ECIL recommendations for the use of biological markers for the diagnosis of invasive fungal diseases in leukemic patients and hematopoietic SCT recipients.

As culture-based methods for the diagnosis of invasive fungal diseases (IFD) in leukemia and hematopoietic SCT patients have limited performance, non-culture methods are increasingly being used. The third European Conference on Infections in Leukemia (ECIL-3) meeting aimed at establishing evidence-based recommendations for the use of biological tests in adult patients, based on the grading system of the Infectious Diseases Society of America. The following biomarkers were investigated as screening tests: galactomannan (GM) for invasive aspergillosis (IA); β-glucan (BG) for invasive candidiasis

Ibrexafungerp: Mechanism of Action, Clinical, and Translational Science.

Ibrexafungerp is a first-in-class triterpenoid antifungal that targets β-(1,3)-glucan synthase in fungal cell wall development. It is indicated for vulvovaginal candidiasis (VVC) and recurrent vulvovaginal candidiasis (RVVC) in adult and post-menarchal pediatric females. The drug was approved in 2021 and 2022 for VVC and RVVC, respectively. Research is ongoing to evaluate the safety of ibrexafungerp in pregnancy and lactation as well as to explore intravenous and oral formulations for invasive candidiasis and invasive aspergillosis. Ibrexafungerp binds and inhibits β-(1,3)-glucan synthase that

A Membrane-Targeted Photosensitizer Prevents Drug Resistance and Induces Immune Response in Treating Candidiasis.

Candida albicans (C. albicans), a ubiquitous polymorphic fungus in humans, causes different types of candidiasis, including oral candidiasis (OC) and vulvovaginal candidiasis (VVC), which are physically and mentally concerning and financially costly. Thus, developing alternative antifungals that prevent drug resistance and induce immunity to eliminate Candida biofilms is crucial. Herein, a novel membrane-targeted aggregation-induced emission (AIE) photosensitizer (PS), TBTCP-QY, is developed for highly efficient photodynamic therapy (PDT) of candidiasis. TBTCP-QY has a high molar absorption co

Deep Cutaneous Candidiasis With Costal Osteomyelitis Following Pectoralis Major Myocutaneous Flap Reconstruction: A Case Report.

Candida is a yeast commonly found in various environments. It usually coexists with the skin without causing harm. It may exhibit pathogenicity when local or systemic immune defense mechanisms are compromised, which creates conditions favorable for its proliferation. This report discusses a case of invasive cutaneous candidiasis with rib osteomyelitis that developed at the donor site of a pectoralis major myocutaneous flap. The patient was a 58-year-old man who underwent reconstruction with a pectoralis major myocutaneous flap for adenoid cystic carcinoma in the midline floor of the mouth. Six

Neonatal Invasive Fungal Infections: Epidemiology, Microbiology, and Controversies in Practice.

As neonatology has moved to caring for infants of lower gestational ages, prevention, prompt diagnosis, and treatment of invasive fungal infections have contributed to the improved neonatal outcomes. Standardized care should include an infection prevention bundle for invasive Candida infections with targeted fluconazole prophylaxis in high-risk patients, antimicrobial stewardship, and central venous catheter-related infection prevention measures. Knowledge and vigilance with skin care, recognition of dermatologic presentation of congenital cutaneous candidiasis, and cutaneous mold infections w

Evaluation of knowledge and experience of fungal infections (mycoses) among clinical doctors in Senegal.

In order to assess the knowledge and experience of fungal infections (FIs) among clinicians in Senegal, a cross-sectional survey was carried out among medical practitioners in Senegal via a questionnaire designed with "Google Forms" between 24 January and 24 April 2022. A total of 100 clinicians responded to the questionnaire. Clinicians in the 31- 40-year-old age group formed the majority of respondents (51%). Male respondents were predominant (72%). Forty-one percent of respondents were general practitioners, 40% were specialist doctors, and the rest were residents. Dermatologists were the m

Invasive fungal infections in solid organ transplant recipients.

Solid organ transplant (SOT) recipients have a significant risk of invasive fungal diseases (IFD) caused mainly by Candida spp. and Aspergillus spp. Candida spp. is the most frequent agent of IFD in the transplant recipient. The absence of clinical trials and the epidemiological differences in IFD in different transplant programmes mean that there are no definitive recommendations for the diagnosis, treatment and prevention of IFD in SOT, so most of the evidence must be based on clinical experience.

Breaking the Mold: A Rare Case of Invasive Colonic Aspergillosis Causing Toxic Megacolon and Perforation in a Patient Without Classic Immunosuppression.

Invasive gastrointestinal aspergillosis is rare and typically occurs in immunocompromised hosts. We present a case of a 24-year-old man with intermittent corticosteroid use presenting with diarrhea and abdominal pain found to have toxic megacolon on imaging. He developed colonic necrosis and perforation requiring emergent colectomy. Histopathology revealed invasive Aspergillus with transmural necrosis. His course was complicated by septic shock, acute kidney injury requiring dialysis, and pulmonary candidiasis. Primary colonic aspergillosis in immunocompetent patients is exceedingly uncommon.

Coping with Candida infections.

