[Kartagener's syndrome: a case report].

The authors report a rare case of Kartagener's syndrome in 8 years old girl revelated by congenital cardiopathy with chronic bronchitis and severe heart failure. Incomplet endocardial cushion defect with single atrium was found and situs inversus suspected, confirmed by ultrasonography. She undergone cardiac surgery in Europe: atrial septation and mitralvalve repair. Surgery redux was neccessary formitral insufficency and residual shunt. Persistent atelectasia in lower inferior lobe indicated bronchoscopy. Lung biopsy confirmed Kartagener's syndrome. Now, she has no cardiac symptom, but bronch

[Prevalence of respiratory diseases in a rural population in the north of Catalonia: La Cerdanya].

BACKGROUND: There are few epidemiological studies on health respiration in the population of Catalonia (Spain). This is a cross-sectional study of prevalence about respiratory diseases in a rural population in La Cerdanya (Catalonia). METHODS: The sample was obtained by a simple random method among the Cerdanya population older than 6 years. The questionnaire used was from the American Thoracic Society for the Division of Lung Disease with an annex attached for extrinsec allergic alveolitis which was worked out. RESULTS: The smoking prevalence was 24.2%, among the farmers (17.1%) was lower t

Obstructive airways disease in non-smoking subjects with pigeon fanciers' lung.

The aim of the study was to assess the degree of airways involvement in non-smoking subjects with extrinsic allergic alveolitis resulting from pigeon-derived antigens. Two hundred and eight pigeon fanciers completed a questionnaire of symptoms, performed spirometry on a Vitalograph Compact spirometer and had IgG antibody to pigeon gamma-globulin measured. Eighty fanciers had the acute form of pigeon fanciers' lung (PFL) and these subjects were less likely to be current smokers (P less than 0.001). Subjects with PFL had a higher prevalence of chronic bronchitis (P less than 0.001), large airway

Determinants of airflow obstruction in severe alpha-1-antitrypsin deficiency.

BACKGROUND: Severe alpha(1)-antitrypsin (AAT) deficiency is an autosomal recessive genetic condition associated with an increased but variable risk for chronic obstructive pulmonary disease (COPD). A study was undertaken to assess the impact of chronic bronchitis, pneumonia, asthma and sex on the development of COPD in individuals with severe AAT deficiency. METHODS: The AAT Genetic Modifier Study is a multicentre family-based cohort study designed to study the genetic and epidemiological determinants of COPD in AAT deficiency. 378 individuals (age range 33-80 years), confirmed to be homozygo

[Does the manner of smoking affect chronic obstructive airway diseases and bronchial cancer?].

Duration of smoking career, number of cigarettes smoked and smoke inhalation pattern seem to have a bearing on the occurrence of tobacco-related lung diseases. The authors therefore determined the smoking pattern and especially the time relation between drawing on a cigarette and inhalation in smokers with and without tobacco-related lung diseases. Based on clinical and radiological findings as well as pulmonary function tests, 91 smokers (without lung disease, with small airway disease, with simple chronic bronchitis, with obstructive bronchitis, with bronchitis and predominantly emphysema an

Treatment of acute bronchitis in adults without underlying lung disease.

OBJECTIVE: To determine whether antibiotic and bronchodilator treatment of acute bronchitis in patients without lung disease is efficacious. DESIGN: A MEDLINE search of the literature from 1966 to 1995 was done, using "Bronchitis" as the key word. Papers addressing acute bronchitis in adults were used as well as several citations emphasizing pediatric infections. A manual search of papers addressing the microorganisms causing acute bronchitis was also done. Data were extracted manually from relevant publications. SETTING: All published reports were reviewed. Papers dealing with exacerbations

Unexplained bronchopulmonary disease with inflammatory bowel disease.

Six patients developed severe, unexplained, chronic bronchopulmonary disease from 3 to 13 years after the onset of nonspecific inflammatory disease of the colon. All had chronic bronchitis, bronchiectasis was diagnosed in four, and an obstructive type of pulmonary dysfunction was noted in five. Four of the six, including the two with only chronic bronchitis, had no history of smoking. There was an initial correlation between the pulmonary symptoms and the intestinal disease, except in two patients who developed overt pulmonary disease following total proctocolectomy. The frequent occurence of

The Tecumseh study of respiratory illness. VIII. Acute infection in chronic respiratory disease and comparison groups.

Individuals with chronic lung disease and their families were selected from the Tecumsch community along with similarly selected families as comparison groups and studied for 1-year periods. Occurence of acute respiratory illness was ascertained weekly by telephone and calculated as an annual rate. Persons with chronic bronchitis not only experienced more acute lower respiratory illness than healthy comparison subjects, but total illness rates were somewhat higher as well. Infection rates were determined from blood samples taken 3 times from each participant during the surveillance year. Antib

Principles of appropriate antibiotic use for treatment of uncomplicated acute bronchitis: background.

