From policy to person-centred care: insights into the development of complications of excess weight clinics in England.

BACKGROUND: The Complications of Excess Weight (CEW) Programme, funded centrally by NHS England, aims to provide innovative, person-centred, holistic care for children and young people living with severe obesity and associated complications (e.g. hypertension, type 2 diabetes, depression). Twenty-one CEW services were established across England in 2021, rising to 38 in 2024. This qualitative study aimed to gather the perspectives of senior professionals involved in the design, development, and commissioning of the CEW programme. METHODS: Eleven online semi-structured interviews were completed

Assessing Cultural, Religious, and Spiritual Confidence and Perceived Preparedness in Community Palliative and End-of-Life Care: A Service Evaluation.

Background: Cultural, religious, and spiritual (CRS) needs are central to holistic palliative and end-of-life care (PEoLC), yet the confidence and perceived preparedness of community and voluntary sector staff in addressing them remain underexplored. As PEoLC increasingly occurs in community settings, understanding staff preparedness for culturally and spiritually sensitive care is vital. Objective: This service evaluation examined CRS perceived preparedness and confidence among staff across Leicester, Leicestershire, and Rutland (LLR), exploring perceived challenges and available resources. Conversational, Longitudinal, Ecological Assessment (CLEA): Exploring a new AI-driven method for qualitative data collection in a behavioural health context.

In this paper, we present conversational longitudinal ecological assessment (CLEA), a novel conversational AI-enabled method for collecting ecologically valid, temporally sensitive qualitative health data via mobile instant messaging. We report findings from an exploratory deployment of an instantiation of CLEA within a 12-week community-based weight management programme, delivered by a charity partner in an area of deprivation. Using WhatsApp, we deployed our CLEA chat-agent to conduct twice-weekly conversational data collection sessions with participants, to elicit data about their experienc

Understanding maternity care provision and experience for autistic women and birthing people and their care providers in the UK: protocol for a scoping review.

INTRODUCTION: Maternity care in the UK is under increasing scrutiny, with reports indicating that services often fail to meet the diverse needs of women and birthing people. Autism spectrum disorder is a neurodevelopmental condition involving persistent difficulties in social communication and interaction alongside restricted and repetitive behaviours. Autistic women and birthing people may face distinct barriers to accessing and navigating maternity care, compounded by limited awareness and inflexible service provision. To improve care responsiveness, it is essential to understand both the ch

Medical Student Perspectives on Female Anatomical Representation in CPR Training.

BACKGROUND: Females who experience an out-of-hospital cardiac arrest (OHCA) are more likely to die than males in the same scenario, with bystanders often reluctant to perform CPR due to worries or uncertainty around touching/exposing breasts. This study aims to explore the perspectives of UK medical students on female anatomy representation in CPR training, as medical students generally attend multiple CPR training sessions, and this training shapes their future clinical practice. METHODS: A Qualtrics questionnaire was formulated, featuring quantitative and qualitative questions. This questio

Communication with children about parental bipolar disorder: a qualitative interview study.

BACKGROUND: The impacts of parental bipolar disorder (BD) on families and children highlight the need to understand how best to talk to children about their parents' diagnosis, especially as their developmental capacity for understanding grows. This qualitative study aims to explore the strategies, challenges, and support needs of parents in relation to communicating with their children (5-12 years) about BD, in order to inform the development of further interventions and resources. METHODOLOGY: Purposive and snowball sampling strategies were used to recruit parents with BD, their partners, a

Under-Served Groups and Myalgic Encephalomyelitis Research Workshop; Multiple Barriers to Effective Healthcare, Research and Public Participation.

Public involvement in research and other initiatives for myalgic encephalomyelitis (ME) (also known as chronic fatigue syndrome) has been crucial in raising awareness of the disease and exposing inadequate healthcare and research funding. An online workshop on ME research and under-served groups took place in July 2024, organised by the first author, a person with ME. The workshop illustrated very low prevalence and thus barriers to healthcare as well as limited research in people from under-served groups, who appear doubly disadvantaged by their illness and their socioeconomic and/or ethnic b

Christian perspectives on palliative sedation: a literature study.

