Optimum design of a biodegradable implant for femoral shaft fracture fixation using finite element method.

Recent developments in biodegradable implant technology have expanded its use in several medical fields, such as orthopedics, cardiology, dentistry, and tissue engineering. Degradable bone-fixing implants, consisting of plates and screws, provide the advantage of completely degrading after efficaciously supporting the broken bone and can accelerate healing through nutrient release while maintaining mechanical stability. Magnesium alloys are considered promising options for bone implants owing to their natural degradability, biocompatibility, and potential to reduce long-term complications, but

Pilot study of oblique lumbar interbody fusion using mobile percutaneous pedicle screw and validation by a three-dimensional finite element assessment.

The purpose of this study was to try oblique lateral interbody fusion (OLIF) using percutaneous pedicle screws (PPS) with mobility. Twelve patients who underwent single-level OLIF were observed for at least one year. These included 6 patients with conventional PPS (rigid group), and 6 with movable PPS (semi-rigid group). Mobile PPS used cosmicMIA, which is a load sharing system. The anterior and posterior disc height, screw loosening and bone healing period, and implant failure were evaluated at final observation by CT. Moreover, the stress on the vertebral body-cage, on the vertebral body-scr

[Anchoring experiments on human cadaver vertebrae using various types of screws].

The firmness of the bone structure of vertebrae of human cadavers was investigated by turning screws of four different types into vertebrae T6 to L5 and subjecting them to axial tension up to the point of extraction. It was found that the cancellous bone screws (rather than the Zielke screw) had the best anchoring characteristics in bone tissue. The shearing fracture stress of the vertebral bone tissue was found to be approx. 1.80 N/mm2; the relation between the extraction forces and the individual vertebrae was also examined. In addition, the extraction forces were determined for screws inser…

Cementing stability: innovative polymethyl methacrylate-assisted salvage of mandibular reconstruction hardware.

Hardware exposure after mandibular reconstruction poses significant management challenges. Incomplete removal of reconstruction plates and screws may compromise stability, particularly when osseointegration prevents screw retrieval. A patient who underwent mandibular resection for odontogenic keratocyst with non-vascularized fibular reconstruction developed exposure of the fixation plate. During revision surgery, exposed screws were removed, but several screws remained firmly integrated within the reconstructed bone and could not be retrieved. To prevent destabilization, the plate was retained

Complications at extraction of the ASIF epiphysiodesis screws.

A retrospective study was performed of 30 consecutive patients operated on with ASIF epiphysiodesis for slipped capital femoral epiphysis. During extraction, complications were encountered in 13/23 patients. A total of 38 screws were removed. Thirteen screws broke and another three screws were embedded in bone, making extensive chiseling of cortical bone necessary before extraction was possible. Due to the frequent complications at extraction, the ASIF epiphysiodesis screw is not suitable for treatment of the slipped capital femoral epiphysis.

Radiographic features of magnesium-based bioabsorbable screw resorption in paediatric fractures.

BACKGROUND: Resorption of magnesium-based alloy bioabsorbable screws produces hydrogen gas, which can be mistaken as a sign of infection and may affect the physis or fixed bone fragment. OBJECTIVE: We evaluated the temporal and spatial occurrence of gas and the occurrence of a breakage of the fixed bone fragment or screw following magnesium screw fixation. MATERIALS AND METHODS: Radiographs of paediatric patients treated with magnesium screws were retrospectively reviewed. Temporal occurrence and distribution of gas in the bone, the physis and soft tissues, breakage of the screw or fixed bon

Surgical management of proximal splint bone fractures in the horse.

Fractures of Metacarpal and Metatarsal II and IV (the splint bones) were treated in 283 horses over an 11 year period. In 21 cases the proximal portion of the fractured bone was stabilized with metallic implants. One or more cortical bone screws were used in 11 horses, and bone plates were applied in 11 horses. One horse received both treatments. Complications of screw fixation included bone failure, implant failure, radiographic lucency around the screws, and proliferative new bone at the ostectomy site. Only two of the horses treated with screw fixation returned to their intended use. Compli…

A finite element study on femoral locking compression plate design using genetic optimization method.

The locking compression plate (LCP) and screw sets are widely used as internal fixator assemblies to treat long bone fractures. However, the surgeon's critical challenge is choosing the implant set (plate and screws) for each patient. The present study introduces a parametrized simulation-based optimization algorithm for determining an LC system with the best bone-implant stability. For this purpose, a three-dimensional fractured bone supported by an LC system was generated, and the discrete genetic optimization approach was utilized to design the optimum implant. Initially, an algorithm was d

[Morphology of the surgically treated dens fracture].

