Beta-agonist aerosol distribution in respiratory syncytial virus bronchiolitis in infants.

UNLABELLED: Bronchodilator aerosols are frequently administered to infants with bronchiolitis but with little success. The efficacy of aerosol treatments depends mainly on adequate targeting of the aerosol particles to the inflamed airways. This study evaluated the lower respiratory tract distribution characteristics of nebulized bronchodilators in infants with acute bronchiolitis. METHODS: Twelve infants (mean age +/- SD, 8 mo +/- 4 mo) who were admitted for acute respiratory syncytial virus bronchiolitis were treated with (99m)Tc-albuterol aerosol. Gamma-scintigraphy was used to assess tota…

Relationship between respiratory syncytial virus bronchiolitis and future obstructive airway diseases.

Evidence from a large number of prospective case-control studies shows that respiratory syncytial virus (RSV) bronchiolitis in infancy is often associated with recurrent wheezing and asthma during subsequent years. However, wheezing tends to diminish and most studies show no significant increase in wheezing compared to controls by school age or adolescence. An unresolved question is whether severe RSV infection during infancy causes the respiratory sequelae or inherent abnormalities predispose an infant to develop severe respiratory infection and sequelae, i.e. RSV is associated with the devel…

Bronchiolitis obliterans with organizing pneumonia associated with acute Coxiella burnetii infection.

We report the case of a 62-year-old man who developed bronchiolitis obliterans with organizing pneumonia (BOOP) associated with acute Coxiella burnetii infection. The diagnosis of BOOP was made by fiber-optic transbronchial biopsy. Treatment with corticosteroids resulted in rapid clinical improvement and complete resolution of airspace opacities. To the best of our knowledge, this is the first well-documented case of BOOP associated with C. burnetii infection. This case suggests that this infection might trigger the development of BOOP.

Prednisolone plus albuterol versus albuterol alone in mild to moderate bronchiolitis.

To evaluate combination therapy of mild to moderate bronchiolitis with bronchiodilators and corticosteroids, we treated 51 young children with first-time wheezing and symptoms of respiratory tract infection with albuterol plus either prednisolone or placebo for 5 days. Disease severity was scored on days 0, 2, 3, and 6. On day 2, prednisolone resulted in significantly lower scores (2.7 +/- 1.4 vs. 4.0 +/- 1.5 in all patients evaluated, p < 0.05) than placebo, whereas there was no detectable difference on day 6, suggesting that addition of prednisolone to albuterol transiently accelerates recov…

Respiratory bronchiolitis associated interstitial lung disease (RB-ILD) presenting with haemoptysis.

Respiratory bronchiolitis associated interstitial lung disease is an uncommon condition in current or ex-smokers. The presentation is non-specific, but haemoptysis is uncommonly reported in this condition. We report the case of a 25-year-old woman who presented with significant haemoptysis, dyspnoea, reduced transfer factor and normal clinical examination. In addition, a Medline literature search was performed to review the clinical features and prognosis of this disease. Other causes of haemoptysis were excluded with extensive investigation. The diagnosis was made on thoracoscopic lung biopsy…

Respiratory status and allergy nine to 10 years after acute bronchiolitis.

In order to evaluate further the relationship between acute bronchiolitis in infancy and subsequent respiratory problems, children prospectively followed up from the time of their admission to hospital were reviewed along with a group of matched controls recruited at the previous five and a half year assessment. Sixty one index children and 47 controls took part. The groups were well matched for age, height, parental smoking, and social class. Although the prevalence of respiratory symptoms had fallen when related to the previous review, there remained an excess of coughing (48 and 17% in inde…

Idiopathic bronchiolitis obliterans organizing pneumonia with peripheral infiltrates on chest roentgenogram.

We report five patients with chest roentgenograms showing peripheral infiltrates similar to that described for chronic eosinophilic pneumonia (CEP). While lung biopsy specimens (transbronchial and open in two and transbronchial only in three cases) revealed typical changes of bronchiolitis obliterans organizing pneumonia (BOOP) as the predominant finding in all cases, two cases had changes consistent with a resolving CEP. We speculate that (1) it may be common for BOOP to present with peripheral infiltrates, (2) chest roentgenograms showing peripheral infiltrates are not diagnostic of any spec…

Examining Disparities in Pediatric Asthma, Pneumonia, and Bronchiolitis Care in General Hospitals.

BACKGROUND/OBJECTIVE: Prior disparity research has focused on evaluating patient outcomes in freestanding children's hospitals, rather than identifying potential drivers of these disparities. Our objective was to evaluate drivers of disparities in respiratory illness care quality and outcomes in a national sample of general hospitals. METHODS: This retrospective study used pretrial data from hospitals in the SIP trial (Simultaneously Implementing Pathways for Improving Asthma, Pneumonia, and Bronchiolitis Care for Hospitalized Children, NCT05206695). Patients were eligible if they had a prima…

Utility of Lung Ultrasound Assessment in Infants with Acute Bronchiolitis: An Observational Study.

