Controlled longitudinal bone growth by temporary tension band plating: an experimental study.

Permanent growth arrest of the longer bone is an option in the treatment of minor leg-length discrepancies. The use of a tension band plating technique to produce a temporary epiphysiodesis is appealing as it avoids the need for accurate timing of the procedure in relation to remaining growth. We performed an animal study to establish if control of growth in a long bone is possible with tension band plating. Animals (pigs) were randomised to temporary epiphysiodesis on either the right or left tibia. Implants were removed after ten weeks. Both tibiae were examined using MRI at baseline, and af

Bone transport over an intramedullary nail for reconstruction of long bone defects in tibia.

BACKGROUND: Although long bone defects may be treated by callus distraction, frequent complications arise from the long duration of external fixation. To reduce such complications, bone transport over an intramedullary nail (BTON) has been done for tibial bone defect. METHODS: In 12 patients (mean age, 46.1 years) of chronic osteomyelitis or bone defect, segmental transport was done using external fixator over an intramedullary nail. Prior to the index procedure, all patients had had serial debridements and five required myocutaneous or free flaps for covering of soft tissue defects. Using Me

Severe Discoloration of the Alveolar Bone Secondary to Long-Term Minocycline Therapy: A Case Report.

Minocycline, the synthetic derivative of the antibiotic tetracycline, has been used for a variety of medical treatments. One such use for minocycline is for acne vulgaris. Although widely used, minocycline has a common side effect of discoloration of tissues, including bone, skin, and mucosa. This case report presents a 19-year-old female patient with a history of long-term minocycline therapy for acne vulgaris who presented for periodontal esthetic crown lengthening. The initial exam revealed a blue-gray discoloration of the mucosa. Upon surgical exploration, it was discovered that the discol

Comparison of distraction epiphyseolysis and partial metaphyseal corticotomy in leg lengthening.

We have used Ilazorov's method of distraction epiphyseolysis for leg lengthening since 1977 and his new method of corticotomy or compactotomy since 1983. The first method was carried out in 22 lower limbs with an average lengthening of 8.25 cm (range 4 to 18 cm); included in this group are 2 patients with achondroplasia in whom both legs were lengthened 12 cm. In the second method, a corticotomy is carried out at the metaphyseal-diaphyseal junction followed by distraction in Ilazorov's apparatus (4 x 0.25 mm a day). We lengthened 30 legs; included are 5 patients who had both legs lengthened fr

Aetiology, diagnosis, and treatment of brachymetatarsia: a narrative review.

Brachymetatarsia (BM), or hypoplastic metatarsal, is an abnormal shortening of one or more metatarsal bones with a female-to-male ratio of 10.53:1. Different causes are described in the literature, such as congenital, acquired, or iatrogenic, associated with different conditions and syndromes. Its presence may develop deformity and pain; however, often feet are pain free and the major worries of patients are cosmetics. Non-operative treatments aim to improve the comfort of metatarsal heads and the possible dorsal conflict through comfortable shoes or the use of specific orthotics. The surgical

Small external fixation devices for the hand and wrist.

Small external fixators have been developed for use in the hand and wrist. The main indications are stabilization of open or infected fracture or non-union of the metacarpals and phalanges; length maintenance in segmental bone loss; and distraction-lengthening of the fingers. The device may be used also for articular or periarticular fracture, closed diaphyseal fracture in adults, fracture in children, various arthrodeses, or corrective osteotomies.

[X-ray changes in persons with facial skeleton deformities of the "long face" type].

The analysis of X-ray data suggested that the syndrome of "long face" could be caused by three types of changes: (1), preferential lengthening of lower jaw bone; (2), combination of lower micrognathia with increased vertical dimensions of middle facial zone; (3), prevailing anterior extension of the middle facial zone. In all the three groups the vertical dimensions of the chin region of mandible were frequently enlarged.

Does Adding Femoral Lengthening at the Time of Rotation Hip Transposition After Periacetabular Tumor Resection Allow for Restoration of Limb Length and Function? Interim Results of a Modified Hip Transposition Procedure.

BACKGROUND: Reconstruction after pelvic tumor resection of the acetabulum is challenging. Previous methods of hip transposition after acetabular resection have the advantages of reducing wound complications and infections of the allograft or metal endoprosthesis but were associated with substantial limb length discrepancy. We therefore developed a modification of this procedure, rotation hip transposition after femur lengthening, to address limb length, and we wished to evaluate its effectiveness in terms of complications and functional outcomes. QUESTIONS/PURPOSES: In this study, we asked: (

Latest Advances in Limb Lengthening Using Magnetically Controlled Intramedullary Lengthening Nails.

