Childhood adversity is associated with hospitalisations and survival following external causes and non-communicable diseases: a 46-year follow-up of a Stockholm birth cohort.

BACKGROUND: Childhood adversity indicated by involvement with child welfare services (ICWS) is associated with increased risks of disease and injuries in young adulthood. It is yet unknown whether such risks are limited to external causes and mental and behavioural disorders or whether they extend beyond early adulthood and to non-communicable diseases (NCDs) with later onset. Moreover, it has not been explored whether ICWS associates with decreased survival prospects following hospitalisation. METHODS: Based on prospective data for a 1953 Stockholm birth cohort (n=14 134), ICWS was operation

Drug overdose and child maltreatment across the United States' rural-urban continuum.

This national study of US counties (n = 2963) investigated whether county-level drug overdose mortality is associated with maltreatment report rates, and whether the relationship between overdose mortality and maltreatment reports is moderated by a county's rural, non-metro or metro status. Data included county-level 2015 maltreatment reports from the National Child Abuse and Neglect Data System, modeled drug-overdose mortality from the Centers for Disease Control, United States Department of Agriculture Rural-Urban Continuum Codes, US Census demographic data and crime reports from the Federal

Thoracoscopic Mesh Repair of Diaphragmatic Defects in Hepatic Hydrothorax: A 10-Year Experience.

BACKGROUND: The objective was to analyze the outcomes of thoracoscopic mesh repair for hepatic hydrothorax (HH) at our institution during the past 10 years. METHODS: A total of 63 patients with refractory HH who underwent thoracoscopic mesh onlay reinforcement to repair diaphragmatic defects from January 2005 to December 2014 were included in the study. Mesh covering alone was used in 47 patients and mesh with suturing was used in 16 patients. Patient demographics, Child-Pugh class, and model for end-stage liver disease (MELD) score were evaluated to predict morbidity and mortality. RESULTS:

Prevalence of Pneumonia and Its Associated Factors Among Children Under Five Years in Eastern Uganda: A Cross-Sectional Hospital-Based Study.

BACKGROUND: Pneumonia remains the leading cause of morbidity and mortality in children under five, accounting for 740,180 deaths globally and 14% of all under-five deaths. In Uganda, it is the second leading cause of mortality in this age group. Despite several studies on pneumonia risk factors, findings have been inconsistent. This study aimed to determine the prevalence and associated factors of pneumonia in children under five in Eastern Uganda. METHODS: A hospital-based descriptive and analytical cross-sectional study was conducted among 385 children aged 2 to 59 months admitted to the pe

Neighborhood deprivation is a risk factor for severe child physical abuse: A multicenter cohort investigation.

BACKGROUND: Our purpose was to investigate whether neighborhood deprivation is associated with outcomes in a multicenter population of children with suspected or confirmed child physical abuse. We hypothesized that community level social determinants of health are associated with worse outcomes following child physical abuse. METHODS: This multicenter retrospective review included children (18 years or younger) admitted with suspected or confirmed physical abuse at six pediatric trauma centers. A national Area Deprivation Index (ADI) score was assigned to each patient based on home address. A

Morbidity and mortality of children aged 2-59 months admitted in the Tanzania Lake Zone's public hospitals: a cross-sectional study.

BACKGROUND: There is a growing concern about child mortality especially in developing countries. The Government of Tanzania and non-governmental organizations are fighting against diseases like malaria, anaemia, diarrhoea and pneumonia that contribute extensively to child mortality. This was a hospital-based, retrospective cohort study involving 1130 under-fives (excluding neonates) being either discharged from or died in public hospitals of the Lake Zone in Tanzania. We extracted information on symptoms and signs at admission, major diagnoses and causes of death from the medical records. We a…

Long-term survival for infants born with orofacial clefts in Western Australia.