OBJECTIVES: To assess the utility of Lung ultrasound (LUS) in predicting clinical outcomes and disease severity in infants with acute bronchiolitis. METHODS: Infants with acute bronchiolitis underwent clinical assessment using the ReSVinet score and LUS to assess B-lines, intercostal space involvement, and sub-pleural consolidations. Nasopharyngeal swabs identified the viral etiology. Outcomes including ICU admission, length of hospital stay, and oxygen/advanced respiratory support were recorded and analyzed. RESULTS: One hundred (100) infants were enrolled with a mean age of 8.04±6.5 mo. On…

Association of Serum Eosinophilic Cationic Protein With Disease Severity in Infant Bronchiolitis.

OBJECTIVES: This study aimed to evaluate serum eosinophilic cationic protein (ECP) levels in young children with acute bronchiolitis compared with healthy controls and to investigate their association with clinical severity and tobacco smoke exposure. We hypothesized that ECP would not function as a diagnostic biomarker but might reflect inflammatory phenotypes associated with severe disease. METHODS: In this prospective cross-sectional study, 96 children aged 1 to 24 months with acute bronchiolitis and 96 age-matched and sex-matched healthy controls were enrolled. Serum ECP levels and routin…

Standardized Respiratory Weaning Protocol to Improve Bronchiolitis Length of Stay and Costs.

Background and objectives High-flow nasal cannula (HFNC) is widely used in the management of bronchiolitis. However, variation in weaning practices may contribute to prolonged hospitalization. At our institution, the average length of stay (LOS) for bronchiolitis admissions was 3.54 days, exceeding the national average. We hypothesized that implementation of a standardized weaning protocol would reduce LOS and associated hospital costs. Our primary aim was to decrease hospital LOS by 20% among patients <24 months of age admitted with bronchiolitis over the course of one year. A secondary objec…

The Persistent Respiratory Impact of Respiratory Syncytial Virus (RSV) Bronchiolitis in Infants.

INTRODUCTION: Respiratory syncytial virus (RSV) bronchiolitis during infancy has the potential to progress to recurrent wheezing and asthma. However, studies evaluating lung function using tidal breath analysis in this age group are limited. OBJECTIVE: This study aimed to assess lung function in infants hospitalized during their first episode of acute RSV bronchiolitis using tidal breath analysis, to compare tidal breathing parameters between the acute phase and 2 months after clinical recovery, and to evaluate the impact of bronchiolitis severity on lung function. SUBJECTS AND METHODS: A to…

Effects of implementing a clinical scoring system on the management of infants with RSV bronchiolitis.

INTRODUCTION: Respiratory syncytial virus (RSV) is a common pathogen of lower respiratory tract infections in infants 1 month to 1 year of age. Infections with RSV are the most common cause for hospitalization for respiratory infections and present with a high rate of complications. Different scores for severity assessment currently exist but only few of them are well validated. The "Inpatient clinical pathway for evaluation/treatment of children with bronchiolitis" from the Children's Hospital of Philadelphia (CHOP) provides clear and concise guidance for severity grading and inpatient manage…

Unexpected decline of pulmonary function in a patient with thymoma mimicking obstructive bronchiolitis.

Obstructive bronchiolitis (OB) is a rare disorder characterized by progressive airflow limitation and is most commonly associated with autoimmune diseases, inhalation of toxins, lung transplantation, and hematopoietic stem cell transplantation. Thymoma is a rare mediastinal tumor frequently accompanied by autoimmune diseases, such as myasthenia gravis. We report a male patient with mild chronic obstructive pulmonary disease who developed thymoma and severe respiratory dysfunction mimicking OB. Postmortem autopsy revealed histopathological findings resembling systemic graft-versus-host disease …

The probability of reducing hospitalization rates for bronchiolitis with epinephrine and dexamethasone: A Bayesian analysis.

BACKGROUND: Bronchiolitis exerts a high burden on children, their families and the healthcare system. The Canadian Bronchiolitis Epinephrine Steroid Trial (CanBEST) assessed whether administering epinephrine alone, dexamethasone alone, or in combination (EpiDex) could reduce bronchiolitis-related hospitalizations among children less than 12 months of age compared to placebo. CanBEST demonstrated a statistically significant reduction in 7-day hospitalization risk with EpiDex in an unadjusted analysis but not after adjustment. OBJECTIVE: To explore the probability that EpiDex results in a reduc…
