Biomechanical comparison of fixation methods on third-generation femoral composite bone models in Pauwels type 3 femoral neck fractures: Contribution of the medial buttress plate to fixation.

OBJECTIVE: To compare the use of cannulated screws in an inverted triangular configuration, medial buttress plates, and the combination of these 2 fixation methods in the treatment of Pauwels type 3 femoral neck fractures. METHODS: Twenty-eight anatomical composite third-generation femoral bone models were divided into 4 groups. The control group (group 1) was formed with 7 third-generation intact bone models. The fracture model was created with a 70° cutting block to fit 21 Pauwels type 3 fracture configurations. Seven models were fixed with an isolated 3.5 mm one-third semi-tubular medial b

Management of Short Proximal Femur Nail Peri-Implant Fracture of Femur Shaft with Implant Removal and Long Distal Femur Locking Plate and Bone Grafting - A Case Report.

INTRODUCTION: Giant cell tumors of the bone are aggressive and potentially malignant lesions. Juxtaarticular giant cell tumors of the lower end radius are common and is a challenge for reconstruction after tumor excision. Several reconstructive procedures like vascularized and non-vascularized fibular graft, osteoarticular allograft, ceramic prosthesis, and megapros thesis are in use for substitution of the defect in the distal radius following resection. Here, we have analyzed the results of aggressive benign Giant cell tumor of the distal radius treated by en bloc excision and reconstruction

Joint-Preserving Reconstruction of a First Metatarsal Aneurysmal Bone Cyst Using Fibular Strut Autograft and Mini-Fragment-Assisted Dual Plate Fixation: A Case Report With Functional Outcome.

BACKGROUND: Aneurysmal bone cyst (ABC) is a benign but locally aggressive osteolytic lesion that predominantly affects children and adolescents. Foot involvement is uncommon, with metatarsal lesions representing a rare subset. Reconstruction of the first metatarsal is particularly demanding due to its critical biomechanical role in forefoot load transmission and push-off during gait. CASE PRESENTATION: A 17-year-old male presented with right forefoot pain after trauma and was found to have a pathological fracture through an expansile lytic lesion of the first metatarsal. Initial conservative

Can locking plate fixation and free Vascularised fibular transfer with skin island achieve good functional outcome in the treatment of large bone defects of Tibia ? A study of 26 cases.

BACKGROUND: Despite the availability of multiple treatment options, management of tibial bone loss continues to be a challenge. Free vascularized fibula graft (FVFG) with a skin paddle offers better advantages over the other methods. We aimed to study the functional outcomes and QALY of patients with large tibial bone defects following FVFG with a locking plate. MATERIALS AND METHODS: We analyzed 26 consecutive patients with large tibial bone defects treated by free vascularized fibular graft (FVFG) and stabilization using a long locking plate between 2009 and 2018. All were followed up for a

Reconstruction After Hemimandibulectomy With a Plate and Soft-Tissue Free Flap Followed by a Titanium Mesh and Particulate Cancellous Bone and Marrow Harvested From Bilateral Posterior Ilia: A Case Report.

A particulate cancellous bone and marrow (PCBM) graft combined with titanium (Ti-) mesh tray has become one of the most popular mandibular reconstruction methods. The technique has been applied to the mandibular discontinuity defects after segmental mandibulectomy. To the best of our knowledge, there are no reports on using the technique after hemimandibulectomy, during which a wide mandibular resection, including the condyle, is performed. Here, we first report a case of mandibular reconstruction after hemimandibulectomy, using a plate and soft-tissue free flap, followed by a Ti-mesh and PCBM

Clavicular hook plate versus dog-bone technique for acute high-grade acromioclavicular joint dislocation: a retrospective cohort study comparing clinical outcome scores, complications, and costs.

INTRODUCTION: Arthroscopically assisted coracoclavicular (CC) ligament fixation techniques have been promoted as providing superior outcomes for the treatment of acute high-grade acromioclavicular joint (ACJ) dislocations. Nevertheless, there is a lack of high-level evidence for clinically relevant benefits. At our institute, orthopaedic surgeons use an arthroscopically assisted coracoclavicular ligament fixation technique (DB), while general trauma surgeons use a clavicular hook plate (cHP) technique. The aim of the study was to compare clinical outcomes, complication rates, and costs between

Fracture gap and working length are important actionable factors affecting bone union after minimally invasive plate osteosynthesis for the treatment of simple diaphyseal or distal metaphyseal tibia fractures.

