A 71-year-old diabetic man presented with progressive cholestatic jaundice secondary to a Bismuth II hilar cholangiocarcinoma. Surgical exploration revealed a tumor confined to the biliary confluence. A limited en bloc resection of the confluence with multiple biliary reconstructions by end-to-side hepaticoduodenostomy was performed without hepatic resection. The postoperative course was uneventful. This case illustrates the feasibility of limited hilar resection with direct biliary reconstruction in selected frail patients when extended hepatectomy is contraindicated.

The triple combination of nizatidine, clarithromycin, and bismuth subcitrate resulted in an ulcer healing rate of 98% and an H. pylori eradication rate of 90%. Corresponding 'intention-to-treat' figures were 92% and 84%, respectively. These results suggest that further studies, shorter in duration, using lower dosages, and possibly testing other combinations with a double-blind methodology, are required.

Planar bismuth triamides with tunable stereoelectronics offer a versatile platform for studying the effects of geometric constraints in p-block elements. Here we show that exploring the low steric bulk regime in these compounds unlocks categorically new phenomena such as epitaxial hierarchical ordering, thermochromism, C─H/O activation, and 6-electron pincer-to-pincer redox transmetalation. These behaviors are not observed in sterically encumbered derivatives, emphasizing the role of steric modulation in unlocking new phenomena in geometrically-constrained main group complexes.
