Continuous intragastric milk feeds in infants of low birth weight.

In a feeding trial 66 infants of low birth weight received continuous intragastric milk feeds from the fourth hour of life, starting with 60 ml/kg/24 hr and reaching a maximum of 300 ml/kg/24 hr on the ninth day. Each infant received only full-strength milk, which was either expressed human breast milk or SMA-S26 (a proprietary low-protein adapted cows' milk) or half-cream Regal milk (partly-skimmed evaporated cows' milk). For various reasons 10 babies had to be withdrawn, and the final assessment was made on the 56 who completed the trial successfully.Persistent vomiting was a problem in only…

Birth Weight and the Development of Functional Gastrointestinal Disorders in Infants.

PURPOSE: To assess the association between birth weight and the development of functional gastrointestinal disorders (FGIDs) in the first year of life. METHODS: This is a secondary analysis of a prospective cohort multicenter study including neonates, consecutively enrolled at birth, and followed up for one year. At birth all infants were classified by birth weight as extremely low (ELBW), very low, or low when <1,000, <1,500, and <2,500 g, respectively, and by birth weight for gestational age as appropriate (AGA, weight in the 10-90th percentile), small (SGA, weight <10th percentile), and la…

Impact of Birth Weight and Length on Primary Hypertension in Children.

BACKGROUND: A child's birth parameters not only enable assessment of intrauterine growth but are also helpful in identifying children at risk of developmental defects or diseases occurring in adulthood. Studies show that children born with a body weight that is small for their gestational age (SGA) are at a greater risk of hypertension though the inverse relation between excessive birth weight and the risk of primary hypertension in children is discussed less frequently. PURPOSE: To assess the impact of both birth weight and length on hypertension occurring in children aged 3-15 years. METHO…

Effects of smoking and preeclampsia on birth weight for gestational age.

OBJECTIVE: A counterintuitive interaction between smoking during pregnancy and preeclampsia on birth weight for gestational age (BWGA) outcomes was recently reported. In this report, we examine the relationship between these factors in a well-documented study population with exposure data on trimester of maternal smoking. METHODS: Preeclamptic (n = 238), gestational hypertensive (n = 219), and normotensive women (n = 342) were selected from live-births to nulliparous Iowa women. Disease status was verified by medical chart review, and smoking exposure was assessed by self-report. Fetal growth…

Low birth weight in the offspring of women with anorexia nervosa.

A growing body of literature has investigated the association between maternal anorexia nervosa and pregnancy outcomes. Infant low birth weight is associated with a number of neurodevelopmental and physical sequelae; however, consistent results on its association with maternal anorexia nervosa are scant. Therefore, a systematic review and meta-analysis of the existing literature were undertaken. PubMed, Embase, and PsychInfo were searched for studies comparing the mean birth weight of babies delivered by mothers with (a history of) anorexia nervosa against those of healthy mothers. Studies wer…

Adiponectin is present in cord blood but is unrelated to birth weight.

OBJECTIVE: In adults, adiponectin is reduced in association with excess adiposity, type 2 diabetes, and hyperinsulinemia. We assessed whether adiponectin was 1) present in the fetal circulation, 2) altered in the fetal circulation in the presence of maternal diabetes, and 3) had relations to fetal cord blood insulin or adiposity. RESEARCH DESIGN AND METHODS: We assessed adiponectin in cord blood in a large cohort of singleton offspring of diabetic mothers (ODM; n = 134) and control mothers (n = 45). RESULTS: Adiponectin was present in cord blood and, in ODM, was higher in those delivered at

The effect of maternal smoking on birth weight in twin pregnancies.

Information on 869 076 singletons and 17 566 twins, born during the period 1983-1991, was obtained from the Swedish Medical Birth Registry. Data on birth weight, gestational duration, vital status, and maternal smoking habits during pregnancy were analyzed in order to investigate whether twinning potentiates the effect of maternal smoking on birth weight and perinatal mortality. The individual birth weights were expressed as percentages of mean birth weight, where mean birth weights of singletons and twins were calculated separately. The birth weight reducing effect of maternal smoking was fou…

Reducing the frequency of low birth weight in the United States.

We sought to determine by what amount the frequency of low birth weight (LBW) could be reduced by eliminating its known, preventable risk factors. Based on the prevalence of risk factors such as cigarette smoking (exposure) in a population, and the degree of association (relative risk) between the risk factors and the outcome of LBW, we estimated the percentage of LBW in the United States attributable to the risk factors (population-attributable risk percentage). We found the following associations with known preventable LBW: Cigarette smoking accounts for 9.6%; alcohol consumption during preg…

The effect of cocaine abuse on birth weight and gestational age.

