Vulval fixed drug eruption following trimethoprim use.

This case report describes an uncommon case of a woman who developed fixed drug eruption (FDE) on her vulval area secondary to trimethoprim use. She had no known history of drug allergy. Some common culprit medications that cause FDE include antimicrobials such as co-trimoxazole and non-steroidal anti-inflammatory agents. Vulval FDE is uncommon and should remain as an important differential diagnosis in women presenting with severe vulval pain accompanied by erythema and swelling, particularly after the introduction of a new medication. Discontinuation of trimethoprim along with the adjunct us

Therapies in early development for the treatment of urinary tract inflammation.

INTRODUCTION: Urinary tract inflammation is a very common clinical condition. It is caused by several pathogens and antibiotic treatment is the mainstay of therapy. Increasing antimicrobial resistance and high recurrence rates represent a challenge. Consequently, there is an unmet need for new therapeutic options. AREAS COVERED: The authors discuss the rationale of emerging management strategies and current experimentation. Furthermore, they focus on both acute and recurrent urinary tract infections (UTIs) and examine a range of therapeutics, including new antibiotics, vaccines, mannosides, h

[Healthcare-associated urinary tract infections in patients with a urinary catheter: Risk factors, microbiological characteristics and patterns of antibiotic resistance].

OBJECTIVE: Catheter-associated urinary tract infections (CAUTIs) are the most common nosocomial acquired infections, with high resistance rates. CAUTIs are a potentially severe complication in hospitalized patients and imply higher costs. Our aim was to analyze the characteristics of CAUTIs in our Urology department. METHODS: Between November 2011 and October 2013, a prospective observational study was carried out analyzing the incidence of healthcare-associated urinary tract infections in patients admitted to the urology ward with an indwelling urinary catheter. Furthermore, we evaluated ass

A multicenter qualitative study on preventing hospital-acquired urinary tract infection in US hospitals.

OBJECTIVE: Although urinary tract infection (UTI) is the most common hospital-acquired infection, there is little information about why hospitals use or do not use a range of available preventive practices. We thus conducted a multicenter study to understand better how US hospitals approach the prevention of hospital-acquired UTI. METHODS: This research is part of a larger study employing both quantitative and qualitative methods. The qualitative phase consisted of 38 semistructured phone interviews with key personnel at 14 purposefully sampled US hospitals and 39 in-person interviews at 5 of

The effect of chlorhexidine and gentian violet on the adherence of Candida spp. to urinary catheters.

Urinary tract infection associated with catheters is the most common infection in the hospital environment. The adherence of microorganisms to the surface is a determining factor in colonization and infection. Antiseptics such as chlorhexidine and gentian violet have been shown to be effective against yeasts, as well as having low toxicity and being low-cost. The objective of the present study was to evaluate whether prior treatment of siliconized latex urinary catheters with antiseptics reduces the adherence of yeasts. Two reference strains of C. albicans (ATCC 645448 and ATCC 90028) and six

Endoscopic Deflux Injection for Vesicoureteral Reflux in a Renal Allograft Complicated by Recurrent Acute Pyelonephritis: A Case Report.

Vesicoureteral reflux (VUR) in a renal allograft ureter is a well-recognized and clinically significant complication that may lead to recurrent febrile urinary tract infection (fUTI) and progressive allograft dysfunction. While open ureteral reimplantation remains a standard surgical option, it is relatively invasive for kidney transplant recipients. Endoscopic subureteral injection of a bulking agent such as Deflux has been widely used for primary VUR in children, but reports in renal transplant recipients are limited. We report a case of a living donor kidney transplant recipient who develop

Comparative effectiveness of oral antibiotics to treat uncomplicated urinary tract infections in male outpatients.

Aim: New IDSA guidelines define uncomplicated UTI (uUTI) as infection limited to the bladder in both men and women. This study compared the effectiveness of β-lactams, nitrofurantoin and trimethoprim/sulfamethoxazole (TMP/SMX) to fluoroquinolones for outpatient uUTI treatment in men. Materials & methods: A retrospective cohort of adult male outpatients diagnosed with uUTI during 2019-2021 in the Department of Veterans Affairs system was created. Inclusion required an Emergency Department, Urgent/Primary Care visit with ICD-10 documentation of UTI and a prescription of interest dispensed. Patie

Extracellular vesicle crosstalk in urinary tract infections: Integrating host exosomes and pathogen-derived membrane vesicles for antimicrobial resistance monitoring and diagnostic innovation.

Urinary tract infections (UTIs) remain a major global health burden, with rising antimicrobial resistance (AMR) posing severe challenges to diagnosis and treatment. While host-derived urinary exosomes have been recognized as promising noninvasive biomarkers for UTI, the role of pathogen-derived outer membrane vesicles (OMVs) in UTI pathogenesis and AMR has been comparatively underexplored. No comprehensive framework currently integrates both host extracellular vesicles (EVs) and pathogen-derived EVs in the context of UTI diagnosis and antimicrobial resistance monitoring. This review addresses

Comparing fosfomycin and ciprofloxacin for uncomplicated UTI treatment in adults: better bacterial eradication but non-significant clinical remission-a systematic review and meta-analysis.

BACKGROUND: Urinary tract infections (UTIs) are common in young children and women and even more so in pregnant women. Ciprofloxacin has been a long-used regime to treat UTIs, although recently the paradigm has shifted towards Fosfomycin. This shift is driven by rising antibiotic resistance, safety concerns with fluoroquinolones, and the need for alternative treatments with broader efficacy and fewer adverse effects. This systematic review and meta-analysis assess the effectiveness and safety of Fosfomycin compared to Ciprofloxacin for treating urinary tract infections in adults. METHODS: A s

Updates in Clinical Management of Recurrent Urinary Tract Infections.

