A rodent model for investigating the neurobiology of contralateral neglect.

BACKGROUND: Contralateral neglect is a common and disabling sequela of right hemisphere strokes. Neglect involves attentional and cognitive deficits, including distortions of contralateral spatial and personal awareness. There are no established successful therapies for neglect, and treatment is often complicated by anosognosia. The disturbances associated with neglect are debilitating to patients and their families, and presence of neglect is a strong predictor of poor prognosis for recovery. OBJECTIVE: The present report reviews findings from 20 years of research using a rat model of neglec

Jaw opening and swallow triggering method for bilateral-brain-damaged patients: K-point stimulation.

The aim of this study was to confirm the response in patients stimulated at the trigger point (K-point). Since we have already clinically encountered patients with hyperactive bite reflexes who were able to open their mouth and swallow after stimulation of the trigger point, we investigated this response in other brain-damaged patients. The trigger point lies on the mucosa lateral to the palatoglossal arch and medial to the pterygomandibular fold at the height of the postretromolar pad. A total of 57 brain-damaged patients, including patients with pseudobulbar palsy due to bilateral upper moto

Current concepts in the pathogenesis of meningitis caused by Streptococcus pneumoniae.

In spite of improved antimicrobial therapy, bacterial meningitis still results in brain damage leading to significant long-term neurological sequelae in a substantial number of survivors, as confirmed by several recent studies. Meningitis caused by Streptococcus pneumoniae is associated with a particularly severe outcome. Experimental studies over the past few years have increased our understanding of the molecular mechanisms underlying the events that ultimately lead to brain damage during meningitis. Necrotic damage to the cerebral cortex is at least partly mediated by ischemia and oxygen ra

Stuttering may be a type of action dystonia.

We observed abnormal involuntary movements, involving principally the facial and neck muscles, in 23 patients with stuttering. These movements were similar to involuntary movements seen in distinct dystonic syndromes. There was a history of stuttering in the first degree relatives of six patients. The association of stuttering with degenerative neurologic disorders and focal brain lesions, cerebral blood flow changes in patients with developmental stuttering, its occurrence as a side effect of centrally acting drugs, induction and alleviation of stuttering by mechanical perturbation, or by ele

The frontal lobes and content-specific delusions.

Contemporary research has shown that delusions are often the product of identifiable neurologic disease, particularly when the delusions have a specific theme or are confined to one topic--monosymptomatic or content-specific delusions. Although these delusions are considered rare, some of them can be found at high rates in certain populations and settings. The literature on several classes of content-specific delusions (misidentification, sexual, and somatic) is critically reviewed. The review demonstrates that when adequate diagnostic workups are conducted, a high proportion of such delusions

Tourette's syndrome and right hemisphere dysfunction.

Poor performances of patients with Tourette's syndrome (TS) on tests requiring visual-perceptual abilities had led previous authors to the suggestion of right hemisphere involvement. We have compared the results of neuropsychological examination of 16 children with TS, with those of 16 children with a lesion of the right hemisphere. No evidence was found of a dysfunction of the right hemisphere in TS.

[Significance of intrapartum asphyxia for the onset of fetal brain damage].

The prevalence of cerebral palsy is around 0.2% and has remained constant during the last 30 years. Retrospective case-control studies do not show a clear correlation between perinatal asphyxia and the development of cerebral palsy. Less than 10% of all cerebral palsy cases show signs of severe asphyxia during labour and delivery as the major pathological and likely cause for the brain damage. Severe cases of birth asphyxia with multiorgan defects and signs of hypoxic-ischaemic encephalopathy have a high mortality and the risk of permanent brain damage is increased by a factor of 10 to 30. Ins

Are patients with constructional disorders different in visuo-spatial abilities?

Many authors hypothesized a linkage between the anatomical region of damage and the nature of constructional apraxia. In particular, the functional impairment of right brain damaged patients should consist in a visuo-spatial defect. However a large amount of researches have showed impaired visuo-spatial abilities in apraxic subjects respect to non-apraxics, but independently by the side of brain damage. In these researches constructional and visuo-spatial abilities have been often evaluated by methods not comparable among them. Aim of this paper is to verify the constructional performances of

Brain damage caused by hydrogen sulfide: a follow-up study of six patients.

