Volume replacement is critical to the resuscitation of the hemorrhaging patient, but this usually can be accomplished quickly and safely with crystalloid and/or colloid solutions. Red cells should be used in addition to asanguinous fluids in the treatment of tissue hypoxia due to anemia. The need for whole blood as opposed to packed red blood cells is controversial. However, plasma should not be used as a volume expander, and its use to supplement coagulation factors during the massive transfusion of red cells should be guided by laboratory tests that document a coagulopathy. Similarly, platel

Amniotic fluid embolism is an uncommon, but potentially lethal, complication of pregnancy. Because amniotic fluid embolism usually is seen with cardiac arrest, the initial immediate response should be to provide high-quality cardiopulmonary resuscitation. We describe key features of initial treatment of patients with amniotic fluid embolism. Where available, we recommend performing transthoracic or transesophageal echocardiography as soon as possible because this is an easy and reliable method of identifying a failing right ventricle. If such failure is identified, treatment that is tailored a

The leading cause of transfusion-related morbidity and mortality in the United States is transfusion-related acute lung injury (TRALI). Diagnostic criteria for TRALI have recently been developed and primarily consist of hypoxia and bilateral pulmonary edema occurring during or within 6 h of a transfusion in the absence of cardiac failure or intravascular volume overload. The primary differential diagnosis is transfusion-associated circulatory overload and differentiation can be difficult. Treatment is supportive with oxygen and mechanical ventilation. Diuresis is not indicated and the role of

INTRODUCTION: The immediate postinjury immune response contributes in a complex manner to trauma-induced coagulopathy. The necessary transfusion of blood components can further impact a patient's immune response, creating a poorly understood cycle of transfusion, inflammation, and coagulopathy. The purpose of this study is to define the relationship between large volume transfusion and variations in the immune response following injury.
METHODS: Utilizing the Pragmatic, Randomized Optimal Platelet and Plasma Ratios trial dataset, the time and volume of transfused blood products were compared

Transfusion-related acute lung injury, an acute life-threatening complication of blood component transfusion, can exhibit symptoms in neonates that are frequently misinterpreted as sepsis. The baby was discharged in good health after successful management after 19 days.

BACKGROUND: Complications from pregnancy, primarily post-partum hemorrhage, including infections like HIV, are the leading causes of maternal mortality in sub-Saharan Africa (SSA). Blood products are not always available for obstetric emergencies because of chronic shortages in SSA countries. Understanding the quantity of blood products used by pregnant and post-partum women with and without HIV infections could inform strategies for the appropriate use of blood in resource-limited settings.
METHODS: A prospective study was conducted of medical charts of all pregnant or postpartum (42 days fo

Seventy thalassaemics (B = 37, EB = 33) and 20 haemophilics (A = 18, B = 1, C = 1) were tested for antibodies to hepatitis C virus (anti-HCV) and markers for hepatitis B virus (HBV). The seropositivity for anti-HCV in thalassaemics and haemophilics was 14.3 and 25 per cent respectively. The subjects who were sero-positive to anti-HCV had had additional exposure to HBV. Anti-HCV positivity was not related to the age of the subject nor the number of units of blood and blood components transfused. Screening of blood donors for anti-HCV, apart from HBV, may minimise the hazards of post transfusion

Allergic reactions to intravenous, alteplase have been reported, usually in doses utilized for thrombolysis. Alteplase is also widely used for clearance of occluded catheter lumens, but allergic reactions have not been reported for this route of administration. A 14-year-old patient with sickle cell disease presented with an occluded apheresis port. During alteplase dwell for port occlusion, he developed an initial reaction concerning for anaphylaxis prior to automated red cell exchange, resulting in hospitalization. Findings included angioedema to the tongue and lip as well as urticaria on hi

Factor XI (FXI) deficiency is a rare bleeding disorder of unpredictable severity that correlates poorly with FXI coagulation activity and that poses great challenges for perioperative hemostatic management and the dialysis methods potentially available to new end-stage renal disease (ESRD) patients. We describe an individual with both ESRD and severe FXI deficiency, who successfully underwent peritoneal dialysis (PD) after emergency abdominal surgery. In the traditional concept, recent abdominal surgery is a contraindication to PD, especially for patients with bleeding risk. However, this case

Type 1 plasminogen deficiency is a rare genetic disorder. Type 1 plasminogen deficiency is characterized by fibrin-rich pseudomembrane formation on mucosal surfaces, particularly the conjunctiva. Tracheobronchial tree involvement is a less common reported manifestation of type 1 plasminogen deficiency. Pseudomembranes in the tracheobronchial tree may result in respiratory compromise and ultimately fail if not recognized and treated. Currently, there is no specific replacement therapy approved for the treatment of congenital plasminogen deficiency. In the present paper, we report that type 1 pl

