The authors describe a diagnostically difficult case of childhood lymphoma that presented as an atypical polyphenotypic lymphoproliferative reaction. Initial immunophenotyping revealed the presence of IgG, IgA, kappa, and lambda within the neoplastic lymphocytes. The patient had circulating plasmacytoid lymphocytes and a polyclonal hypergammaglobulinemia. The patient died of widespread immunoblastic lymphoma in two months. Postmortem tumor DNA showed a oligoclonal pattern of immunoglobulin heavy chain gene rearrangement. Blots for T-cell receptor beta-chain rearrangement showed germline bands.

Mature teratomas represent 5% to 10% of all mediastinal tumors, and malignant transformation is exceedingly rare. A 51-year-old man with newly diagnosed prostate cancer was incidentally found to have an anterior mediastinal mass suggestive of a teratoma on staging workup. Robotic thymectomy was performed, revealing a mature teratoma with malignant transformation to colorectal-type adenocarcinoma. Circulating tumor DNA was negative for measurable residual disease. The patient was managed with surveillance, including serial imaging and tumor markers.

AIM: The present study aims to compare the host immune responses induced by benign (meningiomas) and malignant (gliomas) brain tumors.
METHODS: Peripheral blood samples from 8 meningioma and 7 glioma patients collected pre- and post operatively were assessed for cell-mediated immunity, humoral immunity and IL-6, IL-8 and TNF-a expression.