BACKGROUND: Christianity has always been concerned about the fate of the sick and the dying. Charity is one of the seven Christian virtues and is seen as an act of love for one's neighbour. At the same time, Christianity is a strong defender of the dignity of human life, especially the most vulnerable. In the context of advanced disease and end of life, all major Christian traditions (Anglican, Catholic, Eastern Orthodox and Protestant) advocate appropriate care to protect the dignity of those suffering from illness or facing the prospect of death. This translates into a strong opposition to a

'She didn't know what to do with me': The experience of seeking community mental health support after spinal cord injury.

CONTEXT/OBJECTIVES: Adults with spinal cord injury in the UK do not currently have specialized access to SCI-informed community-based mental health support, despite their elevated risk of mental health decline. The lack of SCI-informed therapeutic support may increase the likelihood of mental health treatment failure. This study sought to qualitatively explore the experience of accessing, or attempting to access, generic (non-SCI-informed) mental health support when living with a spinal cord injury. DESIGN: Qualitative, exploratory study using thematic analysis. SETTING: Community-based samp

A randomized controlled trial of self-help cognitive behavioural therapy for depression in adults with pulmonary hypertension.

OBJECTIVES: Pulmonary hypertension (PH) is a progressive, life-reducing group of conditions associated with an elevated risk of depression. To meet this clinical need, we developed an unguided self-help intervention targeting depression in PH based on cognitive behavioural therapy (CBT) and tested its acceptability, feasibility, and effectiveness. DESIGN: A randomized controlled trial (RCT) design was utilized with a wait-list control group. Acceptability was assessed using content analysis. METHODS: Adults self-reporting difficulties with depression were recruited from global Pulmonary Hype

Patient Experience and Preferences for the Assessment of Olfaction: The Patient International Clinical Assessment of Smell Survey.

INTRODUCTION: Olfactory dysfunction (OD) is common and carries significant personal and societal burden of disease. Accurate assessment of olfaction is required for good clinical care and affords patients insight into their condition. However, the accuracy of assessment varies with technique used, and there is presently little standardisation of clinical practice. We therefore aimed to determine experience of and preferences for olfactory assessment in healthcare-seeking adults. METHODS: An anonymous patient co-produced survey was developed in collaboration with a UK-based OD charity. Distrib

Protocol for the development and validation of a patient-reported experience measure (PREM) for people with hearing loss: the PREM-HeLP.

INTRODUCTION: Hearing loss is a common chronic health condition and adversely affects communication and social function resulting in loneliness, social isolation and depression. We know little about the patient experience of living with hearing loss and their views on the quality of the audiology service. In this study, we will develop and validate the first patient-reported experience measure (PREM) to understand patients' experiences of living with hearing loss and their healthcare interactions with audiology services. METHODS AND ANALYSIS: We will develop the PREM in three phases: (1) deve

Phenoage and longitudinal changes on transthoracic echocardiography in Alström syndrome: a disease of accelerated ageing?

Alström syndrome (AS) is an ultra-rare disorder characterised by early-onset multi-organ dysfunction, such as insulin resistance, obesity, dyslipidaemia, and renal and cardiovascular disease. The objective is to explore whether AS is a disease of accelerated ageing and whether changes over time on echocardiography could reflect accelerated cardiac ageing. Cross-sectional measurement of Phenoage and retrospective analysis of serial echocardiography were performed between March 2012 and November 2022. The setting is a single national tertiary service jointly run by health service and patient cha

Designing and evaluating a patient decision aid for patients with locally advanced or locally recurrent rectal cancer: a national multicentre mixed methods study protocol.

INTRODUCTION: Approximately 5%-10% of new rectal cancers are locally advanced (locally advanced rectal cancer (LARC)) at presentation with 4%-8% recurring (locally recurrent rectal cancer (LRRC)) after initial treatment. Patients with potentially curable disease have to consider many trade-offs when considering major exenterative surgery. There are no decision tools for these patients and current resources have found to not meet minimum international standards. The overall aim of this study is to produce a validated patient decision aid (PtDA) to assist patients considering radical pelvic exen
