Strabismus surgery outcomes in the Infant Aphakia Treatment Study (IATS) at age 5 years.

PURPOSE: To report strabismus surgery frequency and outcomes after monocular infantile cataract surgery with or without IOL implantation. METHODS: The Infant Aphakia Treatment Study (IATS) is a randomized, multicenter clinical trial comparing treatment of aphakia with a primary IOL or contact lens in 114 infants with a unilateral congenital cataract. This report is a secondary outcome analysis of ocular motor data from IATS patients who underwent strabismus surgery prior to age 5 years. RESULTS: Strabismus surgery was performed in 45 (39%) patients (contact lens group [CL], 37%; IOL group, 4…

Glued intraocular lens scaffolding for Soemmerring ring removal in aphakia with posterior capsule defect.

UNLABELLED: We describe using the glued intraocular lens (IOL) scaffolding technique to remove Soemmerring ring during secondary IOL implantation in aphakic eyes with associated posterior capsule defect following previous pediatric cataract surgery. Vitrectomy is performed, and the remnants of the posterior capsule are removed from the visual axis. A 3-piece foldable IOL is injected below the Soemmerring ring, and glued transscleral haptic fixation of the IOL is performed. The Soemmerring ring is dislodged from the periphery with a Sinskey hook and brought into the center of the pupil over the

Iris-claw, retropupillary-fixated, aphakic intraocular lens implantation for traumatic aphakia following penetrating keratoplasty.

We report the correction of aphakia using an iris-claw, aphakic intraocular lens (IOL) fixated in a retropupillary location in a 17-year-old young man who suffered blunt trauma to his eye 5 months after penetrating keratoplasty (PKP). There were no intraoperative complications. At 21 months after implantation, the patient's uncorrected distance visual acuity was 20/28; his corrected distance visual acuity was 20/22, with +0.50 -3.00 × 155. Intraocular pressure was normal, and endothelial cell count was 1798 cells/mm(2).

Posteriorly enclavated iris claw intraocular lens for aphakia: long-term corneal endothelial safety study.

PURPOSE: To report 2 years follow-up experience, corneal endothelial cell loss results, and A constant used in retroiridis-fixated iris-claw aphakic intraocular lens (IOL) secondary implantation to correct aphakia in eyes without adequate capsule support. METHODS: In this prospective, interventional, clinical case series, 16 consecutive eyes of 14 patients (13 adults and 1 child) underwent retroiridis implantation of Artisan iris-claw aphakic IOL (Ophtec BV, Groningen, the Netherlands). Outcome measures included spherical equivalent (SE) of refractive error, uncorrected distance visual acuity…

Traumatic pediatric cataract: a decade of follow-up after Artisan aphakia intraocular lens implantation.

PURPOSE: To describe the long-term clinical outcome of Artisan((R)) aphakia intraocular lens (IOL; Ophtec, Groningen, The Netherlands) implantation in five aphakic eyes of five children, without capsular support, after cataract extraction following penetrating ocular trauma. METHODS: The charts of the five children were retrospectively reviewed. The data collected included follow-up time, nature of injury, age at cataract extraction and IOL implantation, visual outcome, endothelial cell counts, complications, and subsequent surgical interventions. RESULTS: Average follow-up was 11.0 years (r…

Rigid gas permeable contact lens correction of aphakia following congenital cataract removal during infancy.

Rigid gas permeable (RGP) contact lenses were prescribed and worn with little difficulty by 10 consecutive aphakic infants. Fluoroperm 92 lenses were used instead of silicone lenses (Bausch & Lomb) because of their lower cost, ease in handling, flexibility in design, and better neutralization of astigmatism. Lenses were worn on a daily wear basis (with occasional overnight wear) to reduce the risk of keratitis. Keratometry readings were taken at the time of surgery with the patient in a supine position. Trial lens fitting was performed 1 to 7 days later using a Burton Lamp. None of the patient

Globe Axial Length Growth at Age 10.5 Years in the Infant Aphakia Treatment Study.

PURPOSE: To report the change in globe axial length (AL) from the time of unilateral cataract surgery at age 1-7 months to age 10.5 years for infants enrolled in the Infant Aphakia Treatment Study, and to compare AL growth of operated eyes with that of fellow unoperated eyes. DESIGN: Comparative case series. METHODS: AL growth was analyzed relative to treated vs fellow eye, contact lens (CL) vs intraocular lens (IOL), visual acuity (VA) outcome, and the need for surgery for visual axis opacification. Eyes with glaucoma or glaucoma suspect were excluded from the primary analysis but reported …

Corneal Changes in Children after Unilateral Cataract Surgery in the Infant Aphakia Treatment Study.

PURPOSE: We report endothelial cell (EC) characteristics and central corneal thickness (CCT) from the Infant Aphakia Treatment Study (IATS) patients at the 5-year examination. DESIGN: Randomized, controlled trial of the treatment of unilateral cataract with aphakic contact lens (CL) versus primary intraocular lens (IOL) implant. PARTICIPANTS: A total of 114 infants with unilateral cataract. METHODS: The EC density, coefficient of variation (CV), and percent hexagonal cells were measured by noncontact specular microscopy. The CCT was measured using contact pachymetry. Fellow eyes served as c…

Secondary iris-claw anterior chamber lens implantation in patients with aphakia without capsular support.

BACKGROUND/AIMS: To evaluate the efficacy, predictability, safety and complications of secondary iris-claw intraocular lens (IOL) implantation in aphakic eyes without capsular support. METHODS: Retrospective, non-comparative, interventional case series of 128 aphakic eyes, which consecutively underwent secondary iris-claw Artisan IOL (Ophtec BV) implantation, were included. Manifest refraction, uncorrected visual acuity, best-spectacle corrected visual acuity, biomicroscopy, tonometry, funduscopy and central endothelial cell count (cECC) were evaluated before surgery, and at yearly intervals

Predictability of intraocular lens calculation and early refractive status: the Infant Aphakia Treatment Study.

OBJECTIVE: To report the accuracy of intraocular lens (IOL) power calculations and the early refractive status in pseudophakic eyes of infants in the Infant Aphakia Treatment Study. METHODS: Eyes randomized to receive primary IOL implantation were targeted for a postoperative refraction of +8.0 diopters (D) for infants 28 to 48 days old at surgery and +6.0 D for those 49 days or older to younger than 7 months at surgery using the Holladay 1 formula. Refraction 1 month after surgery was converted to spherical equivalent, and prediction error (PE; defined as the calculated refraction minus the …

Accuracy of intraocular lens power calculation in pediatric aphakia with the Artisan iris-claw lens.

PURPOSE: To compare the refractive accuracy of six intraocular lens (IOL) power calculation formulas in pediatric aphakia managed with anterior iris-claw (Artisan) IOL implantation. METHODS: Children aged 18 years or younger with ectopia lentis or microspherophakia who underwent lensectomy and anterior Artisan Aphakia IOL implantation between 2013 and 2024 at the Hospital for Sick Children, Toronto, were reviewed retrospectively. Optical biometry was obtained using the IOLMaster (version 5.4). Predicted refraction was calculated using SRK/T, Holladay 1, Hoffer Q, T2, Barrett Universal II, and…