Urinary tract infections (UTIs) are common and burdensome in women. Here, we discuss challenges with our current models of care and how evolving insights into the female urogenital microbiome have advanced the understanding of how we diagnose, treat, and prevent recurrent UTIs in nonpregnant adult women. Traditional care models attribute recurrent UTIs mainly to gastrointestinal sources, resulting in significant emphasis on eradicating pathogens with potential overreliance on antibiotics. Evidence now shows that the bladder harbors a complex microbiome, with interactions between the urinary an

[Cefixime in urinary tract infections in women].

Urinary tract infections (UTIs) remain one of the most common bacterial infections in outpatient practice, particularly among women of reproductive age and pregnant patients. One of the therapeutic options for UTIs is cefixime, a broad-spectrum third-generation cephalosporin. Cefixime demonstrates high antimicrobial activity with a minimal number of adverse effects, which explains the absence of contraindications for its use during pregnancy. Cefixime has high oral bioavailability, is often resistant to -lactamases, and ensures sufficient urinary concentration when administered at 400 mg once

Advances in the Treatment of Urinary Tract Infection and Bacteriuria in Pregnancy.

Understanding the management of asymptomatic bacteriuria (ASB) and urinary tract infection (UTI) during pregnancy is important, given the prevalence of these conditions, associated risks to the mother and fetus /newborn, and potential requirement for antibiotic treatment. Screening for ASB with a urine culture is required during pregnancy, and a positive culture is treated for 5 to 7 days with culture-specific antibiotics. Recent research on non-antibiotic prevention and shortened duration of antibiotic treatment may offer a slightly different approach to ASB and UTI in pregnancy.

The Diagnosis and Treatment of Adult Urinary Tract Infections in the Emergency Department.

Emergency medicine has been called the art of "making complicated clinical decisions with limited information." This description is particularly relevant in the case of diagnosis and treatment of urinary tract infections (UTIs). Although common, UTIs are often challenging to diagnose given the presence of non-specific signs and symptoms and over-reliance on laboratory findings. This review provides an interdisciplinary interpretation of the primary literature and practice guidelines, with a focus on diagnostic and antimicrobial stewardship in the emergency department.

Neonatal pyocele originating from a urinary tract infection: a case report.

BACKGROUND: The origins of pyocele are primarily idiopathic, with reports suggesting the spread from intraperitoneal or hematogenous infection. However, we found no information in the literature regarding the pathogen's spread from the urinary tract. CASE PRESENTATION: We report here a case of a 23-day-old term Iranian boy (Fars ethnicity) with complaints of new-onset fever, irritability, poor feeding, right hemiscrotal erythema, and edema. Moreover, the physical examination revealed an enlarged, erythematous, tender right hemiscrotum and edematous scrotal walls. Along with leukocytosis and e

Defining the Optimal Duration of Therapy for Hospitalized Patients With Complicated Urinary Tract Infections and Associated Bacteremia.

BACKGROUND: Limited data are available to guide effective antibiotic durations for hospitalized patients with complicated urinary tract infections (cUTIs). METHODS: We conducted an observational study of patients ≥18 years at 24 US hospitals to identify the optimal treatment duration for patients with cUTI. To increase the likelihood patients experienced true infection, eligibility was limited to those with associated bacteremia. Propensity scores were generated for an inverse probability of treatment weighted analysis. The primary outcome was recurrent infection with the same species ≤30 day

Genotypic characteristics of Uropathogenic Escherichia coli isolated from complicated urinary tract infection (cUTI) and asymptomatic bacteriuria-a relational analysis.

BACKGROUND: Uropathogenic Escherichia coli (UPEC) is the predominant agent causing various categories of complicated urinary tract infections (cUTI). Although existing data reveals that UPEC harboured numerous virulence determinants to aid its survival in the urinary tract, the reason behind the occurrence of differences in the clinical severity of uninary tract infections (UTI) demonstrated by the UPEC infection is poorly understood. Therefore, the present study aims to determine the distribution of virulence determinants and antimicrobial resistance among different phylogroups of UPEC isolat

Microbiologist in the clinic: coitally related symptoms with negative urine cultures.

In this first episode of the Microbiologist in the Clinic series, clinicians and laboratory scientists share their perspectives about a 30 y/o woman, who is seeking specialty consultation for frequent episodes of urinary urgency, frequency, and dysuria, which respond to short courses of antibiotics. Although her home dipsticks suggest that she has a UTI, and her urinalysis typically has a moderate number of white blood cells, her urine cultures are always negative. The challenges of this clinical presentation are discussed with evidence for evaluation and treatment.

Gepotidacin: a novel, oral, 'first-in-class' triazaacenaphthylene antibiotic for the treatment of uncomplicated urinary tract infections and urogenital gonorrhoea.

The ongoing spread of antimicrobial resistance has made the treatment of uncomplicated urinary tract infections (UTIs) and urogenital gonorrhoea increasingly difficult. New oral treatment options are urgently needed. Gepotidacin (previously GSK2140944) is a novel, bactericidal, oral, 'first-in-class' triazaacenaphthylene antibiotic that inhibits bacterial DNA replication by blocking two essential topoisomerase enzymes. Mutations in both enzymes would likely be necessary for resistance to occur, thus raising hopes that the drug will be able to maintain long-term effectiveness. Data from Phase I
