Enterococcal Infections in Left Ventricular Assist Device Recipients: Two Clinical Cases and Systematic Review.

Background: Infections remain a major complication in patients with left ventricular assist devices (LVADs). Enterococcal infections are increasingly recognized and are therapeutically challenging because of antimicrobial tolerance, biofilm formation on prosthetic material, and the frequent impossibility of device removal. Objective and method: We report two clinical cases of Enterococcus faecalis LVAD infection and a PRISMA-based systematic review of cases published between 1996 and 2025 that were identified through PubMed/MEDLINE. Primary outcomes were clinical cure, relapse or recurrence, m

A Systematic Review of the Cost-Effectiveness of Long-Term Mechanical Circulatory Support.

BACKGROUND: Mechanical circulatory support (MCS) is an option for the treatment of medically intractable end-stage heart failure. MCS therapy, however, is resource intensive. OBJECTIVE: The purpose of this report was to systematically review the MCS cost-effectiveness literature as it pertains to the treatment of adult patients in end-stage heart failure. METHODS: We conducted a systematic search and narrative review of available cost- effectiveness and cost-utility analyses of MCS in adult patients with end-stage heart failure. RESULTS: Eleven studies analyzing the cost-effectiveness or co

Perioperative management of patients with drug-eluting stents.

Thrombosis of a drug-eluting stent (DES) is a catastrophic complication. The risk of stent thrombosis (ST) is increased in the perioperative setting and is strongly associated with the cessation of antiplatelet therapy. This article reviews the perioperative management of patients with DES with a clinical focus on the perioperative use of antiplatelet therapy. Cessation of dual antiplatelet therapy is the single most significant predictor of perioperative ST. Available data on perioperative management of patients with DES are limited, and recommendations are therefore limited. To avoid ST with

A rare case of myocardial bridge involving left anterior descending, obtuse marginal and ramus intermediate coronary arteries.

Myocardial bridging, a congenital coronary anomaly, is a cluster of myocardial fibers crossing over the epicardial coronary arteries. It is most frequently seen in the left anterior descending artery (LAD), and rarely involves the circumflex (CX) and right coronary artery (RCA). We report a patient with an unusual coronary bridge crossing over the left anterior descending, obtuse marginal and ramus intermediate branches. The patient presented with exercise-induced angina pectoris that was relieved with medical therapy.

Blood Spotlight on iAMP21 acute lymphoblastic leukemia (ALL), a high-risk pediatric disease.

Intrachromosomal amplification of chromosome 21 (iAMP21) defines a distinct cytogenetic subgroup of childhood B-cell precursor acute lymphoblastic leukemia. Breakage-fusion-bridge cycles followed by chromothripsis and other complex structural rearrangements of chromosome 21 underlie the mechanism giving rise to iAMP21. Patients with iAMP21 are older (median age 9 years), with a low white cell count. They have a high relapse rate when treated as standard risk. Recent studies have shown improved outcome on intensive therapy. Molecular targets for therapy are being sought.

[Digital health applications and adherence in depression: a qualitative study from the perspective of healthcare providers].

BACKGROUND: Shortages of healthcare professionals and the rising prevalence of depressive disorders are creating gaps in care. When used correctly and consistently, digital health applications (DiGA) can help bridge these gaps. This study examines, from the perspective of healthcare providers, criteria for DiGA selection, their integration into therapy and the promotion of patient adherence. METHODS: For this qualitative study eight semi-structured expert interviews were conducted with psychiatrists, psychotherapists and general practitioners in and around Osnabrück, Germany. The interviews w

Antimicrobial therapy of travellers' diarrhoea.

Two trials were carried out to evaluate antimicrobial agents in the therapy of travellers' diarrhoea. In the first, 72 students were given 500 mg bicozamycin (BI), a poorly absorbable drug, q.i.d. for three days and 68 were given a placebo. In the second, 37 students were given trimethoprim/sulphamethoxazole (160 mg TMP-SMX 800 mg), 38 TMP (200 mg), and in 35 a placebo was given b.i.d. for five days. Diarrhoea lasted on average 28 hours in those receiving BI compared to 64 hours in the placebo group (p = 0.00009). Significant shortening of diarrhoea after taking BI was seen for all subjects wi

Successful Bridging to Allogeneic Transplantation With Valemetostat in Two Refractory/relapsed Peripheral T-cell lymphoma patients.

