Acute Ankle Diastasis Injuries Treated with Dynamic, Static Fixation or Anatomic Repair: A Meta-Analysis and Systematic Review of Comparison Studies.

BACKGROUND: Acute ankle diastasis injuries are complex and debilitating. These injuries occur when the syndesmotic complex becomes compromised. Treatments of acute syndesmotic injuries include static fixation with screws, dynamic fixation with an elastic device, or anatomic repair of the damaged ligament. However, there is disagreement over which method is most effective. The primary purpose of this study was to compare the 3 treatment methods for acute syndesmotic injuries. METHODS: A systematic literature search was conducted on Embase and PubMed. Studies that compared at least 2 groups wit

Suture button versus syndesmosis screw constructs for acute ankle diastasis injuries: A meta-analysis and systematic review of randomised controlled trials.

BACKGROUND: Ankle syndesmotic injuries can be surgically managed with syndesmosis screws (SS) or suture button (SB) fixation. We performed a meta-analysis of randomized controlled trials (RCTs) aiming to compare the clinical and complication profiles of both modalities. METHODS: A multi-database search up to 4th of March 2018 was performed according to PRISMA guidelines. All RCTs comparing both techniques and published in English were included. RESULTS: Five RCTs with a total of 280 patients (140 SB, 140 SS) were included for analysis. SB had a statistically significant higher AOFAS score at

Impact of clinical decision rules on clinical care of traumatic injuries to the foot and ankle, knee, cervical spine, and head.

INTRODUCTION: Traumatic injuries to the ankle/foot, knee, cervical spine, and head are very commonly seen in emergency and accident departments around the world. There has been much interest in the development of clinical decision rules to help guide the investigations of these patients in a standardised and cost-effective manner. METHODS: In this article we reviewed the impact of the Ottawa ankle rules, Ottawa knee rules, Canadian C-spine rule and the Canadian CT head rule. RESULTS: The studies conducted have confirmed that the use of well developed clinical decision rules results in less r

Do Single-Leg Balance Control and Lower Extremity Muscle Strength Correlate with Ankle Instability and Leg Injuries in Young Ballet Dancers?

Ankle injuries are common among young ballet dancers. These injuries may be attributed to ankle instability, insufficient lower extremity strength, and poor balance control. The purpose of this study was to explore whether these dancers exhibit functional ankle instability and if their single-leg balance control and lower extremity muscle strength correlate with functional ankle instability and leg injuries. Twenty-one ballet dancers (aged 10 to 17 years) participated in the study. The Cumberland Ankle Instability Tool (CAIT) questionnaire was used to examine functional ankle stability. Isomet…

What priorities should be considered for Iranian veterans with ankle-foot injuries? A health needs assessment study, 25 years post-conflict.

Despite the passage of time, a large number of veterans are still affected by injuries acquired during Iran-Iraq war. In addition to their primary injuries, the majority of veterans also experience difficulty with long-term, secondary effects. Studies have shown that the most common of these include a range of disabilities, pain, and dramatic decline in mental health and quality of life. Improving living conditions and providing rehabilitation services to veterans has always been a main priority of authorities. The goal of this study was to explain the methods and materials with which these pr

Prescription footwear for severe injuries of foot and ankle: effect on regularity and symmetry of the gait assessed by trunk accelerometry.

After foot and/or ankle fracture, the restoration of optimal gait symmetry is one of the criteria of recovery. Orthotic insoles and orthopaedic shoes improve gait symmetry and regularity by controlling joint motion and improving alignment. The aim of the present study was to assess the effect of prescription footwear on gait quality by using accelerometers attached to the lower back. Sixteen adult patients with persistent disability after ankle and/or foot fractures performed two 30-s walking trials with and without prescription footwear (insoles and stabilizing shoes). Sixteen control subject

An inverted ankle joint orientation at foot strike could incite ankle inversion sprain: Comparison between injury and non-injured cutting motions of a tennis player.