Although Candida spp. exist as normal flora on much of the human body, life-threatening invasive infections by these organisms have increased in recent decades. Candida spp. are now one of the most common causes of nosocomial blood stream infections worldwide, and patients with critical illness or malignant disease demonstrate particular susceptibility. Risk factors are identifiable for virtually all patients who develop invasive candidiasis, and clinicians must maintain vigilance for Candida infections in the appropriate clinical context, given the attributable mortality. This review addresse

Candida khanbhai sp. nov., a new clinically relevant yeast within the Candida haemulonii species complex.

Invasive fungal infections caused by non-albicans Candida species are increasingly reported. Recent advances in diagnostic and molecular tools enabled better identification and detection of emerging pathogenic yeasts. The Candida haemulonii species complex accommodates several rare and recently described pathogenic species, C. duobushaemulonii, C. pseudohaemulonii, C. vulturna, and the most notorious example is the outbreak-causing multi-drug resistant member C. auris. Here, we describe a new clinically relevant yeast isolated from geographically distinct regions, representing the proposed nov

Invasive Non-Candida Fungal Infections in Acute Burns-A 13-Year Review of a Single Institution and Review of the Literature.

The past decade has demonstrated increased burn wound infections with atypical invasive fungal organisms. The range of previously regiospecific organisms has expanded, and plant pathogens are increasingly represented. Our institution sought to examine changes in severe fungal non-Candida infections in our patients, via retrospective review of patients admitted to our burn center from 2008 to 2021. We identified 37 patients with atypical invasive fungal infections. Non-Candida genera included Aspergillus (23), Fusarium (8), Mucor (6), and 13 cases of 11 different species, including the second-e

Increasing number of cases and outbreaks caused by Candida auris in the EU/EEA, 2020 to 2021.

The number of cases of Candida auris infection or carriage and of countries reporting cases and outbreaks increased in the European Union and European Economic Area during 2020 and 2021. Eight countries reported 335 such cases in 2020 and 13 countries 655 cases in 2021. Five countries experienced outbreaks while one country reported regional endemicity. These findings highlight the need for adequate laboratory capacity and surveillance for early detection of C. auris and rapid implementation of control measures.

The Presence of (1→3)-β-D-Glucan as Prognostic Marker in Patients After Major Abdominal Surgery.

BACKGROUND: While the serological detection of (1→3)-β-D-glucan (BDG) can indicate invasive fungal disease (IFD), false positivity occurs. Nevertheless, the presence of BDG can still be recognized by the host's innate immune system and persistent BDG antigenemia, in the absence of IFD, can result in deleterious proinflammatory immune responses. METHODS: During the XXX (INTENSE) study into the preemptive use of micafungin to prevent invasive candidiasis (IC) after abdominal surgery, the serum burden of BDG was determined to aid diagnosis of IC. Data from the INTENSE study were analyzed to dete

Model-Optimized Fluconazole Dose Selection for Critically Ill Patients Improves Early Pharmacodynamic Target Attainment without the Need for Therapeutic Drug Monitoring.

Fluconazole has been associated with higher mortality compared with the echinocandins in patients treated for invasive candida infections. Underexposure from current fluconazole dosing regimens may contribute to these worse outcomes, so alternative dosing strategies require study. The objective of this study was to evaluate fluconazole drug exposure in critically ill patients comparing a novel model-optimized dose selection method with established approaches over a standard 14-day (336-h) treatment course. Target attainment was evaluated in a representative population of 1,000 critically ill a

Opportunistic yeast pathogen Candida spp.: Secreted and membrane-bound virulence factors.

UNLABELLED: Candidiasis is a fungal infection caused by Candida spp. especially Candida albicans, C. glabrata, C. parapsilosis and C. tropicalis. Although the medicinal therapeutic strategies have rapidly improved, the mortality rate as candidiasis has continuously increased. The secreted and membrane-bound virulence factors (VFs) are responsible for fungal invasion, damage and translocation through the host enterocytes besides the evasion from host immune system. VFs such as agglutinin-like sequences (Als), heat shock protein 70, phospholipases, secreted aspartyl proteinases (Sap), lipases, e

Galleria mellonella for systemic assessment of anti-Candida auris using amphotericin B loaded in nanoemulsion.

Galleria mellonella is a model that uses adult larvae to assess the prophylactic, therapeutic, and acute toxic potential of substances. Given their benefits, G. mellonella models are being employed in investigations of systemic infections caused by highly resistant microorganisms. Among the multiresistant microorganisms, we highlight Candida auris, a yeast with high mortality potential and resistance. Among the potential drugs, amphotericin B (AmB) stands out; however, microbial resistance episodes and side effects caused by low selectivity have been observed. The incorporation of AmB into a n

Yeast Infections in Solid Organ Transplantation.

Invasive candidiasis (IC) remains the most common invasive fungal infection following solid-organ transplant (SOT), but risk factors are evolving. Current challenges include infection due to drug resistant non-albicans and emerging novel species such as Candida auris. Preventive antifungal use in SOT needs to be re-examined in light of these current challenges. Cryptococcosis is the second most common IFI following SOT. Cryptococcus gattii is an emerging pathogen that can have reduced in-vitro susceptibility to antifungal agents. Cryptococcus associated IRIS in SOT is a clinical entity that wa
