Clinical characteristics and management of dasatinib-induced chylothorax: retrospective analysis based on case reports.

BACKGROUND: Dasatinib-induced chylothorax is a rare but potentially serious complication, and its clinical features remain poorly defined. This study aimed to systematically evaluate its clinical characteristics, therapeutic approaches, and patient outcomes to provide practical guidance for clinical management. METHODS: We conducted a retrospective analysis by systematically retrieving case reports of dasatinib-induced chylothorax from relevant databases up to 31 May 2025, to achieve a comprehensive assessment. RESULTS: A total of 24 patients from 22 published case reports and case series we…

Selective Lymphatic Duct Embolization in Patients With Chylothorax and Thoracic Duct Side-Branch Injury.

Chylothorax, the accumulation of chyle in the pleural space, is a potentially fatal condition that necessitates intervention. Options range from conservative treatment, such as dietary modifications and pharmacological therapy, to percutaneous and surgical interventions. This case describes a 56-year-old male status post-lung wedge resection who developed a high-output chylothorax. Initial management with octreotide and a low-fat diet was unsuccessful. A percutaneous lymphangiogram identified extravasation of contrast from a thoracic duct side-branch. Instead of traditional thoracic duct embol…

Bilateral Chylothorax due to Lymphatic Reflux Into the Visceral Pleura From Thoracic Duct Obstruction.

A chylothorax associated with lymphatic reflux is extremely rare. A 77-year-old woman presented with exertional dyspnea. Chest radiography revealed bilateral pleural effusion, and thoracentesis confirmed chyle. Idiopathic chylothorax was diagnosed as no secondary cause was identified. Lymphangiography revealed thoracic duct obstruction cranial to the hilum. A follow-up computed tomography scan showed marked accumulation of contrast material in both the lung parenchyma and hilar regions, a pattern suggestive of lymphatic reflux into the lungs. After left-sided thoracentesis, contralateral pleur…

Post-Esophagectomy Chylothorax with Thoracic Duct Anomaly Successfully Treated with Lymphangiography: A Case Report.

INTRODUCTION: Chylothorax following esophagectomy is a relatively rare but significant complication, with incidences ranging from 1.1% to 3.8%. It typically arises from damage to the thoracic duct or associated lymphatic vessels during extensive lymph node dissection and thoracic surgery. Initial management usually relies on conservative approaches such as dietary modifications, drainage, and pharmacotherapy. If these methods are ineffective, surgical intervention becomes necessary. However, standardized guidelines for the optimal management of thoracic duct injuries are as yet lacking, with d…

Outcomes of Chylothorax Nonoperative Management After Cardiothoracic Surgery: A Systematic Review and Meta-Analysis.

INTRODUCTION: Chylothorax after thoracic surgery is a severe complication with high morbidity and mortality rate of 0.10 (95% confidence interval [CI] 0.06 - 0.02). There is no agreement on whether nonoperative treatment or early reoperation should be the initial intervention. This systematic review and meta-analysis aimed to evaluate the outcomes of the conservative approach to treat chyle leakage after cardiothoracic surgeries. METHODS: A systematic review was conducted in PubMed®, Embase, Cochrane Library Central, and LILACS (Biblioteca Virtual em Saúde) databases; a manual search of refer…

Thoracic duct embolization in treating postoperative chylothorax: does bail-out retrograde access improve outcomes?

OBJECTIVES: To evaluate clinical outcomes of thoracic duct embolization (TDE) for the management of postoperative chylothorax with the aid of the bail-out retrograde approach for thoracic duct cannulation (TDC). MATERIALS AND METHODS: Forty-five patients with postoperative chylothorax underwent Lipiodol lymphangiography (LLG) between February 2016 and November 2019. If targetable central lymphatic vessels were identified in LLG, TDC, a prerequisite for TDE, was attempted. While the conventional antegrade transabdominal approach was the standard TDC method, the retrograde approach was applied …

Does V-A ECMO increase the likelihood of chylothorax after congenital diaphragmatic hernia repair?

BACKGROUND: The authors noticed a relatively large number of patients with congenital diaphragmatic hernia (CDH) repair after extracorporeal membrane oxygenation (ECMO) who had a chylothorax (CT). The data are reviewed. METHODS: The charts of patients from 1990 until 2000 with CDH, treated with or without ECMO, together with the charts of patients treated with ECMO for other reasons and patients with esophageal atresia (EA) repair were reviewed. The diagnosis of CT was made if aspirated fluid appeared chylous and contained more than 90% lymphocytes or if the triglyceride level was more than 1…

Treatment strategy for chylothorax after pulmonary resection and lymph node dissection for lung cancer.

OBJECTIVE: We reviewed our experience with iatrogenic chylothorax after pulmonary resections for lung cancer to evaluate our treatment strategy and to identify factors that predict the need for reoperation. METHODS: From July 1992 through February 2000, a total of 1110 patients underwent pulmonary resection (at least lobectomy) and systematic mediastinal lymph node dissection for lung cancer at our division. Twenty-seven patients (2.4%) had postoperative chylothorax develop. We initially treated 26 of these patients conservatively with complete oral intake cessation and total parenteral nutri…

Severe bilateral fibrosing pleuritis associated with chronic chylothorax in five cats and two dogs.