INTRODUCTION: Indirect reduction of minimally invasive plate osteosynthesis (MIPO) can often result in delayed union in tibia fractures. This study evaluated several factors in MIPO in relation to bone union. HYPOTHESIS: We hypothesized that the fracture gap, plate - tibia distance, or working length would have a substantial effect on the tibia union rate. MATERIALS AND METHODS: Forty-one patients with simple diaphyseal or distal metaphyseal tibia fractures who underwent internal fixation surgery using the MIPO technique were divided into two groups: patients with delayed union and patients

Mandibular bi-directional distraction osteogenesis: A technique to manage both transverse and sagittal mandibular diameters via a lingual tooth-borne acrylic plate and double-hinge bone anchorage.

PURPOSE: Mandibular hypoplasia can develop transversely, sagittally, or in both diameters simultaneously. Current techniques achieve either sagittal or transverse expansion with different surgeries. Here, we present a novel method to obtain transverse and sagittal mandibular distraction in one stage. MATERIALS AND METHODS: The technique consists of a double osteotomy: a dento-alveolar osteotomy comprising four or six anterior teeth and a vertical symphysiotomy underneath. The mandibular basal bone is immediately expanded transversely and fixed to the lower symphysis via a miniplate carrying o

Auxiliary medial small locking plate fixation enhances bone healing and minimizes delayed union in inverted V-shaped high tibial osteotomy: A comparative study of 154 knees.

PURPOSE: To determine whether auxiliary fixation with a T-shaped small locking plate (SLP) placed on the medial tibial cortex can enhance bone healing and reduce delayed union compared with the conventional inverted V-shaped high tibial osteotomy (iVHTO) fixed with a locking compression plate (LCP). METHODS: This retrospective cohort study included 154 knees (135 patients) that underwent iVHTO. The knees were allocated to Group S (n = 72), treated with conventional single LCP fixation, and Group D (n = 82), treated with auxiliary medial SLP fixation in addition to the conventional procedure.

Evaluation of soft tissue and labial plate of bone stability with immediate implant in direct contact versus gap with socket shield: A randomized clinical trial with 1 year follow-up.

OBJECTIVE: In the anterior region, the resorption of the buccal plate of bone after tooth extraction leads to contraction of the overlying soft tissues, resulting in an esthetic problem. In the socket shield technique, the buccal root section of the tooth is maintained, to preserve the buccal bone for immediate implant placement. The aim of this randomized clinical trial was to investigate the effect of leaving a gap or not between implant and retained root fragment on bone dimensions and soft tissue esthetics. METHODS: This was a two armed parallel group randomized clinical trial with alloca

Mechanism of toxicity of TiO2 nanoparticles exposure on restraining bone growth of young rats: acting on HDAC9 nucleocytoplasmic translocation-mediated p53 deacetylation involving in growth plate chondrocyte differentiation and ferroptosis.

Excessive intake of Titanium dioxide nanoparticles (TiO2 NPs) in children may lead to abnormal development of cartilage growth plates. Elucidating the mechanism underlying the toxicity of TiO2 NPs on chondrocytes contributes to the prevention and clinical treatment of short stature in children. Herein, we found that TiO2 NPs inhibited chondrocyte proliferation and differentiation. Elevated levels of oxidative stress and activation of ferroptosis were observed in TiO2 NP-exposed chondrocytes. HDAC9 was downregulated in TiO2 NP-exposed chondrocytes, of which overexpression reversed TiO2 NP-media

Early stage prediction of bone regeneration using FEA and cell differentiation algorithms with 3D-printed PLA and PCL scaffolds: modeling and application to dorsal double-plating in distal radius fractures.

This study introduces an advanced framework that integrates biphasic cell differentiation bone remodeling theory with finite element (FE) analysis and multi-remodeling simulation to evaluate the performance of 3D-printed biodegradable scaffolds for bone defect repair. The program incorporates a time-dependent cell differentiation stimulus (S), accounting for fluid-phase shear stress and solid-phase shear strain, to dynamically predict bone cell behavior. The study focuses on polylactic acid (PLA) and polycaprolactone (PCL) scaffolds with diamond (DU) and random (YM) lattice designs, applied to

Clinical Outcomes of Isobar TTL System with Isthmic Bone Grafting and Pedicle Screw-Vertebral Plate Hook with Direct Repair of Defect for Lumbar Spondylolysis: A Matched-Pair Case Control Study.

OBJECTIVE: Although direct isthmic repair, such as PSVPH, did not affect the mobility of the fixed segment and adjacent segment, it has a relatively low rate of isthmic fusion compared with conventional fusion. The Isobar TTL dynamic internal fixation system has been widely used in clinical practice and has achieved satisfactory clinical results. However, the use of the Isobar TTL system in combination with direct isthmic repair for lumbar spondylolysis has rarely been reported. The aim of this study was to compare the clinical and radiologic outcomes between patients who underwent Isobar TTL
