Reorganization of the innervation of the vas deferens after sympathetic decentralization.

Reorganization of autonomic efferent pathways to the rat vas deferens was noted after chronic (30 days) sympathetic decentralization produced by hypogastric nerve (HGN) transection. In normal rats, electrical stimulation of the HGN elicited an increase in vasal pressure (VP) bilaterally, whereas pelvic nerve (PN) stimulation did not alter VP. However, after unilateral HGN transection, stimulation of the PN on the transected side but not on the normal side increased VP. The decentralized vas exhibited larger VP responses to stimulation of the contralateral HGN in comparison with the normal vas.

Pain Relief in Short-Lasting Unilateral Neuralgiform Headache with Conjunctival inJection and Tearing Syndrome with Intravenous Ketamine: A Case Report.

PURPOSE: Short-lasting unilateral neuralgiform headache with conjunctival injection and tearing (SUNCT) is a rare form of primary headache, classified as trigeminal autonomic cephalalgia. Since the underlying mechanism of the pathogenesis has not yet been determined, a standardized therapeutic strategy for SUNCT is unavailable. We present a case of SUNCT syndrome with successful pain relief by intravenous administration of ketamine, an N-methyl-D-aspartate receptor (NMDAR) antagonist. CASE REPORT: A 56-year-old male patient reported severe throbbing and shooting pain in forehead, temporal and

Effect of transcutaneous auricular vagus nerve stimulation (taVNS) on inhibitory control as a function of motor task complexity.

This study examined the effects of transcutaneous auricular vagus nerve stimulation (taVNS) on inhibitory control during motor tasks of varying complexity. Motor task complexity was manipulated in a Simon task using two approaches: (1) movement complexity, contrasting a simple lifting action with a more complex reaching action, and (2) movement type selection, contrasting both actions performed either in separate blocks or intermixed within the same block (non-active vs. active conditions). Heart rate variability indices were also recorded to assess potential autonomic effects of taVNS. It was

[A possibility of central diffusion during stellate ganglion blockade: "the sheath of the spinal rachidian nerve"].

Stellate ganglion block is a common treatment for neuropathic pain. The technique is not without potentially severe complications when a paratracheal approach is used. A 33-year-old woman complained of atypical facial pain of 15 years' duration with pain intensity of 6 to 8 on a visual analog scale and no pain-free periods upon use of inadequate analgesia. One minute after performing a second stellate ganglion block the patient showed signs of apnea and paralysis of the upper extremities and face, with no involvement of oculomotor muscles or the lower extremities, and no loss of consciousness.

Cluster headache: diagnosis and treatment.

Cluster headache is a rare yet exquisitely painful primary headache disorder occurring in either episodic or chronic patterns. The unique feature of cluster headache is the distinctive circadian and circannual periodicity in the episodic forms. The attacks are stereotypic--they are of extreme intensity and short duration, occur unilaterally, and are associated with robust signs and symptoms of autonomic dysfunction. Although the pathophysiology of cluster headache remains to be fully understood, there have been a number of recent seminal observations. To exclude structural mimics, patients pre

Surgery for atrial fibrillation: recent progress and future perspective.

Development and introduction of radiofrequency ablation devices allowed the maze procedures to be performed safely and easily, further enabling off-pump pulmonary vein isolation through a mini-thoracotomy or thoracoscope. The effects of the maze procedure include prevention of stroke and other complications related to atrial fibrillation, improved cardiac performance, and relief of symptoms. Indications for the maze procedure have been discussed on the basis of the evidence. Pulmonary vein isolation has been shown to be effective in most patients with paroxysmal atrial fibrillation and can be

Reflex erection in the rat: reciprocal interplay between hemodynamic and somatic events.

BACKGROUND: Penile erection is a complex reflex under spinal control and modulated by the brain. The hemodynamic events under autonomic control and the perineal muscles somatic activity are interconnected during the reflex erection at the spinal level, however if the afferent feedback on the corpus cavernosum pressure during an erection affects the somatic activity (perineal muscles contractions) and vice versa is not known. This study was aimed to test this hypothesis using a rat model. METHODS: Intracavernous pressure (ICP) and bulbocavernosus (BC) muscle EMG were recorded during reflex ere

Scorpion venom increases acetylcholine release by prolonging the duration of somatic nerve action potentials.

Scorpionism is frequently accompanied by a massive release of catecholamines and acetylcholine from peripheral nerves caused by neurotoxic peptides present in these venoms, which have high specificity and affinity for ion channels. Tityus bahiensis is the second most medically important scorpion species in Brazil but, despite this, its venom remains scarcely studied, especially with regard to its pharmacology on the peripheral (somatic and autonomic) nervous system. Here, we evaluated the activity of T. bahiensis venom on somatic neurotransmission using myographic (chick and mouse neuromuscula

Whole mount microscopic sections reveal that Denonvilliers' fascia is one entity and adherent to the mesorectal fascia; implications for the anterior plane in total mesorectal excision?

BACKGROUND: Excellent anatomical knowledge of the rectum and surrounding structures is essential for total mesorectal excision (TME). Denonviliers' fascia (DVF) has been frequently studied, though the optimal anterior plane in TME is still disputed. The relationship of the lateral edges of DVF to the autonomic nerves and mesorectal fascia is unclear. We studied whole mout microscopic sections of en-bloc cadaveric pelvic exenteration and describe implications for TME. METHODS: Four donated human adult cadaveric specimens (two males, two females) were obtained from the Leeds GIFT Research Tissu

Case report: Diazepam in severe tetanus treatment.