Ankle sprain is very common in sports. Research on its prevention is as important as on its treatment as recommended in the 2016 consensus statement of the International Ankle Consortium. Successful prevention depends on the understanding of its mechanism, which has been presented with quantities in some recent case reports. Inciting event was suggested to be an inverted ankle joint at foot strike, however, is still lacking evidence from comparison with non-injury trials. This study investigated the ankle joint orientation at foot strike in successful non-injury cases and compared them with a

Frequency and trends in foot and ankle injuries within an English Premier League Football Club using a new impact factor of injury to identify a focus for injury prevention.

BACKGROUND: Foot and ankle injuries are common in football. Prevention strategies exist in order to decrease the incidence of such injuries and minimize the number of days that a player is unavailable for selection. METHODS: Injuries were recorded over a 4-season period while the team was playing in the English Premier League (EPL). We present the epidemiology of foot and ankle injuries within a professional football club and offer a calculation that may be of use in the future to identify areas of injury prevention. RESULTS: Anterior Talo-Fibular Ligament (ATFL) injuries and fifth metatarsa…

Acute Compartment Syndrome Following a Low-Energy Ankle Injury: Case Report and Review of the Literature.

A healthy 21-year-old male active-duty sailor experienced acute compartment syndrome (ACS) of the right lower extremity after sustaining an inversion ankle injury during a summer football practice. In the emergency room, he was diaphoretic, with diffuse tenderness over the lateral leg and ankle and mild paresthesia over the dorsum of the foot. He had palpable pedal pulses and no pain with passive great toe extension. Initial radiographs demonstrated no acute osseous abnormality. However, the emergency department team ordered a creatine kinase (CK) out of concern for rhabdomyolysis. It returned

Presence of Calcaneal Spurs and Injury Severity of Ankle Sprain in Emergency Room Patients: A Retrospective Analysis.

This study aims to evaluate the incidence of calcaneal spurs in patients presenting with ankle sprains and to determine the association between the presence of spurs and the severity of the trauma sustained. Calcaneal spurs occur in 15-20% of the general population, and when symptomatic, they can have a considerable negative impact on an individual's quality of life. We conducted a retrospective analysis of individuals aged 18 years and above who presented with ankle sprains in the emergency department over the course of 1 year. A total of 3,667 patients with ankle sprains were included in the

MRI Evaluation of Ligamentous Injury in Weightbearing-Stable Suprasyndesmotic Ankle Fractures: A Prospective Observational Study.

BACKGROUND: Suprasyndesmotic ankle fractures (Weber C) account for approximately 10% of ankle fractures, and surgery is advised because of the assumed unstable nature of these injuries. Treatment of transsyndesmotic ankle fractures (Weber B) has evolved as weightbearing radiographs are employed as a modality to evaluate ankle joint stability. Joint congruency on weightbearing radiographs indicate sufficient ligamentous integrity to allow for nonoperative treatment. However, no studies have evaluated the ligamentous injury patterns in suprasyndesmotic ankle fractures with a congruent ankle join

Rotational Dynamics of the Distal Tibiofibular Joint After Operative Treatment of Ankle Fractures With Syndesmosis Injury.

BACKGROUND: Syndesmosis injury healing remains poorly understood, despite its high prevalence in ankle fractures. Unstable syndesmosis is commonly addressed with either syndesmosis screw (SS) or suture button (SB) fixation, and up to 20% of operated ankle fractures may require some form of syndesmosis fixation. However, in biomechanical studies no repair technique fully restores the preinjury rotational stability or the anatomical alignment of the tibiofibular joint. METHODS: In a study of 39 patients with operatively treated supination external-rotation type 4 and pronation external-rotation

Outcome, severity of injury and length of sick leave after an ankle fracture: an observational register study.

There is a lack in understanding the reasons for different lengths of sick leave in patients who sustain ankle fractures. The aim of this study is to examine variations in the length of sick leave in ankle fracture patients and how treatment, type of ankle fracture and the patient-reported outcome are associated with the length of sick leave. In this study were data from the Swedish Social Insurance Agency (SSIA) and the Swedish Fracture Register (SFR), combined. Patients who sustained an ankle fracture were identified and the length of the sick leave calculated. Variables associated to the le
