Retinal detachment in the second eye.

Bilateral retinal detachment (RD) occurred in 13% of 737 patients, with an interval between detachments of up to 30 years. Bilateral RD was more common in male patients and in those having more than 3 diopters of myopia. Detachment in the second eye was caused by retinal breaks in previously observed degeneration in 43% and in unsuspected diseased retina 57%. A significant number of patients with bilateral RD had multiple breaks. Aphakic RD occurred in bilateral RD and in unilateral RD patients at a comparable frequency; 28% of bilateral (mean duration, four years) and 35% of unilateral aphaki

Follow-up of aphakic patients with anterior vitrectomy in one eye and uneventful cataract extraction in the fellow eye.

Twenty patients had operations for bilateral cataract in 2 separate sessions, with uncomplicated extraction on one side and vitreous loss managed with anterior vitrectomy on the other. The cataracts, the type of surgery, and the postoperative treatment were identical in both eyes. The differences between the 2 eyes were not statistically significant so far as the amount of astigmatism, final visual acuity, and peripheral anterior synechiae were concerned. The main difference was found to be the period of time the vitrectomised eyes remained red and uncomfortable as compared to the eyes with un

Management of cataracts in infancy and childhood.

In recent years there has been a considerable improvement in our understanding of the mechanism of stimulus deprivation amblyopia and a continuous development of better surgical techniques for the removal of cataracts in very young patients. In spite of these advances and the fact that modern constant wear soft contact lenses are now a reliable proposition, there has not been a parallel improvement in the visual results achieved after cataract extraction and aphakia continues to present considerable problems of management in children. This study analyses over 100 cases treated at Moorfields Ey

Continuous wear soft lenses in the treatment of alphakia.

Continuous wear soft lenses made of Sauflon 85% were used in the treatment of 38 cases of uniocular aphakia. The patients were of all ages, but most of them were aged between 70 and 90 years. The success rate was greater than that achieved by hard contact lenses, especially after senile cataract extraction.

The comparison of corneal densitometry in cases with glaucoma following childhood cataract surgery and juvenile glaucoma.

PURPOSE: To compare the corneal densitometry (CD) in pediatric cases with glaucoma following childhood cataract surgery and juvenile open-angle glaucoma (JOAG). METHODS: This prospective comparative study involved 13 eyes with JOAG, 12 eyes with pseudophakic glaucoma, 13 eyes with aphakic glaucoma, and 15 control subjects. Pentacam HR Scheimpflug corneal topography was employed to evaluate corneal thickness (CCT) and CD values. RESULTS: The mean intraocular pressure (IOP) and CCT were significantly higher in aphakic glaucoma cases than the other groups (p = 0.001). In aphakic eyes, the mean

Anterior segment optical coherence tomography findings in the Infant Aphakia Treatment Study (IATS): a secondary analysis of a randomized clinical trial.

PURPOSE: To correlate the diagnosis of glaucoma among children in the Infant Aphakia Treatment Study (IATS) by age 10 years with anterior segment optical coherence tomography (AS-OCT) findings. METHODS: A multicenter randomized controlled trial of 114 infants with unilateral congenital cataract who were 1-6 months of age at surgery. Data on long-term glaucoma-related status and outcomes were collected when children were 10.5 years old. Participants were randomized at cataract surgery to either primary intraocular lens (IOL) or no IOL implantation (contact lens [CL]). AS-OCT findings in eyes w

Scleral-sutured intraocular lenses: Single-surgeon technique for suture-preloaded intraocular lens insertion through a small-incision corneal wound.

For scleral intraocular lens (IOL) fixation, suture-loaded IOLs have traditionally been folded in half with a forceps and delivered intraocularly through 3.5 to 4.0 mm corneal wounds. We describe a technique in which a suture-loaded scleral IOL is inserted through a small-incision corneal wound using 2 widely available ocular surgery tools; that is, a C or D cartridge and a Bowman nasolacrimal duct probe. This technique is easy to replicate and can be performed without an assisting surgeon.

Scleral fixation of one piece intraocular lens by injector implantation.

