Robot-assisted stereotactic brainstem biopsy in children: prospective cohort study.

Tumours located within the brainstem comprise approximately a tenth of all paediatric brain tumours. Surgical biopsy of these tumours is technically challenging and has historically been associated with considerable risk. To this end, robot-assisted surgery theoretically allows for increased accuracy and precision. In this study we report our experience using the Neuromate robot (Renishaw, Gloucestershire, UK) to perform robot-assisted stereotactic biopsy in children with tumours located within the brainstem. An uncontrolled prospective cohort study was performed (phase II) according to the ID

Cripto as a target for improving embryonic stem cell-based therapy in Parkinson's disease.

Embryonic stem (ES) cells have been suggested as candidate therapeutic tools for cell replacement therapy in neurodegenerative disorders. However, limitations for the use of these cells lie in our restricted knowledge of the molecular mechanisms involved in their specialized differentiation and in the risk of tumor formation. Recent findings suggest that the EGF-CFC protein Cripto is a key player in the signaling pathways controlling neural induction in ES cells. Here we show that in vitro differentiation of Cripto(-/-) ES cells results in increased dopaminergic differentiation and that, upon

Improving outcomes for neurofibromatosis 1-associated brain tumors.

Children and adults with neurofibromatosis type 1 (NF1) are predisposed to developing CNS tumors, including optic pathway gliomas (OPGs), brainstem gliomas (BSGs) and high-grade gliomas. Although current first-line treatments for low-grade gliomas (OPGs and BSGs) may prevent further tumor growth, they rarely result in restoration of the associated visual or neurological deficits. The availability of accurate small-animal models of NF1-associated brain tumors has established tractable experimental platforms for the discovery and evaluation of promising therapeutic agents. On the basis of these

Intracranial neoplasms.

CT has made possible the diagnosis of intracranial neoplasms with greater precision than other currently available neurodiagnostic studies. Because it is not always possible to determine the precise pathologic features of these neoplasms without angiography and surgical biopsy, over-reliance on CT should be avoided.

Comparative Value of 2-Hydroxyglutarate-to-Lipid and Lactate Ratio versus 2-Hydroxyglutarate Concentration on MR Spectroscopic Images for Predicting Isocitrate Dehydrogenase Mutation Status in Gliomas.

PURPOSE: To compare the ability of 2-hydroxyglutarate (2HG)-to-lipid and lactate (2HG/[lipid + lactate]) ratio with the ability of 2HG concentration alone to predict the isocitrate dehydrogenase (IDH) mutation status in patients with glioma. MATERIALS AND METHODS: In this retrospective study, consecutive patients with histopathologically proven glioma were enrolled between July 2016 and February 2019. A total of 79 patients were enrolled (mean age, 44 years; 49 men). The 2HG concentration and other MR spectroscopic parameters were measured by single-voxel point-resolved spectroscopy before su

Expression of Idh1R132H in the Murine Subventricular Zone Stem Cell Niche Recapitulates Features of Early Gliomagenesis.

Isocitrate dehydrogenase 1 mutations drive human gliomagenesis, probably through neomorphic enzyme activity that produces D-2-hydroxyglutarate. To model this disease, we conditionally expressed Idh1R132H in the subventricular zone (SVZ) of the adult mouse brain. The mice developed hydrocephalus and grossly dilated lateral ventricles, with accumulation of 2-hydroxyglutarate and reduced α-ketoglutarate. Stem and transit amplifying/progenitor cell populations were expanded, and proliferation increased. Cells expressing SVZ markers infiltrated surrounding brain regions. SVZ cells also gave rise to

Exploitation of genetically modified neural stem cells for neurological disease.

The successful treatment and potential treatment of the central nervous system (CNS) pathology remains the most challenging frontier in medical science. The clinical modalities presently available are mostly of limited efficacy and with the aging population, neurodegerative diseases and CNS neoplasms are increasingly prevalent. Neural stem cells (NSCs) have provided optimism for the horizon of therapeutic progress in treating neurological diseases. These mutipotent (able to differentiate into neurons, astrocytes and oligodendrocytes) cells can be obtained directly from the CNS or derived from

[Audiovestibular exploration of the brain stem (author's transl)].

In the context of a postgraduate lecture for specialist physicians, the author describes techniques of objective audiology used on a daily basis in the exploration of the brain stem in the ENT department of Lariboisiere Hospital. The following are reviewed for each of them: --the corresponding anatomical pathways, --the principles and techniques used. The results are illustrated by examples. Increase in the threshold of stapes acoustic reflex above 95 dB on a frequency of 1,000 Hz is highly suggestive of acoustic nerve involvement or the lower part of the brain stem (bar sign). By contrast, th

Intraparenchymal clear cell meningioma of the brainstem in a 2-year-old child. Case report and literature review.

We report an unusual case of an intraparenchymal clear cell meningioma of the brainstem, occurring in a 22-month-old girl. She presented with bulbar dysfunction and a right hemiparesis due to an intrinsic tumor of the medulla, which was confirmed by radiologic imaging to be focal and with an exophytic component. At surgery, a partial resection was achieved and no dural attachment was found. Pathologic examination revealed a clear cell meningioma. In reviewing the literature, there have been fewer than 20 reported cases of clear cell meningioma, none of which were intraparenchymal, involved the

The tentorium in axial section. II. Lesion localization.

Most juxtatentorial lesions may be localized accurately on contrast-enhanced axial section CT scans by use of the opacified tentorial bands. Lesions that lie lateral edge of the diverging bands are supratentorial. Lesions that lie medial to the V-shaped tentorial bands are infratentorial and/or incisural. Flattening of the tentorial border of a lesion helps to identify its location. Use of the tentorial bands identifies transincisural extension of meningioma reliably, but does distinguish well between true transtentorial growth of meningioma and marked upward bulging of the tentorium from pure

Brainstem auditory evoked potentials and blink reflexes in patients with pontocerebellar angle tumors.

Brainstem auditory evoked potentials (BAEP) and blink reflexes (BR) were obtained from 13 patients with pontocerebellar angle (PCA) tumors whose pathological diagnosis was as follows: 7 acoustic neurinoma, 3 meningioma, 1 neurinoma, 1 brain stem epidermoid tumor, 1 arteriovenous malformation. The most prominent abnormality was noted in BAEP generated by stimulating the ear ipsilateral to the lesion and ipsilateral R1 reflexes obtained from ipsilateral stimulation of the supraorbital nerve.

[Eye findings in paediatric brain tumour - data basis for a follow-up proposal].

BACKGROUND: Morphologic alterations and functional disturbances of the visual system are frequently observed in children with brain tumours. At present we are not aware of published data giving recommendations for ophthalmic follow-up in these patients. PATIENTS: A retrospective analysis of 125 children with a brain tumour was done to perform a risk analysis. METHOD: According to a sequential test strategy risk factors for various eye findings have been evaluated. RESULTS: Tumour location is the major risk factor for ophthalmological findings. Eye motility pathology was mainly associated wi

Role of endoscopic third ventriculostomy in patients undergoing resection of pulvinar area lesions: Preliminary clinical results.

BACKGROUND: Patients with pulvinar area lesions may develop hydrocephalus at any stage. The role of endoscopic third ventriculostomy (ETV) in this setting remains unclear. METHOD: We retrospectively enrolled 15 patients with a mean age of 43 years who underwent endoscopic resection of pulvinar area lesions using the supracerebellar infratentorial approach (SCITA). We compared the different modalities of hydrocephalus management and their outcomes. RESULTS: Nine of 15 patients (60.0%) had preoperative obstructive hydrocephalus. Five patients underwent ETV before tumor resection, and none deve