The following principles of appropriate antibiotic use for adults with acute bronchitis apply to immunocompetent adults without complicating comorbid conditions, such as chronic lung or heart disease. The evaluation of adults with an acute cough illness or a presumptive diagnosis of uncomplicated acute bronchitis should focus on ruling out serious illness, particularly pneumonia. In healthy, nonelderly adults, pneumonia is uncommon in the absence of vital sign abnormalities or asymmetrical lung sounds, and chest radiography is usually not indicated. In patients with cough lasting 3 weeks or lo

Occupational exposure to pesticides and respiratory health.

This article aims to review the available literature regarding the link between occupational exposure to pesticides and respiratory symptoms or diseases. Identification of epidemiological studies was performed using PubMed. 41 articles were included, 36 regarding agricultural workers and five regarding industry workers. Among the 15 cross-sectional studies focusing on respiratory symptoms and agricultural pesticide exposure, 12 found significant associations with chronic cough, wheeze, dyspnoea, breathlessness or chest tightness. All four studies on asthma found a relationship with occupationa

Airway obstruction in adults with recurrent respiratory infections and IgG deficiency.

In 42 adults with recurrent respiratory infections (RRI) and common variable immunodeficiency or immunoglobulin G (IgG) subclass deficiency, the results of pulmonary function tests were related to factors apt to produce airway obstruction: serum concentration of IgG and IgG subclasses, various features of acute RRI (number/year, time from onset to diagnosis, episodes of pneumonia, etc) and type of chronic lung disease (smoking and nonsmoking related chronic bronchitis, episodic wheezing, and bronchiectasis). Compared with nonsmokers, usually less than 40 years of age, the patients above 40 had

The TNFalpha gene relates to clinical phenotype in alpha-1-antitrypsin deficiency.

BACKGROUND: Genetic variation may underlie phenotypic variation in chronic obstructive pulmonary disease (COPD) in subjects with and without alpha 1 antitrypsin deficiency (AATD). Genotype specific sub-phenotypes are likely and may underlie the poor replication of previous genetic studies. This study investigated subjects with AATD to determine the relationship between specific phenotypes and TNFalpha polymorphisms. METHODS: 424 unrelated subjects of the PiZZ genotype were assessed for history of chronic bronchitis, impairment of lung function and radiological presence of emphysema and bronch

[Biological effect of cement dust].

The harmful effect of cement dust upon living organisms consists in irritating, sensitizing and pneumoconiotic properties of its components. In animal studies it has been observed that cement dust induces atrophic and hypertrophic changes in nasal and pharyngeal mucosa and chronic exfoliative bronchitis. In the lungs of experimental animals slight tissue fibrosis and some emphysema foci were found. The examination of workers exposed to cement dust has shown that disorders of the upper respiratory airways they suffer from include most often chronic rhinitis, laryngitis and pharynx catarrh. Also

Tracheobronchial clearance in patients with emphysema associated with alpha1-antitrypsin deficiency.

Tracheobronchial clearance was studied in five patients who had emphysema associated with deficiency of serum alpha1-antitrypsin but no history of chronic bronchitis. After the patients had inhaled an aerosol of 6 micrometer teflon particles tagged with 99mTc, the radioactivity in the lungs was followed externally during 2 h. Clearance in the emphysematous patients was normal or even rapid as compared with clearance in healthy subjects, and was significantly more rapid than clearance in patients with a history of chronic bronchitis. The results indicate that emphysema can develop in patients w

[Specific respiratory manifestations associated with hemorrhagic rectocolitis. Analysis of a case and review of the literature].

Respiratory impairments occurring as systemic manifestations of ulcerative colitis are often misinterpreted. We report a case of chronic productive bronchitis associated with ulcerative colitis; symptoms appeared after colectomy. Clinical features distinguished it from common chronic bronchitis: it occurred in a woman without respiratory disease or smoking history, a chronic productive cough without bronchiectasis, negative infectious investigations, severe endoscopic and histologic lesions, and above all remarkable steroid efficacy. Systematic research and early treatment of these manifestati

Respiratory infection: the disease.

Infectious pneumonias are inflammations of the lung that can be localized in the alveoli or interstitial tissue or both. The pathogenic agent is usually airborne; more rarely it is hematogenous. Important distinctions are between bacterial and nonbacterial forms, between diseases acquired outside and inside hospitals, and between patients who are basically healthy and those with a previous illness. Pneumococci continue to be the dominant pathogens outside hospitals. In hospitals, gram-negative, anaerobic, and fungal pathogens are more often found. Usually, purulent chronic bronchitis or an acu

[Haemophilus infection in nonspecific inflammatory lung diseases].

Hemophilic infection incidence rate, characteristics and criteria of early detection were studied in 393 patients with nonspecific pulmonary diseases. H. influenzae represented the prevalent pathogenic agent in 17% of the cases: in 11% of acute pneumonias (276 patients) and 30% of chronic bronchitis exacerbations (117 patients). Hemophilic pneumonias were a more frequent finding in patients over 40 years of age, proved secondary, manifested usually interstitial inflammation of the lung tissue, were prone to interlobitis complication and protracted course.
