[Difficult diagnosis: acute appendicitis and appendiceal/cecal carcinoma].

INTRODUCTION: Primary carcinoma of the appendix vermiformis/caecum is very rare. The diagnosis can often be ascertained only postoperatively by histopathological means. Preoperative diagnosis is unusual. METHOD AND RESULTS: We report on five patients with an adenocarcinoma of the appendix/base of caecom, all operated on between July 1998 and October 1999. CONCLUSION: A hemicolectomy "en principe" is not necessary. However, ileocaecal resection should have been performed during the first operation and there should be no positive lymph nodes. Postoperative diagnosis of a carcinoma after simple

Rare Cause of Lower Gastrointestinal Bleeding-Case Presentation and Literature Review.

Background and Clinical Significance: Gastrointestinal bleeding is a critical medical emergency, with upper gastrointestinal bleeding occurring approximately five times more frequently than lower gastrointestinal bleeding (LGIB). The incidence of LGIB tends to increase with age, likely due to a greater prevalence of vascular and diverticular diseases among older patients. However, there are rare or extremely rare causes of LGIB that demand significant diagnostic and therapeutic efforts, some of which may pose unexpected challenges during surgery. Case report: We present the case of a 75-year-o

A case report of a giant appendiceal mucocele and literature review.

A 43-year-old female presented at the accident and emergency department of Princess Marina Hospital, Gaborone, Botswana. She reported a deep dull aching pain of two years duration in the right iliac fossa that has been progressively becoming worse. Ultrasound revealed a large sausage like cystic mass extending from the pelvis up to the medial aspect of the ascending colon. CT scan showed a large sausage like cystic mass extending from the pelvis up to the hepatic flexure of the colon with the cecum displaced. No metastatic features were seen. We made an impression of appendiceal mucocele. A se

Bacteremias caused by Selenomonas artemidis and Selenomonas infelix.

We report two different cases of bacteremia caused by two recently described Selenomonas species, Selenomonas artemidis and Selenomonas infelix. Both species are normally found in human buccal flora. S. artemidis bacteremia appeared in a patient (number 1) who presented with an air-fluid pulmonary cavity and clinical conditions consistent with an anaerobic lung abscess. While the patient improved with antibiotic therapy, cultures of respiratory secretions yielded Mycobacterium tuberculosis. This case demonstrated a strong possibility of a coexisting lung abscess due to S. artemidis. S. infelix

[Peliosis hepatis: 2 new cases].

Peliosis Hepatis is a liver disease characterized by the presence of cystic spaces fill of blood visible through the capsule and on the cutting surface. The etiology has been related to the use of anabolic steroids in cases with malignant tumors and cronic diseases. Only fifty cases have been reported in the world literature. Two anatomical patterns have been described: parenchyma tous and phlebectatic. A combinated pattern including both lesions was also observed. We presented two cases: a 83 years old patient with a cecum carcinoma who developed a phlebectatic pattern, and a 63 years old wom

Primary mucinous adenocarcinoma of the ileum.

A laparotomy was performed on a 67-year-old woman with a provisional diagnosis of intestinal obstruction from a primary small bowel carcinoma. A pathological examination showed a mucinous adenocarcinoma of the ileum, with no regional lymph node metastases. There was no evidence of any other abnormality in the peritoneal cavity. Ten years after the operation, an abdominal computed tomography scan showed a recurrent tumor in the peritoneal cavity. Primary small bowel adenocarcinoma is rare in all malignant gastrointestinal neoplasms, and very few cases of the ileal mucinous adenocarcinoma have b

Laparoscopic resection of a cecal carcinoma with a Retzius vein shunt: A case report.

