Female genital mutilation: what do we know so far?

Female genital mutilation (FGM) is a practice that is now familiar to UK health professionals. It continues to be a problem in high-risk populations and affects girls and women throughout their lives. Complications related to FGM are poorly reported and documented. Health professionals, therefore, must be aware of the adverse effects and how they affect obstetric, gynaecological and sexual function and general quality of life. They must also be sensitive towards the psychological and emotional issues relating to FGM. Sociocultural barriers make research and data-collection difficult in a cohor

What's "normal": female genital mutilation, psychology, and body image.

Despite international efforts to halt the practice of female genital mutilation (FGM), the number of African girls and women undergoing the procedure is not declining as rapidly as international observers had hoped when the World Health Organization began focusing attention on the practice in the 1960s. This article focuses on the psychological effects of FGM through the example of a patient who had undergone the procedure in childhood and now felt that her closed appearance was "normal" and that to be opened would be "abnormal." Western advocates must educate themselves about the various cult

Perinatal complications among ethnic Somalis in Norway.

BACKGROUND: The majority of ethnic Somali women in Norway have undergone an elaborate form of circumcision (infibulation). The aim of this study was to examine the risk of perinatal complications among ethnic Somalis and to discuss its relation to circumcision. METHODS: We conducted a cross-sectional population-based registry study. Data on all births to women born in Somalia (1733) and Norway (702 192) from 1986 to 1998 was provided from the Medical Birth Registry of Norway. We analyzed the risk of perinatal complications among women of Somali origin vs. ethnic Norwegians using univariate an

Male partner circumcision associated with lower Trichomonas vaginalis incidence among pregnant and postpartum Kenyan women: a prospective cohort study.

OBJECTIVE: Trichomonas vaginalis is the world's most common curable STI and has implications for reproductive health in women. We determined incidence and correlates of T. vaginalis in an HIV-uninfected peripartum cohort. METHODS: Women participating in a prospective study of peripartum HIV acquisition in Western Kenya were enrolled during pregnancy and followed until 9 months post partum. T. vaginalis was assessed every 1-3 months using wet mount microscopy. Correlates of incident T. vaginalis were determined using Cox proportional hazards models. RESULTS: Among 1271 women enrolled, median

Evidence-based (S3) Guideline on (anogenital) Lichen sclerosus.

Lichen sclerosus (LS) is an inflammatory skin disease that usually involves the anogenital area. All patients with symptoms or signs suspicious of lichen sclerosus should be seen at least once initially by a physician with a special interest in the disease in order to avoid delay in diagnosis, as early treatment may cure the disease in some and reduce or prevent scarring. The diagnosis is made clinically in most cases. Biopsies should only be performed under certain circumstances. The gold standard for treatment remains potent to very potent topical steroids; however, mild and moderate disease

[Which paediatric urological procedures can be performed in a general hospital?].

Principally, surgical procedures in children should be performed in specialized pediatric centers. Some minor procedures on older children could eventually take place in general hospitals, provided that pediatric medical and nursing competence is available. Adequate infrastructure and an environment appropriate for children are required as well. The comfort of therapy in an adjacent general hospital should not lead to an increased risk for the child. Principles of clinical diagnosis and treatment strategies of frequent pediatric urological entities are described and discussed in this article.

[Shortest possible muscle relaxation in children using atracurium and/or vecuronium].

The aim of this study was to monitor accelographically the impact of atracurium (A) and/or vecuronium (V) on intubation anaesthesia in infants and/or children for elective surgery or minor short-term surgery. An intra-individual evaluation is valid even though a very practical but less established monitoring method is used. Infants receiving a "priming"-dose of V 0.01 mg kg-1 BW or A 0.05 mg kg-1 BW followed 3 minutes later by a main dose of V 0.04 mg kg-1 BW or A 0.2 mg kg-1 BW were found to be in good or even very good intubation condition. These minimal doses afforded very short relaxation

Heterosexual transmission of human immunodeficiency virus.