BACKGROUND: Only two population-based studies have reported survival beyond 15 years for individuals with orofacial clefts (OFC), and only for individuals with isolated OFC. Compared with the general population, long-term survival was similar for individuals with cleft lip only, and lower for individuals with cleft palate only. Results for those born with isolated cleft lip and cleft palate were inconsistent. METHODS: Using linked population-based health data, including a congenital anomaly register with active surveillance and diagnoses up to 6 years, we compared survival at 1, 5, and 20 yea

Atypical Kawasaki Disease in a 4 Years Old Child with Mumps.

Kawasaki disease is an acute febrile condition seen in children. However, it is also well recognized that some patients do not fulfill the classic diagnostic criteria for the diagnosis of Kawasaki disease. The incomplete form of Kawasaki disease is termed as 'Incomplete KD' or 'Atypical KD'. This is a case of 4 years old child with fever and mumps. He had bilateral cervical adenitis. Patient failed to respond to IV antibiotics fulfilled the criteria of incomplete Kawasaki disease. The child was managed with high dose aspirin until the child was afebrile for 48 hours. Kawasaki disease is a comm

Munchausen syndrome by proxy: a profile for medical child abuse.

The term Munchausen Syndrome by Proxy describes a pattern of physical abuse in which a perpetrator produces or fabricates illness in a victim. Victims are almost always children. We alternatively call this behaviour medical child abuse. Our report describes five recent cases. Two presented with infantile apnoea, one with alleged hypoventilation, one with recurrent vomiting and one with recurrent infections. Three of the perpetrators were mothers, two were fathers. In three cases diagnoses were confirmed by covert video surveillance. Contrary to popular beliefs, this behaviour is not rare and i

Efficacy of Child Abuse Evaluations for Infants With Possible Subdural Hemorrhage Identified on Cranial Ultrasound Completed for Macrocephaly.

Abusive head trauma (AHT) is a significant cause of morbidity and mortality for infants. Determining when to pursue a complete physical abuse evaluation can be difficult, especially for nonspecific findings or when a child appears clinically well. This retrospective study of 7 cases sought to describe the presentation, evaluation, and diagnoses for infants with abnormal subdural collections identified on cranial ultrasound for macrocephaly, and to determine how frequently AHT is diagnosed. The results of this study showed that while each patient presented due to asymptomatic macrocephaly, the

Structural equation modelling (SEM) for malaria prevalence and risk factors in Uganda.

BACKGROUND: Malaria remains a leading cause of morbidity and mortality among children under five years in sub-Saharan Africa, despite extensive public health efforts. Its transmission is influenced by a complex interaction of socioeconomic, environmental, maternal, and child health factors. Traditional analytic approaches often fail to capture these multifaceted relationships. This study employs Structural Equation Modelling (SEM) to explore the latent and observed predictors of child malaria prevalence, offering a comprehensive understanding of the underlying pathways. METHODS: Utilizing sec

Acute-on-Chronic Subdural Hemorrhage Due to Late Vitamin K Deficiency.

Vitamin K deficiency bleeding (VKDB) is closely associated with the hemorrhagic disease of the newborn (HDN) and can have a late onset, after one week of birth up to six months of age. It is a major concern in developing countries where vitamin K prophylaxis is not often given to newborns and can lead to significant mortality and morbidity. We report a case of a three-month-old child who was exclusively breastfed. He presented with repeated vomiting and was eventually diagnosed as a case of acute-on-chronic subdural hemorrhage. Timely diagnosis and surgical intervention played a key role in en

[Are surgical shunts still indicated?].

The indication for portosystemic shunts has been influenced by endoscopic approaches, liver transplantation and TIPSS. Underlying disease and functional capacity of the liver are crucial for the therapeutic strategy in patients with esophageal variceal hemorrhage. In Child B and C cirrhosis results of liver transplantation are superior to surgical shunts. In transplantation candidates shunt surgery should not interfere with the transplantation procedure. Therefore, TIPSS, mesocaval shunt or Warren shunt are recommended. If transplantation is not indicated, surgical shunts are generally justifi
