Evaluation of the DSM-5 severity indicator for bulimia nervosa.

This study examined the DSM-5 severity criterion for bulimia nervosa (BN) based on the frequency of inappropriate weight compensatory behaviors. 199 community volunteers classified with BN were categorized using DSM-5 severity levels and compared on demographic and clinical variables. 77 (39%) participants were categorized as mild, 68 (34%) as moderate, 32 (16%) as severe, and 22 (11%) as extreme. The severity groups did not differ significantly in demographic variables or body mass index. Shape and Weight concerns did not differ significantly across severity groups. Binge eating differed with…

A longitudinal investigation of mortality in anorexia nervosa and bulimia nervosa.

OBJECTIVE Although anorexia nervosa has a high mortality rate, our understanding of the timing and predictors of mortality in eating disorders is limited. The authors investigated mortality in a long-term study of patients with eating disorders. METHOD Beginning in 1987, 246 treatment-seeking female patients with anorexia nervosa or bulimia nervosa were interviewed every 6 months for a median of 9.5 years to obtain weekly ratings of eating disorder symptoms, comorbidity, treatment participation, and psychosocial functioning. From January 2007 to December 2010 (median follow-up of 20 years), vi…

A cross-sectional analysis of the cognitive model of bulimia nervosa.

OBJECTIVE: The present study aimed to directly test Cooper et al.'s cognitive model of the maintenance of bulimia nervosa. METHOD: The major predictions of the model were tested cross-sectionally using multiple regression analyses and structural equation modeling in a mixed sample comprising nonclinical female university students (n = 298) and clinical eating disorder patients (n = 44). RESULTS: Consistent with model, negative self-beliefs was associated with negative emotion, negative emotion with positive and negative beliefs about eating, beliefs about eating with eating behavior, and bin…

A review of the definitions of outcome used in the treatment of bulimia nervosa.

There are many terms used to describe treatment outcome for bulimia nervosa. However, the way such terms are conceptualised across various studies differs dramatically, making comparison of outcomes difficult. A consensus working definition of pivotal terms such as remission and recovery is important if treatments are to be adequately evaluated and clinical meaning derived for individuals with bulimia nervosa (BN). The central aim of the current review was to identify different definitions of remission and recovery and their utility in terms of client outcome after treatment for BN. Seventy on…

Body schema and self-representation in patients with bulimia nervosa.

OBJECTIVE: Neuroimaging evidences in eating disorder (ED) patients document dysfunctional neural activity of the posterior parietal cortex, which is engaged in the representation of body schema. Yet a full neuropsychological investigation of body schema representation in ED patients is lacking. We examined mental imagery and body schema representation in patients with bulimia nervosa (BN) and binge eating disorder (BED). METHOD: Consecutive samples of 15 BN patients and 15 BED patients were compared with two groups of 15 age-matched controls in tasks requiring body or object mental transforma…

College students' perceptions of individuals with anorexia and bulimia nervosa.

OBJECTIVE: Eating disorders (EDs) are highly stigmatized conditions. This study explored factors hypothesized to influence this stigmatization including ethnicity, gender, ED subtype, and proposed etiology. METHOD: Undergraduates (N = 235) read scenarios depicting fictional characters varying on ethnicity, gender ED subtype, and etiology. Participants reported perceptions of each character, and completed the EAT-26 and the Level-of-Contact scale. RESULTS: Characters with BN were viewed as more responsible for their ED and more self-destructive than those with AN, who were viewed as more self

An evaluation of the enhanced cognitive-behavioural model of bulimia nervosa.

The original cognitive-behavioural model of bulimia nervosa (BN) has been enhanced to include four additional maintaining mechanisms: low self esteem, clinical perfectionism, interpersonal problems, and mood intolerance. These models have been used to guide cognitive-behavioural treatment for BN, but the enhanced model has yet to be directly evaluated as a whole in a clinical sample. This study aimed to compare and evaluate the original and the enhanced cognitive-behavioural models of BN using structural equation modelling. The Eating Disorder Examination and self-report questionnaires were co…

[Assessment of emotional reactivity to food images in bulimia nervosa].

The aim of the study was to assess the influence on reactivity to food images of the following variables: craving-trait, positive or negative mood state, and food restriction. Emotional modulation of the defense startle reflex (RMS) was assessed in 26 women at risk of suffering from bulimia nervosa; they were assigned one of to two groups: high craving-trait and low craving-trait. Before the test, positive or negative mood and restriction vs. non-restriction states were induced in each of the groups. Skin conductance response (SCR) and electromyogram activity from the orbiculari oculi region w…

Gastroesophageal reflux disease and bulimia nervosa--a review of the literature.

