The purpose of this study was to determine whether lateral radiographs of the pharynx and fluoroscopy of the lateral pharynx in the sleeping patient could be used as predictors of surgical success in patients undergoing soft-palate surgery for obstructive sleep apnea. A total of 12 patients had surgery after radiologic evaluation. There were six surgical successes and six failures. No successes occurred in patients with a soft-palate length of less than 46 mm, nor were there any successes among patients whose initial point of obstruction on fluoroscopy was inferior to the level of the soft pal…

Auditory brainstem response (ABR) has been used by several investigators to study the role of the brain stem in the pathophysiology of obstructive sleep apnea (OSA). These studies have produced conflicting results. We studied 27 preoperative OSA patients and 17 controls using click stimuli presented at a slow (11.7/second) rate and at a fast (57.7/second) rate. ABR was repeated postoperatively in 18 patients. There were no statistically significant differences in the ABRs of preoperative OSA patients when compared with the control group. However, the preoperative recordings showed statisticall…

Of 22 patients investigated for sleep disorders, habitual snoring and/or daytime hypersomnolence, 12(10 men) had obstructive sleep apnea syndrome (OSAS). 3 OSAS were mild, 5 moderate and 4 severe. The leading symptoms were daytime hypersomnolence and habitual snoring. As risk factors we found retro-micrognathia in 2 patients, macroglossia secondary to acromegaly in 1, alcohol abuse in 7 and obesity in 6. Conservative measures improved the disorder subjectively in 6 patients. One patient had a relapse 6 months after uvulopalatopharyngoplasty. 4 patients were successfully treated by nasal CPAP. …

Uvulopalatopharyngoplasty (UPPP) is an operation that is frequently performed for the treatment of obstructive sleep apnea (OSA). While UPPP usually eliminates or decreases snoring and often reduces excessive daytime sleepiness, the decrease in the number of episodes of apnea and hypopnea, and the improvement in oxygen saturation (SaO2) have been less predictable. We compared preoperative and postoperative polysomnography (PSG) in 27 patients with OSA and found that no single PSG parameter could accurately reflect the changes in respiration seen after UPPP. We suggest that a combination of ind…

The effects of metabolic acidosis upon the pattern of apnea during sleep were assessed in ten sleep apnea patients. Four had pure obstructive apnea, two pure central apnea, and four had mixed apnea. Acidosis was induced with acetazolamide. Acid-base shifts had little effect in pure obstructive and pure central apnea, but had a significant effect in mixed apnea. In two of the mixed apneic patients, metabolic acidosis converted predominantly central apnea into nearly pure obstructive apnea, prolonging apneic periods and worsening hypoxemia. A suggested explanation for this is the greater stimula…

Patients with the sleep apnea syndrome are being increasingly referred to the otolaryngologist as it becomes clear that obstructive lesions in the nose, nasopharynx, and oropharynx can play a part in the pathogenesis of the syndrome. Correction of the obstructive lesion obviates the need to do a tracheotomy, which has been the conventional treatment for severe cases. In this paper we discuss two cases of obstructive sleep apnea, in whom surgical correction of nasal valve area obstruction was successful in abolishing the symptoms.

Sleep apnea occurring in association with neurologic lesions is usually of the central type. We have described a patient in whom obstructive sleep apnea followed a lateral medullary syndrome. We hypothesize that the upper airway obstruction during sleep was due to weakness of the palatal and pharyngeal muscles resulting from the neurologic lesion. The patient initially had reduced respiratory center motor output (mouth occlusion pressure) which increased significantly after tracheostomy. We conclude that obstructive as well as central apnea may follow neurologic lesions. Each patient should be…

To determine the characteristics of and mechanisms causing the bradycardia during sleep apnea (SA), both patients with SA and normals were studied. Evaluation of six consecutive SA patients demonstrated that bradycardia occurred during 95% of all apneas (central, obstructive, and mixed) and became marked with increased apnea length (P less than 0.01) and increased oxyhemoglobin desaturation (P less than 0.01). Heart rate slowed 9.5 beats per minute (bpm) during apneas of 10-19 s in duration, 11.4 bpm during 20-39s apneas, and 16.6 bpm during 40-59-s apneas. Sleep stage had no effect unexplaine…

Respiratory rhythm during sleep may be dependent on blood pH with apneas being associated with alkalosis. Acidification may therefore have therapeutic value in some forms of sleep apnea. We administered acetazolamide to six patients with symptomatic central sleep apnea, a disorder of respiratory rhythm with little or no upper airway obstruction. Sleep studies were carried out before and after one week of drug therapy, during which time the mean arterial pH decreased from 7.42 to 7.34. All six patients had significant improvement, demonstrating a 69% reduction in total apneas. Five of the six p…

This report presents six cases of obstructive sleep apnea associated with discrete anatomic abnormalities of the upper airway tract. All patients were relieved of their sleep apnea by surgical correction of the airway obstruction, and therefore permanent tracheotomy was avoided. The obstructive causative factors were nasal septal deformity, adenoidal hypertrophy, nasopharyngeal stenosis, lingual cyst, lingual tonsillar hypertrophy, and laryngeal cyst. The last-named four conditions have not previously been reported as correctable causes of sleep apnea.

