Antibiotic Prophylaxis and Treatment of Neonatal Group B Streptococcus Disease in the Era of Antimicrobial Resistance.

Group B Streptococcus (GBS) remains a major cause of early- and late-onset neonatal sepsis worldwide, despite the widespread use of intrapartum antibiotic prophylaxis (IAP). β-lactam antibiotics, including penicillin G and ampicillin, remain the cornerstone of both GBS prophylaxis and neonatal treatment, supported by sustained susceptibility, favorable pharmacokinetics, and extensive clinical experience. However, increasing global resistance to macrolides and lincosamides has markedly reduced the reliability of clindamycin and erythromycin, which are commonly used as second-line agents in wome…

The Impact of Doxycycline Post-Exposure Prophylaxis on Antibiotic Use at a Boston Sexual Health Clinic.

BACKGROUND: Doxycycline post-exposure prophylaxis (doxy-PEP) reduces the incidence of bacterial sexually transmitted infections (STIs) among men who have sex with men and transgender women (MSMTW), but it may select for antimicrobial resistance (AMR). AMR development will depend in part how doxy-PEP changes rates of antibiotic use. METHODS: We conducted a retrospective electronic medical record review of antibiotic prescriptions received by patients at the Massachusetts General Hospital Sexual Health Clinic from January 1, 2023, to December 27, 2025. Using a Bayesian negative binomial regress

[Fosfomycin as antibiotic prophylaxis in men undergoing transrectal prostate biopsy: a systematic review of the literature].

Transrectal prostate biopsy remains the standard method for prostate cancer diagnosis, but it is associated with a high risk of infectious complications. In recent years, fosfomycin has been increasingly used as an alternative for antibiotic prophylaxis due to the increasing resistance of pathogens to fluoroquinolones, its good safety profile and ease of use. Results of several studies dedicated to the efficiency of fosfomycin are highlighted in this review. Fosfomycin demonstrates significant advantages as a drug for the prevention of infectious and inflammatory complications in patients afte…

Impact of intrapartum antibiotic prophylaxis on the oral and fecal bacteriomes of children in the first week of life.

Intrapartum antibiotic prophylaxis (IAP) is commonly used during C-section delivery and in Group B Streptococcus-positive women before vaginal delivery. Here, we primarily aimed to investigate the effect of IAP on the neonatal oral and fecal bacteriomes in the first week of life. In this preliminary study, maternal and neonatal oral swabs and neonatal fecal (meconium and transitional stool) swabs were selected from a pool of samples from healthy mother-neonate pairs participating in the pilot phase of CELSPAC: TNG during their hospital stay. The DNA was extracted and bacteriome profiles were d…

Etiquette of the antibiotic decision-making process for surgical prophylaxis in Ethiopia: a triangulated ethnographic study.

BACKGROUND: Prophylactic antibiotics reduce surgery-associated infections and healthcare costs. While quantitative methods have been widely … re cleaned, coded, summarized, and analyzed using the thematic analysis approach. RESULT: Surgical antibiotic prophylaxis (SAP) discussions were infrequent during surgical ward rounds in TASH, leading to practices that deviated from established recommendations. Clear documentation differentiating SAP from other antibiotic uses was also lacking, which contributed to unj …

Impact of colonization with multidrug-resistant organisms on antibiotic prophylaxis in patients with cirrhosis and variceal bleeding.

BACKGROUND: The efficacy of antibiotic prophylaxis to prevent rebleeding or infection after variceal bleeding in patients with liver cirrhosis colonized with multidrug-resistant organisms (MDROs) is unknown. METHODS: In this retrospective study, patients with liver cirrhosis and endoscopically confirmed variceal bleeding who were treated at a tertiary care center in Germany and were screened for MDROs at the time of bleeding were eligible for inclusion. Efficacy of antibiotic prophylaxis was evaluated in patients stratified according to microbiological susceptibility testing. RESULTS: From 9…

Timing and duration of antibiotic prophylaxis is associated with the risk of infection after hip and knee arthroplasty.

