Successful pregnancy outcome in a woman with untreated ectopia vesicae: a case report and review of literature.

We report the case of a pregnancy in a 25-year-old woman who was born with ectopia vesicae and split pelvis, but had not undergone any reconstructive or diversion surgery in childhood. Her antenatal period was uneventful and the infant was delivered by cesarean section at term due to breech presentation. The baby had no congenital anomalies. The postoperative period was uneventful and they were discharged from the hospital in a good general condition.

[Epidural anesthesia for labor].

Since the last decade, lumbar epidural analgesia has gained widespread use in obstetrics. Approximately 80% of parturients receive epidural analgesia for labour and vaginal delivery as well as caesarean section in most centres. There is little doubt that the most successful application of epidural analgesia during labour, considered by more than 75% of primiparas as extremely painful. The quality of analgesia is far superior to that which can be achieved by either parenteral or inhalation approaches and, unlike these methods, the mother remains alert. Epidural analgesia also prevents, or great

[Unusual obstetrical situations: role of locoregional anesthesia].

Breech or twin delivery is a difficult obstetric situation in which both maternal and neonatal risks are increased and where the incidence of caesarean section and instrumental extraction is high. Epidural anaesthesia may worsen the obstetric situation especially if high doses of local anaesthetics are used resulting in a dense motor blockade. If, on the contrary, effective analgesia is obtained by the epidural injection of low concentrations of local anaesthetics especially with opiates, epidural anaesthesia may increase neither the duration of the second stage nor the incidence of caesarean

Is cesarean section necessary for delivery of a breech first twin?

Eighty-two pregnancies involving a breech first twin were retrospectively analyzed over two study periods during which the cesarean section rates were 20.9 and 94.9%, respectively. The neonatal and maternal morbidity in both study periods was compared. There were no neonatal deaths during either study period and no cases of interlocking twins among patients delivered vaginally. Neonatal morbidity was unrelated to the mode of delivery. The incidence of maternal fever was, however, significantly higher in the cesarean section group compared with the vaginal delivery group (p < 0.001). Our study

Symphysiotomy for the trapped aftercoming parts of the breech: a review of the literature and a plea for its use.

The most dreaded complication of vaginal breech delivery is entrapment of the aftercoming head. When this is due to disproportion, persistent attempts at vaginal extraction are likely to result in a dead or damaged baby. A largely unknown solution in this desperate predicament is to surgically enlarge the pelvis by means of a symphysiotomy. A review of the literature shows that symphysiotomy performed to free the trapped aftercoming head will save at least 80% of babies if the procedure is performed without delay. Every obstetrician should be prepared to perform a symphysiotomy if the aftercom

[Neonatal spinal cord injuries and magnetic resonance imaging].

Two cases of spinal cord injury involving the lower cervical region are reported. In both, the delivery of a breech presentation was difficult. Severe hypotonia, areflexia and autonomic nervous system disturbances suggested a spinal cord injury. The evidence, type, and location of the cord lesion were given by nuclear magnetic resonance (NMR).

Erb/Duchenne's palsy: a consequence of fetal macrosomia and method of delivery.

Methods of delivery, maternal and neonatal characteristics were examined to determine their role in the occurrence of Erb/Duchenne's palsy. Data from 210,947 Washington state birth certificates from 1980 through 1982 were examined. The incidence was 50.2 cases of Erb's palsy per 100,000 live births. A case control study design was used to analyze 106 cases and 386 controls by both univariable and multivariable analysis. Birth weight was shown to be a significant risk factor regardless of which method of delivery was used. A high birth weight infant (4001-4500 g) had 2.5 times the risk of incur

[Acute peripheral circulatory disorders with gangrene in infancy].

Two cases of acute venous circulation disorders of the lower extremity are reported. During shock treatment including rehydration and additional application of heparin the circulation disorders were quickly resolved. As etiological factors, difficult delivery due to incomplete breech-footling presentation (case 1) and hypersodium dehydration (case 2) are discussed. Compared with older children infants might have a better prognosis when acute circulation disorders occur.

[Unexpected risks during labour and delivery following a low risk pregnancy (author's transl)].