AIM OF STUDY: With an ab-interno technique of transscleral suturing of current one-piece posterior chamber intraocular lenses (PC IOLs) by injector implantation in the absence of capsular support, we aimed to demonstrate the possibility of the implantation of one-piece acrylic PC IOLs that might be produced in the future for only scleral fixation through small clear corneal incision. MATERIALS AND METHODS: Case report and literature review. RESULTS: This procedure has been performed in eight aphakic eyes with four different types of IOLs. Good centration was achieved with minimal technical e

[Supracapsular implantation with optic capture of posterior chamber intraocular lens in aphakic school-age children after traumatic cataract].

OBJECTIVE: To assess the clinical effects of supracapsular implantation with optic capture of the posterior chamber intraocular lens in school-age children with traumatic cataract. METHOD: It was a retrospective case series study. Thirteen cases (13 eyes) received posterior curvilinear capsulorhexis with optic capture of the posterior chamber intraocular lens. Pre- and post-operative visual acuities were recorded. Intra-o and post-operative complications were observed. The follow-up period ranged from 6 to 42 months. RESULTS: Implantation of optic capture of the posterior chamber intraocular

Ab externo technique for accurate haptic placement of transscleral sutured posterior chamber intraocular lenses.

A modified technique is presented for transscleral suture fixation of posterior chamber intraocular lenses for aphakic correction in cases with inadequate capsular support. An ab externo approach for suture externalization is described that minimizes intraocular manipulation and facilitates accurate placement of haptics in the ciliary sulcus, thereby reducing the risk of lens tilt and decentration. This technique eliminates intraocular suture knots and reduces the amount of time the globe is open during surgery by creating two intraocular suture loops in preparation for posterior chamber intra

Placement of a secondary foldable intraocular lens over a capsule remnant.

A bilaterally aphakic patient received a silicone intraocular lens (IOL) over an intact capsular bag. The fellow eye had a large capsule rupture. Implantation of a 3-piece silicone IOL was attempted over the capsule remnant and achieved without complication. One year postoperatively, the IOL remained stable.

[Secondary intraocular lens implantation in children].

OBJECTIVE: The study was designed to determine the visual outcome and indication of secondary posterior chamber intraocular lens (IOL) implantation. METHODS: The operation was performed on 34 eyes with aphakia in children after separation of posterior iris synechia from posterior capsule and formation of a posterior chamber space large enough to insert an intraocular lens. Based on whether the integrity of the posterior capsule was complete or not, various techniques of secondary IOL implantation were adopted. RESULTS: The follow-up periods ranged from 6 to 24 months. The results showed that

Corneal diameter in childhood aphakic glaucoma.

BACKGROUND: Glaucoma occurring after successful cataract surgery in infancy and childhood requires persistence to establish timely diagnosis and presents a therapeutic challenge. Previous studies have sought to identify risk factors, which would alert the clinician for the likelihood of developing glaucoma after cataract surgery in the young patient. Some risk factors are early age of surgery, microcornea, poor pupillary dilation, retained lens cortex, coexisting ocular anomalies, and cataract type. Other studies did not find these associations. In our experience, the majority of young aphakic

[Influence of the yellow-tinted intraocular lens on spectral sensitivity].

One of the complaints of patients with aphakic eyes or pseudophakic eyes implanted with UV or non-UV intraocular lenses (IOLs) is chromatopsia. To determine the extent of color distortion, we measured the spectral sensitivity curve of normal subjects with visual acuity above 1.0 with correction, if any, including subjects implanted with the yellow-tinted IOL (UVCY IOL: HOYA Co.). We found that color sensitivity in the blue range (400-440 nm) declined rapidly with subjects age. We drew a regression line plotting age against sensitivity for 66 normal subjects. Sensitivity for pseudophakic eyes i

Photorefractive keratectomy for the treatment of myopia after epikeratoplasty: a case report.

An aphakic eye with a +9.00 diopter (D) refraction developed high myopia of -13.00 D after applying epikeratoplasty using a +14.00 D lenticule. We preformed photorefractive keratectomy by excimer laser. The procedure was uneventful, with no postoperative pain. Epithelialization occurred as usual. Recovery of the cornea followed the usual course with initial overcorrection followed by certain regression. After 7 months follow-up the spherical equivalent was -3.25 D and the lenticule was clear. In myopia after epikeratoplasty photorefractive keratectomy is an effective alternative to lenticule e