The causes of death in tetanus are muscle spasms and spasm of the larynx, which are caused by blocking the release of inhibitory neurotransmitters in the spinal synapses, causing the uncontrolled spread of impulses. Diazepam controls the spasms by blocking the polysynaptic reflexes, working peripherally, without depressing the cortical center and has no cardiovascular or endocrine effects. High dose diazepam had been used and proved to be a good muscle relaxant. Diazepam seems to work better with tetanus than pancuronium bromide, but both drugs need mechanical ventilation. In cases where the d

Electrophysiologic study in benign human botulism type B.

Conventional electromyography (EMG) and single fiber EMG (SFEMG) were performed in a 64-year-old diabetic woman with mild type B botulism. The main clinical signs were autonomic dysfunction and cranial nerves paresis. Conventional EMG was normal, except for small changes that were consistent with mild mixed peripheral neuropathy in the lower limbs and were related to diabetes. Repetitive stimulation and results of single stimulus before and after full effort were normal. SFEMG showed increased jitter and impulse blocking in clinically normal muscles. The jitter was frequency dependent and impr

Techniques and results of neurolysis for chronic pancreatitis and pancreatic cancer pain.

Chronic abdominal pain can be associated with benign and malignant disease. Pain associated with pancreatic cancer and chronic pancreatitis can be severely debilitating, with significant impairment in quality of life. Frequently, chronic abdominal pain is not adequately responsive to conventional medical therapies, including nonsteroidal anti-inflammatory drugs and opioids. For this reason, alternative methods to alleviate pain have been developed. Celiac plexus neurolysis and celiac block involve injecting an agent at the celiac axis, with the goal of either selectively destroying the celiac

Mechanism of 'inappropriate' sinus tachycardia. Role of sympathovagal balance.

BACKGROUND: "Inappropriate" sinus tachycardia (IST) is an uncommon and poorly defined atrial tachycardia characterized by inappropriate tachycardia and exaggerated acceleration of heart rate with "normal" P wave. The mechanism of this tachycardia is unknown. The purpose of the present study was to determine the role of autonomic balance in the genesis of IST. METHODS AND RESULTS: Six female patients aged 23 to 38 years with IST and 10 age- and sex-matched control subjects were assessed with the following autonomic function tests: (1) sympathovagal balance to the sinus node assessed by calcula

Immunotherapeutic Protection From a Soman Nerve Agent Model Compound.

Chemical warfare agents include potent nerve agents (NAs). NAs covalently inhibit cholinesterases (ChEs) in the blood and in the central nervous system of animals administered NAs. An example of a very toxic NA is soman. After administration to animals, NA-ChE inhibition results and causes acetylcholine (ACh) to build up in the synapse where ordinarily only small amounts of ACh are present. This stimulates autonomic receptors and blocks neuromuscular junction receptors, causes severe neurological side effects including seizures and neuropathology and can result in death by respiratory failure.

Effects of aldose reductase inhibitor on microneurographically assessed peripheral sympathetic nerve activity in rats.

Autonomic neuropathy, one of the serious complications of diabetes, decreases quality of life. Aldose reductase inhibitor (ARI) blocks sorbitol production, and results in prevention of damage of nerve fibers. Beneficial effects of ARI have usually been confirmed through nerve conduction velocity tests in motor and sensory nerves. On the other hand, few reports have dealt with the effects of ARI on the small fiber activity such as sympathetic nerve one. In the present study, we administered eparlestat, ARI orally for 3weeks, to streptozotocin-induced diabetic (STZ+ARI) rats, and then recorded p

Botulinum Neurotoxins beyond Neurons: Interplay with Glial Cells.

In recent years, numerous studies have highlighted the significant use of botulinum neurotoxins (BoNTs) in the human therapy of various motor and autonomic disorders. The therapeutic action is exerted with the selective cleavage of specific sites of the SNARE's protein complex, which plays a key role in the vesicular neuroexocytosis which is responsible for neural transmission. The primary target of the BoNTs' action is the peripheral neuromuscular junction (NMJ), where, by blocking cholinergic neurons releasing acetylcholine (ACh), they interfere with neural transmission. A great deal of expe

Mitral isthmus ablation with pulsed field ablation technology through an epicardial approach with the CENTAURI system.

UNLABELLED: Mitral isthmus (MI) ablation is an essential part of persistent atrial fibrillation (AF) ablation, particularly in redo procedures where pulmonary vein isolation (PVI) is durable, but AF recurs. While radiofrequency (RF) ablation is commonly used, creating a complete MI line can be challenging and associated with significant risks. Pulsed field ablation (PFA) offers a safer alternative, providing tissue selectivity with fewer complications. This case report explores a hybrid endocardial and epicardial approach for PFA of mitral isthmus. Specifically, a 64-year-old woman with recurr

Cardiac Neuromodulation and Neurocardiology.

Neurocardiology has mostly been a specialty of medicine led by anatomists and physiologists. The characterization of the cardiac autonomic nervous system has resulted in a new understanding and appreciation of neurocardiology, leading to potential novel neuromodulation therapies in clinical cardiology and cardiac electrophysiology. Sympathectomy or spinal cord stimulation for the treatment of angina pectoris, as well as cardiac sympathetic denervation for the treatment of long QT syndrome associated with malignant ventricular arrhythmias, have been available and performed for more than half a