Of the maternity patient of the Munich perinatal study from 1975-1977 54.4% were considered to be low risk at the end of the pregnancy. Of these every third patient (17%) developed at least one risk during labour and delivery. These were considered to be consequence risks if they and been present at the onset of labour and delivery or progress risks if they developed during labour and delivery. The progress risks were defined as unpredictable and occurred in 4.7% of the total study. In this group the incidence of spontaneous delivery was only 36.0% compared with 88.5% in the compare group. Comparison of Tocolytic Agents for Successful External Cephalic Version: A Bayesian Network Meta-Analysis of Sixteen Randomized Controlled Trials.

BACKGROUND: Cesarean section is frequently performed for breech presentation; however, external cephalic version (ECV) is recommended as an alternative strategy to increase the likelihood of vaginal birth. Tocolytic agents are commonly administrated to improve ECV success, yet the comparative effectiveness of different regimens remains inadequately characterized. AIMS: To systematically evaluate and compare the efficacy and safety of various tocolytic agents in facilitating successful ECV through a Bayesian network meta-analysis. STUDY DESIGN: Bayesian network meta-analysis. METHODS: Bayesi…

Face-to-pubes rotational maneuver for bilateral nuchal arms in a vaginal breech birth, resolved in an upright maternal position: A case report.

BACKGROUND: A physiological breech birth is one in which the woman is encouraged to remain active throughout her labor and able to assume the position of her choice for the birth. Use of this strategy within the United Kingdom National Health Service has led to the use of innovative maneuvers to relieve obstruction when women give birth in upright positions, for example, kneeling or standing. This includes use of the face-to-pubes rotational maneuver to relieve extended nuchal arm(s). In this paper, we report a case where the face-to-pubes rotational maneuver was used to relieve bilateral nuch

Expertise in physiological breech birth: A mixed-methods study.

BACKGROUND: The safety of vaginal breech birth depends on the expertise of birth attendants, yet the meaning of "expertise" remains unclear and subjectively defined. The objective of this study was to define expertise and the roles experts may play in expanding access to this service. METHODS: We performed an integrative analysis of two strands of data concerning expertise in physiological breech birth, including the following: survey data from a Delphi study involving 26 very experienced clinicians (mean experience = 135 breech births) and 2 service user representatives, and interviews from

The Cost Implications in Ontario, Alberta, and British Columbia of Early Versus Delayed External Cephalic Version in the Early External Cephalic Version 2 (EECV2) Trial.

OBJECTIVE: According to the Early External Cephalic Version (EECV2) Trial, planning external cephalic version (ECV) early in pregnancy results in fewer breech presentations at delivery compared with delayed external cephalic version. A Cochrane review conducted after the EECV2 Trial identified an increase in preterm birth associated with early ECV. We examined whether a policy of routine early ECV (i.e., before 37 weeks' gestation) is more or less costly than a policy of delayed ECV. METHODS: We undertook this analysis from the perspective of a third-party payer (Ministry of Health). We appli…

An observational study of the success and complications of 2546 external cephalic versions in low-risk pregnant women performed by trained midwives.

OBJECTIVE: To evaluate the success of an external cephalic version (ECV) training programme, and to determine the rates of successful ECV, complications, and caesarean birth in a low-risk population. DESIGN: Prospective observational study. SETTING: Primary health care and hospital settings throughout the Netherlands (January 2008-September 2011). POPULATION: Low-risk women with a singleton fetus in breech presentation, without contraindications to ECV, were offered ECV at approximately 36 weeks of gestation. METHODS: Data were collected for all ECVs performed by midwives, and were entered…

[Vaginal delivery versus cesarean section for term breech delivery].

BACKGROUND/AIM: The optimal method of delivery for breech presentation at term still remains a matter of controversy. This is probably due to the fact that the skills of vaginal breech delivery are being lost. The aim of this study was to examine risk factors: mother's age, parity, labor's duration, estimated neonatal birth weight for the mode of breech presentation delivery at term as well as the influence of the delivery mode on neonatal outcome. METHODS: A retrospective study of 401 terms (more than 37 week's gestation) breech deliveries at the Institute of Gynecology and Obstetrics, Belgr…

The birth of Kaiser William II (1859-1941) and his birth injury.

This paper describes the events leading up to the birth of Kaiser William II in 1859. There is a full description of the clinical aspects of his breech delivery that resulted in an Erb-Duchenne palsy. The later physical and psychological effects of his paralysed left arm are discussed fully, as are the comments about Dr Eduard Arnold Martin (1799-1875), the obstetrician who delivered him.