AIMS: Antibiotic prophylaxis involving timely administration of appropriately dosed antibiotic is considered effective to reduce the risk of surgical site infection (SSI) after total hip and total knee arthroplasty (THA/TKA). Cephalosporins provide effective prophylaxis, although evidence regarding the optimal timing and dosage of prophylactic antibiotics is inconclusive. The aim of this study is to examine the association between cephalosporin prophylaxis dose, timing, and duration, and the risk of SSI after THA/TKA. METHODS: A prospective multicentre cohort study was undertaken in consentin…

The Risk of Opportunistic Infections and the Role of Antibiotic Prophylaxis in Patients on Checkpoint Inhibitors Requiring Steroids.

BACKGROUND: Immune-related adverse events (irAEs) often require treatment with high-dose systemic steroids (SS) and other immunosuppressive agents (ISAs). NCCN Guidelines recommend prophylactic antibiotics for Pneumocystis jirovecii pneumonia (PJP) for patients receiving prolonged SS/ISAs. However, there is a paucity of evidence regarding the incidence of opportunistic infections (OIs) and non-OIs and the role of prophylactic antibiotics in patients on SS/ISAs for irAEs. METHODS: A retrospective analysis was conducted of patients treated using immune checkpoint inhibitor (ICI) therapy at 5 Me

Compliance with national guidelines for antibiotic prophylaxis prescription with a surgical abortion: Results of the Mya study.

PURPOSE: Performing an antibiotic prophylaxis (ATBP) with a surgical abortion (SA) is recommended in numerous countries, including France where the CNGOF has issued guidelines. There is little data regarding the actual application of these guidelines in clinical practice. The aim of this study was to evaluate the practice of ATBP with SA and to assess the degree of compliance with the national guidelines. METHODS: This longitudinal, prospective, multicenter, non-interventional study was undertaken between December 2013 and July 2014 with the objective of assessing the modalities of prescripti…

Glycopeptide vs. non-glycopeptide antibiotics for prophylaxis of surgical site infections: a systematic review.

BACKGROUND: Patients receive prophylactic antibiotics against surgical site infections (SSIs) before or during many procedures. Glycopeptide antibiotics are effective against most strains of methicillin-resistant Staphylococcus aureus (MRSA), but their wider use risks increasing resistance. Our objective was to review the evidence for clinical effectiveness that might help to determine whether there is a threshold of MRSA prevalence at which switching from non-glycopeptide to glycopeptide antibiotic prophylaxis might be justified. METHODS: We performed a systematic review of randomized trials…

Antibiotic prophylaxis with two doses of cephalosporin in patients managed with internal fixation for a fracture of the hip.

A prospective, randomized, double-blind study was performed to evaluate the effects of antibiotic prophylaxis on the development of a wound infection in 239 patients who had immediate stabilization of a fracture of the proximal part of the femur with a dynamic hip screw. The effects of two perioperative doses of cefotiam, given twelve hours apart, were compared with those of two doses of a placebo. Sixteen perioperative risk factors were evaluated to determine whether it was possible to identify patients who were at risk for a wound infection. All patients were followed for a minimum of six we…

Challenging Weight-Tiered Antibiotic Prophylaxis in Obese Patients Undergoing Colorectal Surgery Using CT-Derived Body Composition.

OBJECTIVE: To determine whether CT-derived body composition and kidney function better predict cefazolin concentrations at the surgical site than body weight alone in patients with obesity undergoing colorectal surgery. BACKGROUND: Current cefazolin prophylaxis guidelines recommend dose escalation from 2 g to 3 g in patients weighing ≥120 kg. However, body weight inconsistently reflects antibiotic distribution at the surgical site. METHODS: We conducted a prospective study of adults with obesity undergoing elective colorectal surgery who received cefazolin prophylaxis and had preoperative CT

Single Versus Extended-dose Antibiotic Prophylaxis Regimens in Reduction Mammaplasty: A Systematic Review and Meta-analysis.

Extended postoperative antibiotic regimens remain widely prescribed in reduction mammaplasty, despite variability in practice and insufficient evidence to support their superiority over single-dose preoperative prophylaxis. This divergence in clinical practice carries significant implications for antimicrobial stewardship, healthcare costs, and patient safety. We aimed to synthesize contemporary evidence comparing single-dose preoperative versus extended postoperative antibiotic prophylaxis in reducing surgical site infection (SSI) and postoperative morbidity in reduction mammaplasty.