INTRODUCTION: Red blood cells exchange transfusion has been demonstrated to be helpful in treatment of sever P. falciparum malaria. However, no large scale randomized controlled trials have been completed to date and the CDC does not recommend RBC exchange transfusions as standard of care. We present a case of severe cerebral malaria in a patient with extremely high parasitemia and severe altered mental status who improved rapidly with automated RBC exchange.
REPORT: Seventy-two year old female presented with 1 day history of weakness, altered mental status, malaise, and cyclic sweats after r

Factor VII deficiency is rare, with an estimated prevalence rate of 1/1,000,000. It is transmitted as an autosomal recessive trait. It can cause simple nosebleeds up to cerebral hemorrhage. Our study aims to focus on the clinical features and the importance of screening in patients with this rare deficit. We report the cases of two brothers with this deficit. Child aged 8 years, born to non-consanguineous marriage who was the youngest of two children. He had a history of post-circumcision bleeding and was admitted to our Department for the treatment of recurrent nosebleeds occurred over the la

BACKGROUND: Numerous recent reports have described the use of recombinant factor VIIa (rFVIIa) in non-hemophilia bleeding situations for achievement of hemostasis. However, its use in clinical situations other than hemophilia patients with inhibitors has been complicated by some reports of thrombotic events. rFVIIa has been used successfully to treat coagulopathic and/or bleeding neonates. The prevalence of thrombotic events in these neonates is completely unknown. This study was initiated to determine the risk of thrombotic events associated with rFVIIa use in neonates.

BACKGROUND: Few published data are available regarding perioperative blood usage in lung transplantation.
STUDY DESIGN AND METHODS: The medical records of all patients undergoing lung transplantation at a university medical center in 1994 and 1995 were reviewed.
RESULTS: Ninety patients underwent lung transplantation during this period. Six patients were excluded: two received a living related-donor lung, three underwent retransplantation and one underwent concomitant repair of a tetralogy of Fallot. Of the 84 evaluable patients, 59 underwent single lung transplantation and 25 double lung tr

BACKGROUND: Red blood cell transfusion is an important component of supportive care in selected patients with nonvariceal upper gastrointestinal bleeding (NVUGIB). Although current guidelines emphasize restrictive transfusion thresholds, the clinical impact of transfusion delay after the hemoglobin trigger is less clear. This study examined whether delayed transfusion after a hemoglobin value≤7.0g/dL was associated with adverse in-hospital outcomes in patients with NVUGIB.
METHODS: We conducted a retrospective cohort study of adult patients hospitalized with NVUGIB who reached a hemoglobin tr

Objectives: Previous studies have demonstrated an association between transfusion and increased organ dysfunction. We sought to determine the association between transfusion of blood components (red blood cell [RBC], plasma, platelet, and cryoprecipitate transfusions) with development of acute kidney injury (AKI) in children following the Fontan procedure. Methods: This is a single center, retrospective cohort study from 2009 to 2016. All children who underwent the Fontan procedure during the prescribed period who had transfusion and laboratory data available were included. Results: Eighty-eig

OBJECTIVE AND BACKGROUND DATA: Reduction in cellular elements of blood, secondary to hypersplenism is an established component of non-cirrhotic portal hypertension. Prior transfusion of blood or blood components is frequently required for safe surgical intervention. Due to thrombocytopenia, epidural catheter insertion for effective and durable analgesia is not possible. The aim of the present study was to objectively demonstrate the gain in blood components following early ligation of splenic artery for splenectomy in shunt surgery.
METHODS: From Jan 2008 to July 2010, 30 patients underwent e

BACKGROUND AND OBJECTIVES: The adrenaline-takotsubo-anaphylaxis-Kounis, or the ATAK complex, where there are clinical and pathophysiological overlaps between takotsubo and Kounis syndromes, in which histaminergic, adrenergic and other mediators may play roles, was recently described. The objective of this report was to describe three cases where the ATAK complex was suspected to have occurred after transfusion.
MATERIALS AND METHODS: Three cases were recently reported to the New Zealand Blood Service haemovigilance programme that appeared to have features in common suggestive of the ATAK comp

In 2002, transfusion-associated transmission (TAT) of West Nile virus (WNV) infection acquired through blood transfusion marked the emergence of a new threat to the U.S. blood supply. Although mosquito-borne transmission remains the predominant mode of WNV transmission, identification of TAT underscored the need for WNV screening of donated blood. In June 2003, blood-collection agencies (BCAs) implemented investigational WNV nucleic acid-amplification tests (NATs) to screen all blood donations and identify potentially infectious donations for quarantine and retrieval. This screening was perfor

OBJECTIVES: Blood transfusion is life-saving for patients experiencing acute blood loss and severe anaemia. In low-income and middle-income countries (LMICs), low blood donation rates and unavailability of whole blood and blood components (blood products) impairs timely blood transfusion. To fulfil patient-specific blood orders, a hospital blood transfusion service (HBTS) receives orders from a prescriber for blood transfusion, tests and prepares blood products for the patient. This study sought to describe the current state of LMIC HBTS.
DESIGN: A cross-sectional survey explored LMIC HBTS ac