Magnetically controlled intramedullary lengthening nails (MCILN) have revolutionized the field of limb lengthening and deformity correction. They allow for accurate and precise distraction with excellent patient outcomes and satisfaction. Though potentially technically easier than external fixation, general deformity principles and bone and soft-tissue biology must be considered for successful use. MCILN can address deformities of a wide range of etiologies including congenital, posttraumatic, post-infectious, tumor, and many others with excellent healing rates and outcomes as well as better p

Bifocal femoral deformity correction and lengthening using a circular fixator construct in a dog.

A 7 mo old male rottweiler was evaluated for a right hind limb lameness caused by malunion of a Salter-Harris type II fracture. Radiographs and computed tomography (CT) revealed that the right distal femur had valgus, procurvatum, external rotation, and was 35% (70 mm) shorter than the contralateral femur. Distal femoral wedge ostectomies were performed to acutely correct the angular and rotational deformities. Lengthening of the femur was accomplished by distraction osteogenesis performed over 53 days at a second, proximal diaphyseal osteotomy using a circular fixator construct. This bifocal

If one looks at the final results obtained in children, one should conclude that replantation of large segments is more often indicated in c … for soft tissue assessment (skin retraction treatments, tendon lengthening, muscle sliding) or for bone lengthening. The final outcome being a functional arm, special care has to be taken in nerve repair integrated with possible secondary tendon transfers to compensate the functional deficit. With all these limitations in indications, care in emergency, and corre …

Clavicular Distraction Osteoplasty With Sternoclavicular Reconstruction for a Medial Clavicle Fracture: A Case Report.

CASE: An 80-year-old man with osteoporosis sustained a complex, comminuted medial clavicle (MC) fracture that failed nonoperatively. A novel reconstruction involved MC resection, midclavicle distraction osteoplasty (DO) with iliac crest bone graft, and sternoclavicular (SC) joint reconstruction using a semitendinosus tendon allograft. By 1 year, the patient achieved successful healing and functional recovery. CONCLUSION: Patients with excessive MC bone loss may require a clavicular lengthening to approximate the MC to the manubrium. Restoring clavicular length through a DO in conjunction with

Triple arthrodesis for adult acquired flatfoot.

The primary goal of triple arthrodesis for stage III and IV adult acquired flatfoot is to obtain a well-aligned plantigrade foot that will support the ankle in optimal alignment. Ancillary procedures including posterior muscle group lengthening, medial displacement calcaneal osteotomy, medial column stabilization, peroneus brevis tenotomy, or transfer and harvest of regional bone graft are often necessary to achieve adequate realignment. Image intensification is helpful in confirming optimal realignment before fixation. Results of triple arthrodesis are enhanced with adequate preparation of jo

Ipsilateral fibular transfer: a valuable option for treatment of large tibial defects in children.

Large tibial defects pose a great therapeutical challenge. Defects longer than 5 cm should be treated using various techniques with almost similar final results. Besides the method of bone lengthening and free fibular flap, there is also the possibility of reconstruction with ipsilateral fibular transfer. This method, mainly for children and adolescents, is now still an alternative with only few complications which should not be abandoned. We present the case of a 6-year-old boy with floating knee and tibial fracture with excessive bone loss which was solved by ipsilateral fibular transfer as …

A New Classification for Ankle Arthrodesis When Using an External Fixator.

BACKGROUND: We have classified ankle arthrodesis when using an external fixator into four types based on the deformity and defect. Each of the four types of technique have been evaluated retrospectively. MATERIALS AND METHODS: Thirty-three lower limb segments in 30 patients (average age 49 years) were treated by ankle arthrodesis using an external fixator in our institution. We classified the pre-treatment problems into four types and adjusted the surgical treatment accordingly: type I-no bone defect, no or mild deformity; type II-no bone defect, severe deformity; type III-bone defect with th

Prosthesis-free solution for below-knee amputations.

The Ilizarov fixator can be used as a bone regenerator to correct limb-length discrepancies in patients who require limb lengthening.A 17-year-old man presented with 12 cm of left lower-limb shortening and amputation at the left mid-forefoot region, which was the result of severe burns of the left lower limb at 2 years of age. The burns damaged the distal femoral, proximal, and distal tibial physes. Scarring around knee and leg acted as a bowstring and slowed normal longitudinal femoral and tibial bone growth. The patient wished to be prosthesis free. Limb lengthening was performed with the Il
