Radionuclide hepatobiliary imaging is a useful procedure for the diagnosis of acute cholecystitis. Visualization of the gallbladder essentially rules out acute cholecystitis. Nonvisualization suggests acute cholecystitis but may also be associated with chronic gallbladder disease or other conditions. We recently observed five patients in whom a rim of increased parenchymal liver activity was seen adjacent to the gallbladder fossa. All five patients had acute gangrenous cholecystitis. The rim of increased activity appears to be a useful secondary sign of acute cholecystitis.

A slight rise in bilirubin values and rises of the serum transaminases belong to the symptoms of acute cholecystitis and do not necessarily stand for an occlusion of the bile duct, i.e. for a cholestase. They admit conclusions as to the seriousness of the inflammation and especially the alkaline phosphatase gives clear indication of additional choledocholithiasis. Even with a general attitude in favour of early surgery, there should be undertaken a comprehensive laboratory diagnostic prior to surgery.

The study was performed to assess the pathogenic basis of sonographically demonstrable changes in the gallbladder wall in acute cholecystitis in an attempt to predict the degree of inflammation and to define a set of sonographic criteria for the diagnosis of acute cholecystitis. Sonograms in a control group of 30 patients (group A) without biliary tract symptoms, ascites, or cholelithiasis and 24 patients (group B) with proven diagnosis of acute cholecystitis were reviewed. The histologic sections of the gallbladder wall in the cholecystitis patients were correlated with sonographic findings. …