Severe bilateral fibrosing pleuritis was diagnosed in 5 cats and 2 dogs with chronic chylothorax. All animals were dyspneic on initial examination and remained moderately to severely dyspneic after thoracentesis. Radiographic evidence of fibrosing pleuritis included rounded lung lobes and failure of the lungs to reexpand following effective pleural drainage. Fibrosing pleuritis was also suggested in several animals with radiographic evidence of pleural fluid, in which pleural fluid could not be retrieved. Macroscopically, the lung lobes of all animals were compressed and atelectatic to various…

Chylothorax--a rare complication after anterior and posterior spinal correction. Report on six cases.

The authors observed six cases of chylothorax of 2,000 operations for spinal deformities performed in the past 5 years: in one case of VDS, two Harrington operations, and three anterior strut graft operations. Chylothorax resulted from direct injury to the lymphatic system in four cases of anterior spinal surgery. One of the Harrington cases had a previous history of spontaneous chylothorax that had been managed surgically. The other cases of chylothorax observed after a Harrington operation was bilateral and probably caused by the central venous catheter. All cases were treated conservatively…

Pulmonary tuberculosis occurring during the diagnosis and treatment of sarcoidosis complicated by chylothorax: A case report.

Sarcoidosis is a systemic granulomatous disorder, and chylothorax is a rare but severe pleural complication of this disease and exhibits a prominent diagnostic overlap with tuberculosis. The present study reported the case of a 48-year-old woman who was admitted in May 2018 with chronic cough and chest tightness. After hospitalization for 17 days, comprehensive examinations excluded tuberculosis and malignancy, and the patient was finally diagnosed with sarcoidosis complicated with pleural effusion. Treatment with prednisone combined with therapeutic thoracentesis was initiated to relieve symp…

Chronic Chylothorax in Childhood: Clinical Complexity and Management Challenges-A Case Report and Literature Overview.

Background and Clinical Significance: Pleural effusion results from an imbalance between fluid production and lymphatic drainage within the pleural space and may arise from a wide range of conditions. Chylothorax, characterized by the accumulation of chyle in the pleural cavity, is an uncommon cause of pleural effusion in the pediatric population. Although octreotide has been widely used in neonatal postoperative chylothorax, evidence supporting its role in acquired chylothorax beyond infancy remains limited, and dosing strategies are not standardized. This report describes the management of m…

Re-evaluating the anatomy of iatrogenic chylothorax - lymphangiography imaging of the transverse branch of the thoracic duct.

Iatrogenic chylothorax is generally attributed to injury to the main thoracic duct and its tributaries. However, the current anatomic classification of the thoracic duct primarily focuses on the morphology and outlet of the main duct. Using thoracic ductography, this study aimed to identify the presence of a large-caliber transverse branch of the thoracic duct and to evaluate its role as a cause of chylothorax. This retrospective study included 25 patients who underwent thoracic ductography for chylous leakage between May 2020 and May 2025. Thoracic duct branching patterns and leakage sites we…

A case of severe retroperitoneal chyle leakage and subsequent chylothorax secondary to partial laparoscopic nephrectomy.

Retroperitoneal chyle leakage combined with chylothorax after nephrectomy is rare in clinical practice. We report a 41-year-old female who developed severe chyloretroperitoneum along with subsequent chylothorax after partial nephrectomy. The patient underwent nutritional control, received octreotide treatment and had a complete recovery after 58 days. A damage to the cisterna chylic could account for the leakage. The chylothorax was a displacement through the diaphragmatic hiatus. Comprehensive review showed that near 30 % of such patients would require re-surgery or interventional treatment. …

Use of a Postoperative Care Management Pathway Reduces the Incidence of Chylothorax Post-Fontan Palliation.

Pleural effusions and chylothorax are challenging morbidities post-Fontan palliation. We sought to evaluate the efficacy of our Fontan Care Pathway (FCP) in reducing the incidence of post-operative chylothorax and Time to Chest Tube Removal (TTCTR), and to determine risk factors associated with longer TTCTR. Between 2016 and 2022 our institutional approach to post-Fontan care fell into three categories: Group 1 (n = 36): no standardized approach; Group 2 (n = 30): a prophylactic chylothorax diet (fat content < 5%); Group 3 (n = 57): the FCP (a chylothorax diet, fluid restriction, supplemental …

Traumatic Chylothorax Following Pulmonary Segmentectomy: A Case Report and Review of Postoperative Investigation and Management.

The incidence of iatrogenic traumatic chylothorax is on the rise secondary to the preferred use of minimally invasive thoracic surgery over thoracotomy. Most reported causes of chylothorax occur following pneumonectomy or lobectomy. There have been no reported cases of traumatic chylothorax following segmentectomy according to our literature review. Complications following lung resection typically include pneumonia, atelectasis, or prolonged air leak. Here, we present a rare case of postoperative chylothorax following minimally invasive segmentectomy to diagnose an enlarging singular pulmonary…

Dasatinib-induced chylothorax in a patient with chronic myeloid leukaemia: a case report and literature review.

Dasatinib is a potent and effective second-generation oral tyrosine kinase inhibitor that is clinically indicated for the treatment of imatinib-resistant or imatinib-intolerant breakpoint cluster region-Abelson (BCR-ABL)-positive chronic myeloid leukaemia (CML) or for Philadelphia chromosome-positive acute lymphocytic leukaemia. The most common adverse events associated with dasatinib therapy are skin rash, gastrointestinal upset, pancytopenia, pulmonary hypertension, and fluid retention, including pleural effusion. However, chylothorax secondary to dasatinib administration has rarely been rep…