Bulimia nervosa and other eating disorders have been on the increase for the past half century. Self-induced vomiting is often practiced as a method of weight control in these patients, potentially causing acidic damage to the esophagus of the kind observed in cases of gastroesophageal reflux disease. To ascertain whether patients suffering from bulimia nervosa had an increased rate of reflux-related symptoms, potentially placing them at risk of developing sequelae such as Barrett's esophagus and esophageal adenocarcinoma, a literature review was performed via searches of databases including P…

Vomiting and feeling fat--coincidence of achalasia and bulimia nervosa.

OBJECTIVE: In clinical practice it is often difficult to distinguish between organic and psychological vomiting. METHOD: We present a case of a 34-year-old woman with an 18-month history of vomiting with the initial diagnosis of bulimia nervosa (BN). However, the patient showed an unusual vomiting behavior. Therefore, further somatic diagnostic procedures were carried out leading to the additional diagnosis of achalasia. After successful interventional and surgical treatment for achalasia, further psychotherapeutic interventions were necessary because of worsening of the BN symptoms. A certai…

The use of duloxetine in chronic bulimia nervosa: a case report.

Selective serotonin reuptake inhibitors are routinely prescribed as off-label treatments for bulimia nervosa. There is, however, a paucity of literature addressing the efficacy of the serotonin norepinephrine reuptake inhibitors in the pharmacologic management of this disorder. This article describes a clinical situation in which duloxetine, a serotonin norepinephrine reuptake inhibitor, effectively treated a patient with long-standing bulimia nervosa, purging type, and comorbid generalized anxiety disorder.

Painful parotid hypertrophy with bulimia: a report of medical management.

In eating disorders, such as bulimia nervosa, body image disturbance often extends beyond the realm of weight and shape into the dermatologic spectrum. While commonly associated conditions due to binging and self-induced vomiting include cutaneous entities (e.g., Russell's sign, acne, alopecia and hypertrichosis) and oral pathologies (e.g., enamel erosion, caries and mild parotid hypertrophy), a rare but troubling manifestation is disfiguring parotid enlargement (sialoadenomegaly). This article presents a case of painful sialoadenomegaly associated with hyperamylasemia in a bulimic patient suc…

Severity of bulimia nervosa. Measurement and classification into health or pathology.

AIMS: In order to identify the most important components of the severity of bulimia nervosa (as well as identifying clinical cases), we explored the relation between dimensional and categorical assessment. This was achieved by studying the performance of variables from standard instruments (measuring specific and general psychopathology) in predicting an expert rating of overall syndrome severity. METHOD: In total, 213 cases were selected (across the whole range of severity). We applied regression with optimal scaling to model nonlinear relations in the data, and the lasso method with bootstr…

[The role of a psychiatric nurse in the treatment of anorexia and bulimia nervosa].

BACKGROUND: An alarming number of individuals are developing eating disorders in the western world. Eating disorder is a complex compulsion to eat in a way which disturbs physical, mental and psychological health. There is no one cause of eating disorders because many things contribute to them. AIM: The aim of this article is to clarify whether a nurse could play a role in the treatment of patients with eating disorders such as anorexia and bulimia nervosa. METHOD: After an accurate research for the world's centers that are in forefront of scientific research in eating disorders, we identifi…

Cue exposure in the treatment of resistant adolescent bulimia nervosa.

OBJECTIVE: A percentage of bulimic patients do not greatly improve with the usual treatment. Therefore, the objective was to further evaluate cue exposure (CE), in order to attain better results in clinical settings. METHOD: Twenty-two adolescent patients who fulfilled DSM-IV diagnostic criteria for bulimia nervosa (mean age 16.7, SD 1.5) and who were resistant to the usual treatment followed a program of 12 CE sessions. Clinical characteristics were evaluated and different psychopathological scales were administered at the beginning and the end of the CE program and at 6 month follow-up. Sub…

Are there subgroups of bulimia nervosa based on comorbid psychiatric disorders?

OBJECTIVE: The current study sought to determine whether there are subtypes of bulimia nervosa (BN) differentiated by comorbid psychiatric disorders. METHOD: Data on comorbid psychiatric diagnoses in female relatives of probands and controls in the Collaborative Study of the Genetics of Alcoholism (COGA) who met criteria for BN (as outlined in the 3rd Rev. ed. of the Diagnostic and Statistical Manual of Mental Disorders) were analyzed using latent class analysis. Resulting latent classes were compared on a variety of variables related to impulsive behaviors and psychological functioning. RES…

Beliefs of women concerning the severity and prevalence of bulimia nervosa.

BACKGROUND: Few studies have examined attitudes towards eating-disordered behaviour among women in the general population. METHODS: A vignette describing a fictional person meeting diagnostic criteria for bulimia nervosa (BN) was presented to a community sample of women aged 18-45. Respondents (n = 208) were asked a series of questions concerning the severity and prevalence of the problem described. RESULTS: Most respondents viewed BN as a distressing condition whose sufferers are deserving of sympathy. However, more than one-third of respondents had at some stage believed that it 'might not…