Studies of the Treacher-Collins syndrome have emphasized hearing and surgical considerations. Although craniofacial anomalies have been associated with respiratory disorders in infancy, the presence of such problems in older children has not been emphasized. An eight-year-old with Treacher-Collins syndrome presented a history of recent behavioral problems at home, poor attention span and performance in school, daytime somnolence, and sleep apnea with relatively long periods of chest movement but no airflow. He also had abnormal sleep behavior consisting of rocking to and fro on his hands and k…

Periodic apnea and exercise hypoventilation were observed in a 14-year-old boy. Hyperphagia, obesity, serum hyperosmolality without diabetes insipidus or appropriate thirst, and retardation of growth and sexual development indicated a hypothalamic disorder. Neurologic evaluation was normal except for electroencephalographic changes induced by apnea. Pulmonary function tests, resting arterial blood gases in the wakeful state, and ventilatory response to inhaled CO2 were also normal. Acute hypoxemia and respiratory acidosis occurred with apnea during sleep and with insufficient ventilation durin…

Obstructive sleep apnea (OSA) is strongly associated with hypertension, yet the apnea-hypopnea index (AHI), while it defines the presence and frequency of respiratory events, predicts cardiovascular risk less reliably in the individual patient than metrics that incorporate the depth and duration of hypoxic exposure. Patients with comparable AHI values can differ substantially in nocturnal hypoxemia, sympathetic activation, ambulatory blood pressure behavior, and target-organ damage, exposing a persistent gap between respiratory classification and cardiovascular prognosis. This review synthesiz…

STUDY OBJECTIVES: The prevalence of obstructive sleep apnea (OSA) increases dramatically in adolescents with overweight or obesity. The gold standard for diagnosis of OSA is in-laboratory polysomnography (PSG). However, access to PSG can be challenging, necessitating development of alternative devices. This study aimed to compare the accuracy of WatchPAT, a device that indirectly detects apnea and hypopnea events through peripheral arterial tonometry, to PSG in adolescents with overweight and obesity.
METHODS: Thirty-eight participants (15.5 ± 1.6 years; 76% female; 40% Hispanic/Latino; body …

Background/Objectives: Cardiac Syndrome X (CSX) is associated with significant physical and psychiatric morbidity despite no obvious effect on long-term mortality. Obstructive sleep apnea (OSA) is a prevalent condition in close association with numerous cardiovascular diseases. The precise relation between CSX and OSA remains unclear. The aim of this study is to explore the relation between OSA and CSX, as well as the impact of continuous positive airway pressure (CPAP) therapy on myocardial ischemia. Methods: This single-center prospective cohort study examined patients who were selected cons…

BACKGROUND: Continuous Positive Airway Pressure (CPAP) telemonitoring is increasingly important in managing obstructive sleep apnea (OSA). The Apnea-Hypopnea Index reported by CPAP devices (AHIflow) is used as a key indicator of treatment effectiveness. However, discrepancies in AHIflow calculation rules between manufacturers may affect clinical decision-making. No prior studies have investigated whether manufacturers' choices to exclude certain apnea-hypopnea events from the AHIflow calculation may influence the number of patients presenting an AHIflow alert. The aim of this proof-of-concept …

BACKGROUND: Obstructive sleep apnea (OSA) has adverse effects on bones; however, the incidence of vertebral fractures (VFs) in patients with OSA remains uncertain. Additionally, there is an ongoing debate about whether VFs represent an early occurrence in OSA’s natural progression. This study aimed to assess the prevalence and determinants of VFs in patients newly diagnosed with OSA. METHODS: In this cross-sectional case–control study, we recruited 110 patients (74 male and 36 female) recently diagnosed with OSA (based on overnight polysomnography) and compared them with a control group of ind…

Background and Objective: Obstructive sleep apnea (OSA) is a sleep-related breathing disorder featuring a repeated closure of the upper airway during sleep. Craniofacial anatomy is a potential risk and worsening factor for OSA. This study aims to assess the relationship between cephalometric features of craniofacial morphology and OSA severity in a population of patients with OSA. Material and Methods: A sample of forty-two patients (n = 42, M = 76%, mean age = 57.8 ± 10.8) with a polysomnographically (PSG) confirmed diagnosis of OSA were recruited and underwent cephalometric evaluation of 16 …
