Computerized Automatic Diagnosis of Innocent and Pathologic Murmurs in Pediatrics: A Pilot Study.

OBJECTIVE: Computer-aided auscultation in the differentiation of pathologic (AHA class I) from no or innocent murmurs (AHA class III) would be of great value to the general practitioner. This would allow objective screening for structural heart disease, standardized documentation of auscultation findings, and may avoid unnecessary referrals to pediatric cardiologists. Our goal was to assess the quality of a novel computerized algorithm that automatically classifies murmurs in phonocardiograms (PCGs) acquired in a pediatric population. DESIGN: This is a pilot study testing the ability of a nov…

[Referral practice and clinical assessment of heart murmurs in children].

BACKGROUND: Heart murmurs in infants and children are common, and in most cases these are physiological murmurs. In a few cases they represent congenital heart disease; it is important to detect those few children by referral to paediatric cardiologists. MATERIAL: 220 consecutive children with heart murmur were investigated prospectively as outpatients in the Children's clinic, Haukeland University Hospital from January through December 2001. Referrals were from general practitioners (n = 157), paediatricians or residents at the Children's clinic (n = 51), and from one community-based paediat…

The diagnosis of innocent murmurs in childhood.

Innocent murmurs are common in childhood. They require accurate diagnosis to avoid unnecessary anxiety and/or restrictions. With this in mind, we studied prospectively children diagnosed by a paediatric cardiologist as having innocent murmurs to review the necessity of investigations in excluding organic abnormalities. We included 63 children in the study. The majority had the so-called Still's, or musical/vibratory murmur. None had abnormal investigations, suggesting that such investigations are largely superfluous for the purpose of diagnosis in those patients with innocent murmurs seen by a

Awareness, Attitudes, and Perceptions Toward Partial Heart Transplantation.

OBJECTIVE: Partial heart transplantation (PHT) is a new procedure that delivers growing heart valve implants for children. However, awarenes … tudents were aware of PHT. In contrast, almost all congenital heart surgeons (96.88%) and pediatric cardiologists (100%) were aware of PHT. Although surgeons and cardiologists understand the concepts of PHT, cardiologists were less likely to recommend and inform their patients about the procedure if they meet the criteria (Likert scale scores of 4.68 vs 3.14, P …

Use of a remotely controlled mechanical pump for coronary arteriography: a study of radiation exposure and quality implications.

BACKGROUND: Exposure to radiation is a hazard of invasive cardiology. To minimise the risk it is essential to keep the doses received as low as possible. AIM: To assess the effect on cardiologist radiation exposure and the quality of coronary artery opacification of the use of a remotely controlled mechanical pump for coronary arteriography. A secondary aim was to assess any disadvantages and safety. METHODS: 319 patients were randomised to have coronary arteriography carried out with contrast injected either by hand or by a remotely controlled mechanical pump. Six cardiologists participated…

Cardiac conduction disturbances in rheumatologic disease: a cross-sectional study.

BACKGROUND: Cardiovascular abnormalities are common in patients with rheumatologic disorders. Tachy-arrhythmias occur more frequently in these patients than the general population; however, the prevalence of bradyarrhythmias in this group is less clear. This investigation aimed to analyze the incidence and predictors of bradyarrhythmias and conduction disturbances, as well as the presence and influence of cardiologist management, in patients with rheumatologic disorders. METHODS: We performed a retrospective chart review of 57 consecutive patients with rheumatologic conditions evaluated at a

[Disappearing borders between cardiology and cardiothoracic surgery: status quo].

Cardiology and cardiothoracic surgery are closely related so that collaboration and communication are required to offer optimal therapy for patients. During the last decades many innovations have reduced the borders between cardiology and cardiothoracic surgery. Today, cardiologists may perform coronary interventions with good results that would have previously been the domain of coronary bypass surgery. In addition new valvular interventions have been developed, such as transfemoral or transapical aortic valve implantation and endovascular mitral valve reconstruction. New developments in card…

Congenital heart disease in adults: catheterization laboratory considerations.

Congenital heart defects are the most common birth defects and represent an increasing proportion of adolescent and adult patients followed by cardiologists. While many of these patients have undergone successful palliative or corrective surgery with excellent functional results, most of them still require careful follow-up. Further, even complex lesions may first be diagnosed in adolescence and adulthood. Therefore, cardiologists caring for adults need to become more familiar with these defects. Assessment of the patient with known or suspected congenital heart defects requires a careful hist…

Hepato-cardiac disorders.

Understanding the mutual relationship between the liver and the heart is important for both hepatologists and cardiologists. Hepato-cardiac diseases can be classified into heart diseases affecting the liver, liver diseases affecting the heart, and conditions affecting the heart and the liver at the same time. Differential diagnoses of liver injury are extremely important in a cardiologist's clinical practice calling for collaboration between cardiologists and hepatologists due to the many other diseases that can affect the liver and mimic haemodynamic injury. Acute and chronic heart failure ma…

Age, Sex, and Gene Expression Score identifies a symptomatic, nondiabetic male patient as being at high risk of obstructive coronary artery disease.

In October 2015, a 74-year-old Caucasian male patient (past medical history of hyperlipidemia, paroxysmal atrial fibrillation, hypertension, and hypothyroidism) presented to the cardiologist for follow-up outpatient evaluation of exertional chest pain. The patient had recently been seen at the Emergency Department for the same complaint. At that time, the patient's cardiac markers, EKG, and pharmacological nuclear stress testing were all reported as normal. At presentation to the cardiologist, the patient's physical examination findings were unremarkable. Over the course of the following year,

Innocent murmurs: parental viewpoint, what are the most common misconceptions and how to avoid them.

The aim of this study was to investigate the parental reaction after they have been informed that their child has a heart murmur. This study also explored whether their reaction was influenced by the fact that the heart murmur is innocent, which actually means that the child is healthy. One hundred parents participated in this cross-sectional study. According to the statistical results, minor parent concern was notable after cardiologist's examination and consulting. Whereas before the cardiologist's examination 98% of parents were concerned about their child's health, later, less than half of