We report the case of 2 patients with relapsed/refractory peripheral T-cell lymphoma treated with valemetostat tosylate, a selective dual inhibitor of histone-lysine N-methyltransferases enhancer of zest homolog 1 and 2, and subsequently bridged to allogeneic stem cell transplantation. Valemetostat led to a quick response and was well tolerated, offering a promising bridge therapy to transplantation for patients with relapsed/refractory peripheral T-cell lymphoma, which is still an unmet medical need.

Pseudo-Wellens Syndrome Caused by Myocardial Bridging.

BACKGROUND: Wellens syndrome is a preinfarction electrocardiographic pattern classically associated with critical proximal left anterior descending artery stenosis. However, nonatherosclerotic conditions, including myocardial bridging, may mimic this pattern. CASE SUMMARY: A 58-year-old man presented with intermittent chest pain and Wellens T-wave changes in the anterior leads. High-sensitivity troponin T levels were normal. Coronary angiography revealed normal coronary arteries with a mid left anterior descending myocardial bridge and no fixed stenosis. Beta-blocker therapy was deferred owin

[Myocardial bridge: description, significance, treatment].

The myocardial bridge is a well known anomalous trajectory of the coronary arteries, the significance of which has been discussed for a long time. It now appears that some myocardial bridges may cause myocardial ischaemia with clinical electrocardiographic, scintigraphic or metabolic signs--even myocardial infarction or sudden death. In these symptomatic cases, treatment is usually medical and rarely surgical. In the last few years, angioplasty and stenting have been used more frequently in cases resistant to medical therapy and appear to be an effective alternative to surgery.

[Perioperative complications of transurethral resection of bladder tumor in patients receiving antithrombotic therapy].

We examined perioperative complications of transurethral resection of bladder tumor (TURBT) in patients receiving antithrombotic therapy. We retrospectively studied 276 patients who underwent TURBT in our institute from January 2007 to March 2013. The study group consisted of 105 patients (38%) who were receiving antithrombotic agents, and the other 171 patients (62%) without antithrombotic agents were assigned to the control group. The period of discontinuation of antithrombotic agents complied with our institutional rule. The most frequently used agent was aspirin (69 patients : 66%), follow

Successful Implementation of Peripheral Venoarterial ECMO in a Patient With Eisenmenger Syndrome as a Bridge to Lung Transplant: A Case Report.

Eisenmenger syndrome (ES) is a form of cyanotic congenital heart disease resulting in pulmonary hypertension and right-to-left shunting. End-stage ES often requires lung transplantation and patients may require extracorporeal membrane oxygenation (ECMO) as a bridge, though the optimal cannulation strategy is not standardized. A 49-year-old woman with ES due to unrepaired ostium secundum atrial septal defect presented with acute on chronic pulmonary embolism and refractory hypoxemia, ultimately requiring VA ECMO cannulation, which was complicated by venous cannula migration across the ASD and i

Successful bridge to heart transplantation in a pediatric patient with biventricular heart failure associated with Barth syndrome: a case report.

Long waiting times for donor organs in pediatric patients have increased the need for mechanical circulatory support as a bridge to heart transplantation. We report the case of a one-year-old boy with Barth syndrome who presented with cardiogenic shock due to severe biventricular heart failure. Emergency biventricular assist device support using centrifugal pumps was initiated. Under severe pulmonary hypertension and pulmonary regurgitation, the right ventricular assist device outflow was temporarily directed to the left atrium to optimize left ventricular preload. As pulmonary hemodynamics im

Mirogabalin as a Therapeutic Option for Neuropathic Pain Emerging Post-endodontic Treatment: A Two-Case Report.

INTRODUCTION: Occlusal and percussion pain may manifest occasionally following endodontic treatment, influencing retreatment decisions. Two cases of periapical neuropathic pain, classified as post-traumatic trigeminal neuropathic pain according to the International Classification of Orofacial Pain, are presented. Although mirogabalin is effective in managing neuropathic pain, there is a lack of clinical reports on its use for occasional post-traumatic trigeminal neuropathic pain after endodontic treatment. These cases highlight clinical symptoms and successful treatment with mirogabalin for po