A retrospective study of 343 women who lacked prenatal care was conducted to ascertain the effect of recent cocaine abuse on birth weight and gestational age. All pregnant women admitted in labor to a large urban teaching hospital between January 1 and December 31, 1986 who had not received prenatal care were included. The charts of these women were evaluated to obtain information about medical and obstetric complications of pregnancy, labor and delivery, and birth weight and gestational age of the infant. Information about drug use was obtained by urine toxicology at the time of admission. Re…

A causal analysis of birth weight in the offspring of monozygotic twins.

Data were collected on the birth weights of 1,694 offspring of 385 sets of twins including 108 male and 131 female monozygotic pairs. To resolve the influence of birth order from the genetic, environmental, and maternal effects on birth weight, we analyzed the full-sib and maternal and paternal half-sib correlation matrices for birth orders one to five using a causal model that assumed each live-born child had an influence on the weight of the subsequent birth. Prenatal maternal influences explained 40% of the variation in birth weight of the first-born child and 52% for the fifth child; genet…

Comparison of current health, functional limitations, and health care use of young adults who were born with extremely low birth weight and normal birth weight.

OBJECTIVE: The objective of this study was to compare the current health status, physical ability, functional limitations, and health care use of extremely low birth weight and normal birth weight young adults. METHODS: A longitudinal study was conducted of a population-based cohort of 166 extremely low birth weight survivors (501-1000 g birth weight; 1977-1982 births) and a group of 145 sociodemographically comparable normal birth weight individuals. Current health status, history of illnesses, hospitalizations, use of health resources, and physical self-efficacy were assessed through questi…

Birth weight-specific mortality for extremely low birth weight infants vanishes by four days of life: epidemiology and ethics in the neonatal intensive care unit.

BACKGROUND: The persistent differences between those who question the appropriateness of aggressive resuscitative measures for many extremely low birth weight (ELBW) infants (birth weight < 1001 g) and those who generally initiate such treatment has been a source of ongoing tension for physicians, parents, judges, and policymakers. We believe that much of this tension may be a result of the way the issue is framed. We began this study with the intuition that although many ELBW infants die, most succumb quickly. Were this true, discussions that considered only survival rates might miss the poin…

Predicting mortality in extremely low birth weight infants: Comparison between gestational age, birth weight, Apgar score, CRIB II score, initial and lowest serum albumin levels.

We explored GA, BW, Apgar score, CRIB II score, and serum albumin levels as univariate predictors of mortality in extremely low birth weight infants. Medical records of 564 extremely low birth weight infants were reviewed retrospectively. The infants were grouped as survivors (group I), expired ≤ 7th postnatal day (group II), and expired > 7th postnatal day (group III). The predictive value for mortality of gestational age, birth weight, Apgar scores at 1 and 5 min, clinical risk index for babies II score, and first and lowest serum albumin levels was assessed by calculating the associated are…

Weight gain measured at 6 weeks after birth as a predictor for severe retinopathy of prematurity: study with 317 very low birth weight preterm babies.

BACKGROUND: Recent studies suggest that postnatal weight gain can play an important role in the development of retinopathy of prematurity (ROP). AIM: To analyze the low weight gain (WG) from birth to 6 weeks of life to predict the development of severe retinopathy of prematurity (ROP) among very low birth weight preterm babies (VLBW). METHODS: A prospective cohort study included 317 newborns with birth weight (BW) <or=1,500 g and gestational age (GA) <or=32 weeks. The main outcome was the development of severe ROP (defined as threshold ROP and higher stages of ROP). In all patients, the prop…

The relationship of birth weight and birth weight discordance to cerebral palsy or mental retardation later in life for twins weighing less than 2500 grams.

OBJECTIVE: This study tested the hypothesis that twins with a birth weight < 2500 gm who subsequently had cerebral palsy or mental retardation had a birth weight distribution or birth weight discordance distribution differing from that of the total live-born twin population also weighing < 2500 gm at birth. STUDY DESIGN: All twins born in Sweden between 1973 and 1980 (n = 5382) and having a birth weight < 2500 gm were identified by using information stored at the Medical Birth Registry, the National Board of Health and Welfare, Stockholm, or at Statistics Sweden. To identify twins with cerebr…
