Dissecting the shared genetic architecture of bipolar disorder, major depressive disorder, and attention-deficit hyperactivity disorder.

Major depressive disorder (MDD), bipolar disorder (BPD), and attention-deficit hyperactivity disorder (ADHD) are prevalent, highly heritable psychiatric disorders with significant degrees of genetic overlap. Using open-sourced summary statistics from the Psychiatric Genomics Consortium and 1000 Genomes European reference panel, we fit a latent factor model (F1) capturing the shared genetic liability across MDD, BPD, and ADHD. Multivariate GWAS identified 350 linkage disequilibrium-independent loci, 105 of which have not been previously reported by the contributing univariate GWASs. Univariate,…

Is a Genetic Variant associated with Bipolar Disorder Frequent in People without Bipolar Disorder but with Characteristics of Hyperactivity and Novelty Seeking?

OBJECTIVE: The objective is to verify whether a genetic condition associated with bipolar disorder (BD) is frequent in old adults adapted to their environment, without BD, but with aptitudes for hyperactivity and novelty seeking (H/NS). METHODS: In this cross-sectional study, the study sample included healthy elderly people (40 participants, aged 60 or older) living in an urban area and recruited from a previous study on physical exercise and active aging, who were compared with 21 old adults with BD from the same area. The genetic methodology consisted of blood sampling, DNA extraction, real…

Association between the so-called "activation syndrome" and bipolar II disorder, a related disorder, and bipolar suggestive features in outpatients with depression.

BACKGROUND: Activation syndrome (AS) is a cluster of symptoms listed by the US Food and Drug Administration as possible suicidality precursors during antidepressant treatment. We aimed to clarify whether AS is associated with bipolar II disorder (BP-II) and its related disorder, i.e., bipolar disorder not otherwise specified (BP-NOS), which are often mistreated as major depressive disorder (MDD), as well as bipolar suggestive features in outpatients with depression. METHODS: The frequency of AS, bipolar suggestive features, and background variables in consecutive outpatients with a major depr…

Convergent and Distinct Transcriptional Signatures in Suicide, Bipolar Disorder, and Major Depressive Disorder.

Globally, a person dies by suicide every 40 seconds, underscoring a major global public health concern. In the United States, suicide is the second leading cause of death among individuals aged 10-34 years. Suicide is a complex, multifactorial phenomenon influenced by genetic and epigenetic factors and is strongly associated with mood disorders, particularly major depressive disorder (MDD) and bipolar disorder (BD). However, the extent to which their molecular signatures converge or diverge across disorders and brain regions remains incompletely understood. To address this gap, we conducted a …

Binge Eating Disorder in Bipolar I Disorder: Prevalence, Childhood Trauma, and Clinical Correlates.

INTRODUCTION: Bipolar I disorder (BD-I) frequently co-occurs with eating disorders, particularly binge eating disorder (BED), which may adversely affect the clinical course, including recurrence risk and functional impairment. Research exploring the specific association between BED and childhood trauma (CT) in BD-I remains limited. This study aimed to investigate the prevalence of BED among euthymic BD-I patients and to examine its associations with CT and clinical characteristics. METHODS: This cross-sectional study recruited 150 euthymic BD-I patients diagnosed according to DSM-5 criteria.

Differentiation between bipolar disorder and major depressive disorder based on AMPA receptor distribution.

An accurate diagnostic method using biological indicators is critically needed for bipolar disorder (BD) and major depressive disorder (MDD). The excitatory glutamate α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor (AMPAR) is a crucial regulator of synaptic function, and its dysregulation may play a central role in the pathophysiology of psychiatric disorders. Our recently developed positron emission tomography (PET) tracer, [11C]K-2, enables the quantitative visualization of AMPAR distribution and is considered useful for characterizing synaptic phenotypes in patients with psych…

Comorbidity of bipolar disorder and borderline personality disorder: Phenomenology, course, and treatment considerations.

AIM: Bipolar disorder (BD) and borderline personality disorder (BPD) are both serious psychiatric conditions that elevate the risk for harmful outcomes. Although these conditions represent distinct diagnostic entities, existing research suggests that approximately 20% of individuals with BD meet the criteria for comorbid BPD. Individuals with comorbid BD/BPD appear to have a markedly more severe and phenomenologically distinct clinical course when compared with those with BD alone. However, treatments have generally not been tested in this specific population, and currently, no formal treatmen…

Differentiation Across Bipolar Disorder and Major Depressive Disorder by Whole-Night Polysomnographic Findings.