RATIONALE: A Retzius shunt between the ileocecal vein and inferior vena cava is rare. To avoid major hemorrhage due to shunt injury, it is essential to evaluate vascular abnormalities on preoperative imaging and to confirm anatomic structures in detail during the operation. PATIENT CONCERNS: A 71-year-old woman with diabetes and asthma developed anaemia. DIAGNOSES: She was diagnosed with cecal cancer by endoscopy and found to have a venous malformation forming a Retzius shunt from the ileocecal vein to the inferior vena cava on computed tomography. INTERVENTION: Laparoscopic ileocecal resec

Vaginal natural orifice transluminal endoscopic surgery (vNOTES) right hemicolectomy with intracorporeal anastomosis for cecal cancer.

Vaginal natural orifice transluminal endoscopic surgery (vNOTES) for colorectal cancer utilizes transvaginal access for bowel mobilization, vascular pedicle ligation, oncological resection, and bowel anastomosis, along with subsequent transvaginal natural orifice specimen extraction (NOSE), reducing or eliminating the need for transabdominal access. In this report, we describe the technique of vNOTES right hemicolectomy for cecal cancer, with intracorporeal anastomosis and transvaginal NOSE, including a step-by-step operative video. The patient was a 59-year-old Chinese female (body mass index

Rare Case of Cecal Signet Ring Cell Carcinoma with Distal Intramural Spread.

In this paper, we report a rare case of cecal Signet ring cell carcinoma (SRCC) with Distal intramural spread (DIS) along with a review of the literature. A 71-year-old woman suffering from vomiting, abdominal pain, and abdominal distension was admitted to a hospital and was suspected to have ileus. She was transferred to our hospital and diagnosed with cecal cancer with intestinal obstruction. Laparotomy was performed, after which she was diagnosed with cecal SRCC by histopathological examination. A submucosal lesion was located 55 mm from the distal side of the main tumor. This lesion was al

Laparoscopy-assisted resection of ileocecal intussusception caused by ileal pedunculated lipoma.

We report on a case of ileal lipoma that prolapsed into the ascending colon and was resected by laparoscopy-assisted surgery. A 31-year-old male Japanese patient was admitted to our hospital because of hematochezia and anemia. Colonoscopy revealed a pedunculated polyp arising from the ileum. The surface was covered with slightly edematous mucosa. Abdominal computed tomography showed a low-density mass in the ascending colon. A diagnosis of pedunculated ileal lipoma with intussusception was made, and laparoscopy-assisted surgery was performed. The intussusception was reducted by resection of th

Multi-detector row CT: spectrum of diseases involving the ileocecal area.

The ileocecal area is a relatively short segment of the gastrointestinal tract but may be affected by pathologic conditions that are either common throughout the gastrointestinal system or exclusive to this area. These conditions include benign and malignant tumors, inflammatory processes (appendicitis, diverticulitis, epiploic appendagitis, Crohn disease), infectious diseases, and miscellaneous conditions (cecal ischemia, typhlitis, cecal volvulus, duplication cyst). The various components of the ileocecal area (cecum, appendix, ileocecal valve, terminal ileum) are close to one another, so th

Benign lymphoid polyp (localized lymphoid hyperplasia) at the tip of the cecum associated with lymphoid hyperplasia of the appendix.

A case of benign lymphoid polyp (localized lymphoid hyperplasia) manifesting as a sessile polypoid lesion in the tip of the cecum is described. Solitary lymphoid polyps are extremely rare in the proximal part of the large bowel. The polyp was located at the base of the appendix, and microscopically both the polypoid lesion and the appendix revealed lymphoid hyperplasia.

[A case of advanced colon cancer resected successfully after neoadjuvant chemotherapy].

This case concerns a 54-year-old male patient who had been identified as having a type 2 tumor in the cecum, which subsequent pathologic examination revealed to be an adenocarcinoma. The results of a computed tomography (CT) scan suggested that the tumor had directly invaded the right iliopsoas. Neoadjuvant chemotherapy was performed to avoid a non-curative resection. CT and positron-emission tomography(PET) findings after the 6th course of chemotherapy revealed a significant reduction in tumor size, at which point a right hemicolectomy with D3 nodal dissection was performed. The changes from
