Nutrition/Dietary Supplements and Chronic Pain in Patients with Cancer and Survivors of Cancer: A Systematic Review and Research Agenda.

BACKGROUND: Chronic pain is one of the most often seen, but often undertreated, sequelae in survivors of cancer. Also, this population often shows significant nutritional deficiencies, which can affect quality of life, general health status, and even risk of relapse. Given the influence of nutrition on brain plasticity and function, which in turn is associated with chronic pain in the population with cancer, it becomes relevant to focus on the association between pain and nutritional aspects in this population. OBJECTIVE: To identify relevant evidence regarding nutrition and chronic pain in p

Targeting CGRP signaling alleviates cancer-associated pain in oral squamous cell carcinoma.

BACKGROUND: Pain is the most frequent complication in patients with oral squamous cell carcinoma (OSCC) and is associated with poor prognosis. Calcitonin gene-related peptide (CGRP), abundantly expressed in sensory neurons, has been implicated in multiple pain disorders; however, its role in cancer-associated pain remains unclear. We investigated the relationship between CGRP and OSCC-associated pain and evaluated the analgesic potential of Rimegepant, a CGRP-receptor antagonist approved for migraine. METHODS: Plasma CGRP was measured in OSCC patients and correlated with preoperative pain sco

Buprenorphine for Cancer Pain: Evidence, Guidelines, and Clinical Rationale for Early Use.

As cancer survivorship increases, long-term opioid therapy (LTOT) has become more common in oncology, raising concerns about opioid-related harms. Buprenorphine offers unique pharmacologic advantages that position it as a safer alternative to full agonist opioids (FAOs), yet it remains underutilized in the management of cancer pain (CP). This article reviews the clinical rationale, supporting evidence, and practical considerations for using buprenorphine in managing CP throughout the cancer care continuum, with guidance from the NCCN Adult Cancer Pain Guideline Panel. This review synthesizes f…

Decoding Pancreatic Cancer Pain: From Tumor-Nerve Crosstalk to Targeted Analgesic Strategies.

Pancreatic cancer (PC) remains one of the most lethal malignancies, with pain being a prevalent and debilitating symptom that severely impairs patients' quality of life and independently predicts poorer survival. While historically attributed to tumor mass effects, the mechanisms driving PC pain are far more complex. This review aims to synthesize the current understanding of the intricate biological underpinnings of PC-associated pain and discuss the paradigm shift towards mechanism-based therapeutic strategies. The pathogenesis of PC pain is fundamentally driven by dynamic tumor-nerve intera

Macrophage-derived exosomes induce M2 microglial polarization to alleviate bone cancer pain.

Bone cancer pain (BCP) is a frequent and debilitating complication in patients with malignant tumors, arising from a multifactorial interplay of bone destruction, neural injury, and inflammatory responses. Microglia can polarize into either an M1 phenotype, which aggravates nociception, or an M2 phenotype, which facilitates pain resolution. Activation of the TLR4/NF-κB signaling cascade is known to drive M1 polarization, thereby amplifying inflammation and neuronal damage. This study aimed to investigate whether macrophage-derived exosomes could mitigate BCP by modulating the TLR4/NF-κB pathwa…