Hydrogen sulfide (H2S) poisoning involves a risk of hypoxic brain damage. Six patients who lost consciousness due to H2S poisoning are described. The symptoms varied from anosmia in the patient with the shortest but highest exposure to delayed neurological deterioration in the patient with the longest exposure. The two patients with the most serious symptoms developed pulmonary edema, which may have prolonged the hypoxia. The patients were reexaminated 5 years or more after the poisoning. The five patients who had been unconscious in H2S atmosphere for from 5 to 15-20 min showed persisting imp

Changes in the basal dendrites of cortical pyramidal cells from alcoholic patients--a quantitative Golgi study.

Although a variety of pathological changes have been described in the brains of chronic alcoholic patients, there have been no studies which have addressed the question of alterations in cortical neuronal dendritic arborisation. Loss of neurons from the superior frontal gyrus and shrinkage of neurons from both the superior frontal gyrus and motor cortex has been documented in chronic alcoholic patients; these areas were chosen for this study. Using a modified rapid Golgi technique the basal dendritic arborisation of layer III pyramidal neurons was compared in 15 male alcoholic patients and 15

Cell population depletion associated with fetal alcohol brain damage: mechanisms of BAC-dependent cell loss.

Neuronal death is one of the most serious consequences of alcohol exposure during development. Studies described in this paper used a neonatal rat model to address factors affecting neuronal death following alcohol exposure during the period of rapid brain growth, and relate them to possible mechanisms of damage. The profile of blood alcohol concentrations (BACs) is an important variable influencing both brain growth deficits and neuronal death--a smaller daily dose of alcohol can be more damaging than a larger daily dose, if it is consumed in a binge-like pattern that produces relatively high

Constructive disabilities in focal brain-damaged patients. Influence of hemispheric side, locus of lesion and coexistent mental deterioration.

The performance of 185 focal brain-damaged patients on two non-verbal constructive tasks ('Copying Drawings' and 'Copying Drawings with Landmarks') was evaluated with regard to three variables: laterality of cerebral lesion, intrahemispheric locus of lesion, and coexistence of mental deterioration. The aim of the study was to assess the relationships between each of these variables and both the incidence and severity of constructional apraxia. Different evaluations were carried out in order to partial-out the possible interference of unilateral spatial neglect. Results showed that, regardless

Neglect in a patient with partial callosal disconnection.

A right-handed woman suffered destruction of the anterior two thirds of the corpus callosum and the adjacent medial frontal lobes as a sequel of bleeding from an anterior cerebral artery aneurysm. When drawing and writing with her right hand, the patient displayed neglect of the left hemispace. In line bisection each hand deviated towards the side ipsilateral to the lesion, but the size of error decreased when the lines were placed to the ipsilateral side of the body. When tested for her verbal report of tachistoscopically presented, horizontal pairs of simple geometrical figures, she showed a

[Oculomotor and visual sensory changes in the neurologically lesioned child].

Pathological changes in the visual system are often present in children with neurological handicaps. Strabismus, refractive errors, amblyopia, optic atrophy are the more frequent findings. The ophthalmological evaluation in these children includes: eliciting of pupillary light reflex; occlusion test; cover-test; observation of symmetry of corneal light reflexes; retinoscopy and ophthalmoscopy. Strabismus is the most frequent alteration. The so-called "caotic" strabismus, with very variable angle of squint and abnormalities of the vertical movements, is peculiar in children with neurological ha

The clinical utility of reproduction drawing tests with low IQ patients.

Bedside mental status testing in low IQ, poorly educated patients is often difficult. These patients, who comprise a significant proportion of the population served by major inner city public hospitals, have a limited ability to perform adequately on many higher cortical function tests. Because of this, it is often difficult to accurately differentiate between an organic brain syndrome and the effect of depressed intelligence in these patients. Copying line drawings and other constructional tasks are a very important element in a complete mental status examination, as they are frequently faile
