UNLABELLED: This systematic review sums up the state of knowledge about complications and reasons for failure when children with Blount disease are treated with temporary hemiepiphysiodesis. Twelve studies meet the inclusion criteria. The main reported obstacles were under correction and the poor predictability of the amount of correction that was obtainable. Obese patients with severe deformity are the most challenging groups of patients in the treatment of angular deformities. Patients with Blount disease require a careful evaluation regarding the deformity, age, body weight, and social back

Craniometaphyseal dysplasia is a rare genetic condition characterised by hyperostosis of the skull base and sclerosis of craniofacial bones. This can cause nasal obstruction. This paper presents the case of a 14-year old with craniometaphyseal dysplasia presenting with nasal obstruction successfully treated with turbinoplasty. A literature search was conducted using PUBMED and EMBASE. In conclusion, in cases of craniometaphyseal dysplasia with nasal obstruction conventional techniques such as submucosal diathermy and outfracturing of inferior turbinates may not be adequate. Bony turbinoplastie

Corrective osteotomy for recalcitrant varus deformity secondary to adolescent and infantile Blount's disease can be challenging because of a combination of severity of deformity, complexity of deformity, and frequent association with patient obesity. We present here the outcome of treatment by osteotomy and gradual deformity correction by circular external fixation in 31 patients with either infantile or adolescent Blount's disease. We used a unique classification scheme to quantify and qualify complications in this patient group: category I, complications not requiring an alteration in the tr

A 20-year-old woman was referred to our hospital with complaints of both knees due to patellofemoral pain. Osteopathia striata, a rare radiographic skeletal disorder, manifested by fine, linear striations primarily in the long bones, was a chance finding.

OBJECTIVE: Although arachnoid cysts and intracranial aneurysms are very common lesions, their association in the same patient is rare. We present a case of a middle cerebral artery aneurysm ruptured into an arachnoid cyst. We found only six cases with intracystic hemorrhage reported in the literature. The presence of an arachnoid cyst can mislead clinical presentation. The patient presented a paradoxically small temporal fossa and thickening of the temporal and sphenoid bone. The authors suggest that this uncommon association (arachnoid cyst, atypical cranial vault, and "mirror-like" cerebral

A skeletal dysplasia with previously unreported features is presented. Its evolution was characterized by growth abnormalities of bones without involvement of other organs. Advanced bone age, increased stature and irregular epiphyseal ossification with stippling of the main long bones were documented. Physeal overgrowth was massive in the left proximal humerus and femur. Furthermore, the hip joint appeared fused with an abundant mass of pathological calcific tissue extending from the femur to the ilium. Pathological epiphyses were characterized by anarchic cartilaginous proliferation with mult

The authors studied retrospectively 33 hips in 26 children at an average of 9 years 5 months after simultaneous open reduction and Dega transiliac osteotomy for developmental dislocation of the hip (DDH). All children were younger than 2 years of life at surgery. At the latest follow-up 89% of the clinical results and 72% of the radiographic results were rated excellent or good, despite a 45% rate of avascular necrosis observed before surgery. The authors attempted to identify factors influencing the final result. Given the low complication rate (3%) and low reoperation rate (3%), the authors

UNLABELLED: The diagnosis of mucopolysaccharidosis (MPS) VI is usually not made before the age of 2 or 3 years when the main clinical signs of dwarfed stature, skeletal deformities, coarsening facies, stiff joints and hepatosplenomegaly described in textbooks are recognisable. Conversely, accelerated growth with advanced bone age, a precocious feature of this condition although not suggestive of a storage disorder, is usually neither recognised, nor adequately interpreted. We report on two infants with MPS VI who presented with these two "unexpected" features within the 1st year of life.

OBJECTIVE: Recognition of prenatal-onset skeletal dysplasias has improved with advances in ultrasound imaging. Skeletal abnormalities can be recognized by two-dimensional (2D) ultrasound, but generating a precise diagnosis can be challenging. We aimed to determine whether three-dimensional (3D) imaging conferred any advantages over 2D imaging in these cases.
METHODS: We studied five women with fetuses of 16-28 gestational weeks referred for abnormal ultrasound skeletal findings. First 2D and then 3D sonography was performed and the results compared.
RESULTS: The pregnancies resulted in the f

BACKGROUND: Diastrophic dysplasia results in severe disproportionate short stature, generalized joint deformities, and early osteoarthritis. The knee joint often has an abnormal valgus position and is unstable, and degeneration of all joint compartments occurs, even during growth. The purpose of our study was to evaluate the clinical and radiographic results of total knee arthroplasty in a series of patients with diastrophic dysplasia.
METHODS: Between February 1992 and March 2000, twenty-one primary total knee arthroplasties were performed on fourteen consecutive patients with severe osteoar

Sclerosteosis is a rare genetic disorder of bone modelling, similar to, but distinct from, van Buchem disease; it has been described almost exclusively in Afrikaners of South Africa, a white population of Dutch ancestry. Isolated cases have been reported in a girl in Japan, a boy in Spain, and in multiracial families in Brazil and USA. Here we report a case of sclerosteosis in a black man born in Senegal. He presented with the full features of the disease: tall stature; syndactyly: nail dysplasia; massive sclerosis of the long tubular bones, the ribs, the pelvis and the skull; multiple cranial

The clinical, radiographic, and morphological findings in 25 cases of atelosteogenesis and boomerang dysplasia have been reviewed. The review confirms the nosologic grouping of atelosteogenesis type I with boomerang dysplasia and the clinical and radiographic overlap of features between atel- osteogenesis I and atelosteogene- sis II (synonymous with De la Chapelle dysplasia) and a group of patients with atelosteogenesis type III. A common pathogenesis is suggested for atelosteogenesis type I and boomerang dysplasia. A marked excess of male fetuses with boomerang dysplasia was observed. Atelost

Blount disease or idiopathic tibia vara is a developmental deformity of the proximal tibial medial physis that produces a varus deformity of the knee. It is classified by early or late onset. The etiology is not entirely clear but is presumed to be related to an abnormal compression on the medial aspect of the proximal tibial physis. Early diagnosis is important for a better prognosis. In general, the later the onset and the greater the progression of the deformity, the increased likelihood of articular changes. Orthotics are recommended as the first type of treatment. If progression occurs or

Adenosine deaminase (ADA) deficiency may manifest as severe combined immunodeficiency (SCID) in early infancy. Some of these children develop radiologic changes which may be in part related to effects of this enzyme deficiency on the bony epiphysis. We describe the radiologic changes in a neonate with ADA deficiency and their resolution with polyethylene glycol conjugated adenosine deaminase (PEG-ADA, ADAGEN: Enzon, Inc., South Plainfield, NJ) enzyme replacement therapy.

Orbital hypertelorism as defined by increased bony interorbital distance is a complex manifestation of many congenital anomalies. As such, it is associated with a multitude of orbital, adnexal, and surrounding soft-tissue deformities. Treatment of these problems requires extensive experience and a comprehensive team, permitting adequate preoperative evaluation, diagnosis, and treatment. The results obtained may be dramatic. The overall safety of the procedures has been demonstrated. Continued refinements and long-term follow-up will undoubtedly continue to make significant improvements.