Abstract. Background: There is growing evidence that understanding the role of sleep disturbance in bipolar disorder (BD) and major depressive disorder (MDD) is helpful when studying the high heterogeneity of patients across psychiatric disorders. Objective: The present study was designed to investigate the transdiagnostic role of sleep disturbance measured by polysomnography (PSG) in differentiating from MDD with BD. Methods: A total of 256 patients with MDD and 107 first-episode and never medicated patients with BD using the Diagnostic and Statistical Manual of Mental Disorders, Fourth Editi…

Impact of comorbid obsessive-compulsive disorder on suicidality in patients with bipolar disorder.

This study evaluated the impact of comorbid OCD on suicide attempt risk and suicide methods in 990 patients with main diagnosis of BD. Two hundred and one patients (20.3%) had lifetime comorbid OCD. No significant differences were found comparing rates of lifetime suicide attempts between patients with or without comorbid OCD (30.3% vs 24.6%). In the subgroup of patients with concomitant OCD more subjects performed suicide attempts with violent methods (48.3% vs 28.7%). Therefore, our results suggest a correlation between comorbid OCD and violent suicide attempts. This finding is worthy of int

Suicide Risk in Comorbid Bipolar Disorder and Obsessive-Compulsive Disorder: A Systematic Review.

INTRODUCTION: The co-occurrence of bipolar disorder (BD) and obsessive-compulsive disorder (OCD) seemed to be a poor prognostic factor associated with greater disability, lower social and occupational functioning, poorer treatment response, and higher suicidal ideas and attempts compared to BD patients. MATERIALS AND METHODS: A systematic review was conducted on the risk of suicide in BD-OCD patients compared to BD patients. Relevant papers published through August 2018 were identified searching the electronic databases MEDLINE, EMBASE, PsycINFO, and the Cochrane Library. RESULTS: In all cas…

Clinical management of comorbid bipolar disorder and obsessive-compulsive disorder: A case series.

BACKGROUND: Apparent comorbidity between bipolar disorder (BD) and obsessive-compulsive disorder (OCD) is a common condition in psychiatry and it has important nosological and therapeutic implications. Although antidepressantsare the first line treatment for OCD, they can induce mood instability in BD. An optimal treatment approach remains to be defined. METHODS: Longitudinal clinical observation of three severe OCD patients who developed a manic episode during treatment with different classes of antidepressants. RESULTS: In our cases, three features support the hypothesis of an underlying b…

Thought Disorder in Schizophrenia and Bipolar Disorder Probands, Their Relatives, and Nonpsychiatric Controls.

Thought disorder (TD) has long been associated with schizophrenia (SZ) and is now widely recognized as a symptom of mania and other psychotic disorders as well. Previous studies have suggested that the TD found in the clinically unaffected relatives of SZ, schizoaffective and bipolar probands is qualitatively similar to that found in the probands themselves. Here, we examine which quantitative measures of TD optimize the distinction between patients with diagnoses of SZ and bipolar disorder with psychotic features (BP) from nonpsychiatric controls (NC) and from each other. In addition, we inve…

Diagnostic validity of comorbid bipolar disorder and obsessive-compulsive disorder: a systematic review.

OBJECTIVE: At least 50% of bipolar disorder (BD) patients have an additional diagnosis, one of the most difficult to manage being obsessive-compulsive disorder (OCD). Defining the nosology of BD-OCD comorbidity has important clinical implications, given that treatments for OCD can worsen BD outcomes. METHOD: A systematic review was conducted on: i) BD-OCD comorbidity lifetime prevalence and ii) on standard diagnostic validators: phenomenology, course of illness, heredity, biological markers, and treatment response. Relevant papers published through March 30th 2013 were identified searching th…

Bipolar disorder and gambling disorder comorbidity: current evidence and implications for pharmacological treatment.

BACKGROUND: The co-occurrence of bipolar disorder (BD) and gambling disorder (GD), though of clinical and public health importance, is still scarcely investigated. Comorbid BD-GD subjects experience a more severe course of illness and poorer treatment outcome, due to a range of clinical and psychosocial factors that collectively impede remission and recovery. The aim of our paper is to review the role of pharmacotherapy in the treatment of comorbid BD-GD, in order to support clinical decisions according to the best available evidence. METHODS: A qualitative systematic review of studies on pha…
