This report describes a hand infection caused by an alga. Prototheca wickerhamii. This rare organism presents most often as chronic dermatitis or bursitis of the upper extremity. Routine stains and cultures are easily confused with fungi. Only early, radical excision of the involved structures will effect a cure.

Anthopsis deltoidea was found to be the cause of an olecranon bursitis in a 79-year-old golfer. Serial histological sections of the olecranon bursa showed faintly stained, brown-walled, septate, hyphal elements in the centers of the necrotic debris. The combination of bursectomy and flucytosine treatment cured the infection.

BACKGROUND: A 19-year-old National Collegiate Athletic Association Division I male basketball player presented with pain in the posterior region of the right shoulder. Prototheca is a microalgae known to cause infections in humans, with protothecosis most commonly presenting as olecranon bursitis or localized soft tissue infection. Disseminated disease can be seen in immunocompromised patients. In this retrospective single-institution case series, we describe our experience with 7 patients with Prototheca infections.

We report a case of focal tuberculous involvement of the posterior margin of the calcaneus with preservation of the articular margin. The route of infection was direct extension through tuberculous retrocalcaneal bursitis, a rare and atypical pathogenesis. Magnetic resonance imaging was helpful in ruling out neoplasm and in limiting the diagnosis to an inflammatory infectious process.
