Measurement of aldosterone in blood.

A detailed method for measuring aldosterone in serum/plasma is described. Aldosterone is first extracted into dichloromethane to improve the specificity of the assay. A radioimmunoassay is performed on the dried extract using an antibody raised in rabbits to aldosterone conjugated at position C3, and iodinated aldosterone. Details are given for optimizing conditions for the second antibody, raised to rabbit immunoglobulins, and carrier protein used in the separation of the antibody bound fraction from the free aldosterone at the end of equilibration. Details are provided in additional notes, f

[Incidents of blood donation].

In France, more than two million blood donations are needed for transfusion each year. The rules and requirements for blood donors selection are strict and aim to protect the donors' health as well as the patients'security. However, the review of the literature shows that adverse effects of blood withdrawal occur quite frequently. They can be classified in immediate or delayed events, or local (hematoma, nerve injury, arterial puncture injury, allergy, veinitis, thrombosis) and general reactions (vasovagal reaction, citrate toxicity, cardiovascular failure, allergy to ethylene oxide, adverse e…

Statins and blood coagulation.

The 3-hydroxy-3-methylglutaryl (HMG)-coenzyme A (CoA) reductase inhibitors (statins) have been shown to exhibit several vascular protective effects, including antithrombotic properties, that are not related to changes in lipid profile. There is growing evidence that treatment with statins can lead to a significant downregulation of the blood coagulation cascade, most probably as a result of decreased tissue factor expression, which leads to reduced thrombin generation. Accordingly, statin use has been associated with impairment of several coagulant reactions catalyzed by this enzyme. Moreover,…

Risks of blood transfusion.

OBJECTIVE: To review the current status of risks of blood transfusion. DATA SOURCES, EXTRACTIONS, AND SYNTHESIS: English-speaking literature, literature search using key works, human data, and follow-up with key bibliographic citations. CONCLUSIONS: Substantial advances have been achieved in blood safety during the last 20 yrs, particularly for transfusion-transmitted viral infections. Currently, the most serious known risks from blood transfusion are administrative error (leading to ABO-incompatible blood transfusion), transfusion-related acute lung injury, and bacterial contamination in pl…

Leukoreduction of blood components.

During the past year, blood component therapy witnessed two quite contradictory trends in the area of leukoreduction. On the one hand, the year saw widespread forced implementation of leukoreduction by several national blood suppliers, including the American Red Cross, who refused to sell hospitals nonleukoreduced blood. The forced implementation came at high cost to hospitals and with the strong endorsement of the US Food and Drug Administration, which stopped short of mandating universal leukoreduction in the United States. On the other hand, the year saw the publication of several pivotal c…

Immunomodulation by blood transfusions.

Blood transfusions can affect the immune response in two opposite ways. They may either lead to immunization or to tolerance induction. Immunization is reflected by the induction of HLA alloantibodies and T cell activation while the induction of tolerance is suggested by the enhanced graft survival in transfused versus non-transfused recipients. The immunological mechanism leading to downregulation of the alloimmune response is not clear. One possible explanation is the induction of a Th2 response by non-professional antigen presentation by the transfused blood cells. On the other hand, eviden…

The bloodless blood knot.

Just as it is difficult to describe adequately the exhilaration one feels when using a fly rod to land a trout caught from a mountain stream, there is also a tremendous amount of satisfaction in the successful completion of an obstetric operation. Until recently, we were woefully ignorant of how fly fishing expertise could benefit pregnancy. We report with great pride an instance in which fly fishing knot skill was essential to successful placement of a cervical cerclage for a woman with an incompetent cervix.

Pharmacogenetics and blood dyscrasias.

Pharmacogenetic differences in the handling of and response to drugs can markedly alter the risk of severe idiosyncratic adverse drug reactions, including neutropenia, agranulocytosis and aplastic anaemia. Inherited deficiencies of drug metabolizing enzymes can shunt the metabolism of drugs to metabolites which are directly toxic (e.g. 6-mercaptopurine metabolism to 6-thioguanine nucleotides) or towards electrophilic metabolites which can kill cells and/or lead to a host immune response (e.g. sulphonamide metabolism to hydroxylamine metabolites). Defects in detoxification pathways (e.g. glutat

Retinopathy of blood dyscrasias.

BACKGROUND: Retinopathy often accompanies blood dyscrasias. METHODS: Three cases are presented: 1) a 75-year old patient with anemia of chronic disease, extensive posterior pole hemorrhaging and ischemic changes, 2) a 70-year old patient in whom retinopathy was the presenting sign of his chronic myelocytic leukemia and 3) a 56-year old patient with polycythemic retinopathy and papilledema. RESULTS AND CONCLUSIONS: These unusual cases are presented with a general discussion of retinal vascular manifestations of blood dyscrasia.

Physiology of blood coagulation.

Blood coagulation forms part of an integrated series of haemostatic reactions, involving plasma, platelet, and vascular components. Platelets adhere to damaged endothelium or to subendothelium under the influence of adhesive proteins, and when activated they aggregate and expose binding sites for coagulation factors. Platelets, therefore, act as vehicles to concentrate and potentiate coagulation reactions on the damaged vessels. Following interaction of the 'contact' factors XII and XI, the coagulation protease zymogens undergo sequential activation, resulting in the generation of thrombin, th…

Blood groups and filariasis.

Only a little is known about the studies done with filariasis in relation to blood groups. The present communication reports the results of a preliminary study carried out to investigate any relationship of ABO and Rho(D) blood groups in persons with circulating microfilariae (mf) in blood and with disease manifestations compared with healthy normal controls within a population in similar epidemiological and ecological conditions. Blood groups ABO and Rho(D) were investigated among 271 persons with filarial disease and 172 normal subjects from an endemic area of bancroftian filariasis. No rela…
