most common lower limb injury in physically active individuals. Loss of function, decreased postural control (PC), strength deficit, and reduced range of motion (ROM) are common after acute lateral ankle sprains. Some patients experienced long lasting symptoms, with recurrent sprains, and episodes of giving-way: a condition known as chronic ankle instability (CAI). Evaluating the function in patients with CAI in the clinical environment is important to identify the severity of the condition, in addition to allowing to assess the effectiveness of a given treatmen…

Overuse Injuries of the Ankle and Foot.

Overuse injuries of the foot and ankle typically result from repetitive microtrauma rather than from a sudden sprain or a single impact, in which the cumulative load on tendons, fascia, bones, and surrounding soft tissues exceeds the tissues' reparative capacity. Repetitive lower extremity loading and complex movement patterns in sports such as soccer, basketball, gymnastics, and running increase the risk of overuse injuries. Common symptoms include swelling, pain, and limited mobility, which tend to worsen with continued activity. While conservative treatment is often sufficient, persistent p…

[Corrective osteotomy of the ankle joint].

Osteoarthritis of the ankle joint affects a younger and often occupational and sport-active patient group than hip and knee arthritis and in the majority of cases has a secondary or posttraumatic origin. Corrective osteotomy is a biological and functionally attractive option for these young and active patients with asymmetric arthritis and correctable malalignment as the native joint is preserved and subsequent arthrosis can be potentially prevented or delayed by arthrodesis. Decisive for the success are consistent 3‑dimensional diagnostics, precise planning, correct interpretation of the indi…

Orthobiologics for Foot and Ankle Tendinopathy.

Foot and ankle tendinopathies are common problems for which there exists a "treatment gap" between surgery and traditional conservative management. Orthobiologics have potential to help fill that gap, aiming to use molecular biology to steer host cell responses, with the goal of improving tissue homeostasis and function. They are primarily used for chronic tendon degeneration and angiofibroblastic disrepair-"tendinosis"-rather than acute inflammation-"tendonitis." As the technology, techniques, and clinical evidence for orthobiologics continue to develop and advance, several biologic therapies…

The Use of Allografts in Ankle Injuries.

Ankle sprains can damage the lateral ankle ligament complex, resulting in lateral ankle instability or intra-articular cartilage damage, like osteochondral defects (OCD). Both ankle instability and OCD can lead to ankle osteoarthritis, resulting in substantial disability and negatively impacting patients' quality of life. Treating these lesions could be challenging, and various surgical procedures have been described in the literature, including allografts, which have gained popularity and shown promising results in the last few years. The main advantages of allografts include the absence of d…