The peculiarities within the microarchitecture of the base of the dens have to be assumed as one of the underlying causes for type II and III fractures of the dens according to Anderson and D'Alonzo. As can be demonstrated in a section of the dens of a patient after primary osteosynthesis using two small fragment AO cancellous screws this leads to multiple local trabecular fractures and destruction of the cancellous microarchitecture. Further more compression by the screws results in a higher density of the bone mass in the region of fracture. Apart from the fact that two AO screws guarantee r

Effect of repeated cycles of steam sterilization on the integrity of cannulated surgical screws.

The process of repeated handling and steam sterilization of bone screws and allows debris to deposit on the surface and inside cannulated screws. Individually packaged screws could alleviate these potential problems. This research intended to evaluate the effect that multiple cycles of sterilization may have, if any, on the mechanical integrity of cannulated screws. The results indicate that the mechanical integrity of the screws tested was not compromised.

Production and repair of implant-induced microdamage in the cortical bone of goats after long-term estrogen deficiency.

SUMMARY: By using an ovariectomized goat model, we found that estrogen depletion decreases bone quality and makes it susceptible to screw-induced mechanical microdamage. Both diffuse microdamage and linear cracks accumulated up to 3 weeks after screw implantation, and the microdamage was repaired gradually after 4-8 months. INTRODUCTION: The aim of this study was to observe the effect of long-term estrogen deficiency on the creation and repair of microdamage in cortical bone adjacent to bone screw. METHODS: Cortical bone screws were placed in the tibial diaphyses 28 months after ovariectomy …

Rigid fixation and facial asymmetry.

The use of rigid fixation for the correction of facial asymmetry is in principle no different from its use in other forms of craniomaxillofacial deformity. The use of screws and plates demands more precise techniques than the use of wires. Attention to detail in passively fitting the plate to the defect and accurate insertion of the drill holes and screws enables the production of good occlusion and a symmetrical face. The precise osteotomies and bone grafts needed in the correction of all forms of skeletal asymmetry are as described for hemifacial microsomia or temporomandibular ankylosis. In

The effects of pedicle screw fit. An in vitro study.

STUDY DESIGN: The effects of pedicle screw size (major diameter and length) on fixation stiffness in osteoporotic and nonosteoporotic vertebrae were evaluated in vitro. METHODS: Lumbar vertebrae were obtained from two fresh frozen human spines. Bone mineral densities were determined using dual energy radiograph absorptiometry, followed by nondestructive mechanical testing of the specimens instrumented with pedicle screws. A loading technique was used that more closely mimics loading of pedicle screws in vivo. RESULTS: Testing revealed that for good quality bone, screw size had a significant

Effects of interference fit screw length on tibial tunnel fixation for anterior cruciate ligament reconstruction.

Graft-tunnel mismatch during arthroscopically assisted anterior cruciate ligament reconstruction using the central-third patellar tendon results in less than 20 mm of bone plug remaining in the tibial tunnel. We decided to evaluate the strength of bone plug fixation using interference fit screws that were less than 20 mm in length. Biomechanical testing was performed on 48 porcine hindquarters using 9-mm diameter interference fit screws that measured 12.5, 15, and 20 mm in length. No significant difference was noted between the different-length screws for insertion torque, divergence, stiffnes

Biomechanical in vitro assessment of screw augmentation in locked plating of proximal humerus fractures.

INTRODUCTION: Proximal humerus fracture fixation can be difficult because of osteoporosis making it difficult to achieve stable implant anchorage in the weak bone stock even when using locking plates. This may cause implant failure requiring revision surgery. Cement augmentation has, in principle, been shown to improve stability. The aim of this study was to investigate whether augmentation of particular screws of a locking plate aimed at a region of low bone quality is effective in improving stability in a proximal humerus fracture model. MATERIALS AND METHODS: Twelve paired human humerus sp

A novel acoustic emission screwdriver reduces surgeons´ cancellous screw stripping rate - A biomechanical study.

BACKGROUND: Orthopedic surgeons refine their torque sensitive skills from tightening cancellous screws. Still, experienced surgeons exhibit surprisingly high screw stripping rates in osteopenic cancellous bone. Whether Acoustic-Emission technology, detecting energy waves from microstructural damage during screw purchase, can reduce these rates is unclear. Our aim was to evaluate if surgeons, irrespective of their experience, reduced cancellous screw stripping rate by combining their skills with feedback from an innovative Acoustic-Emission screwdriver. METHODS: Thirteen orthopedic surgeons wi

Success rate of miniscrews relative to their position to adjacent roots.

The purpose of this study was to evaluate, histologically, root contact, proximity to a root, and proximity to marginal bone level as possible risk factors for the failure of mini-screws when inserted between neighbouring teeth. Twenty mini-screws were inserted into the mandible of five beagle dogs. Each dog received two bracket screw bone anchors in each lower quadrant, between the roots of the second and third, and third and fourth premolars. Every six weeks, apical radiographs were taken and vital stains were administered. Twenty-five weeks after insertion of the screws, the dogs were sacri
