OBJECTIVE: To describe three patients with giant cell arteritis (GCA) who suffered profound, irreversible visual loss due to delayed initiation or discontinuation of corticosteroid therapy pending results of histopathologic examination of temporal artery biopsy specimens and to review the appropriate management of GCA once the clinical diagnosis is made.
STUDY DESIGN: Case series.
SETTING: University-affiliated eye hospital.
PATIENTS: Three women with biopsy-proven GCA and bilateral visual loss due to delayed institution or discontinuation of corticosteroid therapy.

X-linked congenital stationary night blindness (XL-CSNB) is a nonprogressive disorder of the retina, characterized by night blindness, reduced visual acuity, and myopia. Previous studies have localized the CSNB1 locus to the region between OTC and TIMP on the short arm of the X chromosome. Drugs which affect the processing of vitamin A in the retina or pigment epithelium can cause ocular toxicity. It is shown here that the retinoic acids, which are used in the treatment of skin disorders and which cause night blindness, inhibit the ocular retinol dehydrogenases in an in vitro system. This is shown to lead to a decrease in the formation of the visual chromophore 11-cis-retinal, thus explaining why night blindness might occur.

A 49-year-old man presented with blurred vision 9 days following frontal head trauma. Visual loss progressed to bilateral blindness. Magnetic resonance imaging revealed hemorrhage and swelling within the optic nerves and chiasm. Indirect trauma to the anterior visual pathways may cause delayed blindness due to hemorrhage and edema within these structures.

The present paper describes a 62-year-old male patient with progressive night blindness and generalized peripheral neuropathy linked to HLA A29 and HLA B12. Multiple yellowish patches, radiating from the optic disk, were observed at the level of the retina, though they were not clearly outlined during angiofluorography. Blindness from misuse of over-the-counter (OTC) eye drop preparations is reported. This is a previously unreported category of visual loss. Four eyes were blinded in three patients who used OTC preparations when they suffered angle-closure glaucoma. Another eye suffered a reduction in vision from 20/20 to 20/200 when the patient treated a subluxed lens implant with OTC preparations for three months. During November 1985 a survey was carried out to determine the prevalence and causes of blindness in the Elim Hospital district of Gazankulu in the Northern Transvaal, South Africa, and to assess the Eye Department's effectiveness in preventing blindness. Using a random cluster sample technique, we screened 18,962 of the estimated 71,200 inhabitants of the district (26.6%). We found 109 blind people. Two siblings with autosomal-recessive congenital stationary night blindness were clinically blind in infancy. Both had markedly abnormal electroretinograms that, in the first child, led consultants at two university centers to make the diagnosis of Leber's congenital amaurosis. Diabetic eye disease is the leading cause of new cases of legal blindness in American adults under the age of 65 yr. Diabetic persons are at risk for visual loss due to diabetic retinopathy, glaucoma, and cataracts. Better understanding of the natural histories of these complications and recent advances in treatment have provided a rationale for developing an approach to prevent visual loss. This approach requires that diabetic patients who are at high risk for visual loss, and not under the care of ophthalmologists, be systematically screened, referred, and treated.

Visual loss is a rare and dreaded complication of surgical procedures of the paranasal sinuses. A case of sudden unilateral blindness following irrigation of the frontal sinus is presented and the presumptive pathophysiology is discussed. An awareness of this potential complication as well as early diagnosis with prompt medical and surgical management are essential.