Heterosexual human immunodeficiency virus (HIV) transmission accounts for most cases of HIV infection and acquired immunodeficiency syndrome worldwide. Moreover, in the developed world the proportion of individuals infected heterosexually is rising faster than the proportion infected by other routes, with an increase in the number of women infected. Defining the factors associated with heterosexual transmission may help control new infections.

Pediatric office procedures.

The practice of pediatric urology has changed much during the last 25 years. Procedures that were once done only on inpatients are now done as ambulatory cases, comprising more than 60 per cent of all surgery. This trend has continued, with even more cases being done as office procedures. These consist of circumcision, meatotomy for stenosis, lysis of labial adhesions, and meatal dilatation after hypospadias repair. If an operation is done with attention to detail, it can be completed with minimal complications, although, as evidenced with circumcision, those that do occur can carry significan

The circumcision controversy: an update.

One constructive feature has come out of all of the circumcision controversy. It has made us aware that neonatal circumcision is not a simple procedure and that it should not be regarded as "routine." It is a surgical procedure and it should be taught as one. The authors of this article wrote a review article on circumcision in 1981 and Dr. Grossman has recently published a book designed to teach proper circumcision technique, and to familiarize the operator with the common congenital anomalies as well as the prevention and treatment of the complications of circumcision. The advocacy of neonat

The effectiveness of surgical interventions for women with FGM/C: a systematic review.

BACKGROUND: Female genital mutilation/cutting (FGM/C) changes normal genital functionality and can cause complications. There is an increasing demand for treatment of FGM/C-related complications. OBJECTIVES: We conducted a systematic review of empirical quantitative research on the outcomes of interventions for women with FGM/C-related complications. SEARCH STRATEGY: A search specialist searched 16 electronic databases. SELECTION CRITERIA: Selection was performed independently by two researchers. We accepted quantitative studies that examined the outcome of an intervention for an FGM/C-rela

Defibulation to treat female genital cutting: effect on symptoms and sexual function.

OBJECTIVE: Women who have undergone type III female genital cutting may suffer long-term complications. Defibulation (reconstructive surgery of the infibulated scar) can alleviate some of these complications. We studied the physical and sexual outcomes after defibulation and evaluated both patient and husband satisfaction. METHODS: The medical records of 40 consecutive women with a history of type III female genital cutting who underwent defibulation between 1995 and 2003 were reviewed. Data collected included demographics, indications for the procedure, closure type, intraoperative and posto

Female Genital Mutilation in the Democratic Republic of Congo: Prevalence, Types, Sociodemographic and Cultural Characteristics, and Long-Term Pelvic Floor Outcomes.

INTRODUCTION: In 1999, the prevalence of female genital mutilation (FGM) was 5% in the Democratic Republic of the Congo (DRC). This study aims to assess FGM prevalence and types, describe its demographic and sociocultural characteristics, and evaluate its long-term impact on pelvic floor and sexual function. METHODS: A cross-sectional study was conducted from 2021 to 2023 among 519 adult women living in six provinces of the DRC, selected to represent the country's ethnolinguistic diversity. Pregnant women, those within 6 months of childbirth, survivors of sexual violence, and those with war m

A Profile of Gender-Based Violence Research in Europe: Findings From a Focused Mapping Review and Synthesis.

This article reports the findings from a new form of review: a focused mapping review and synthesis. The aim was to create a contemporary, snapshot profile of the nature and scope of gender-based violence (GBV) studies conducted in Europe. GBV is one of the most prevalent human rights violations in the world affecting mainly women and girls. The policy context of GBV in Europe has gathered momentum in recent years, but we do not have a clear picture of how this relates to research activity. Thirteen journals were purposively selected on their likelihood to publish GBV research. All articles pu

Female genital mutilation--experience of The Royal Women's Hospital, Melbourne.

This study was performed to improve our knowledge and understanding of the needs of women affected by female genital mutilation. We looked at the types of complications of these practices which present to a large metropolitan women's hospital in order to determine how we can appropriately treat and support affected women. This was an observational study of women from countries with a high prevalence of female genital mutilation who presented to the Royal Women's Hospital between October, 1995 and January, 1997. Fifty-one patients with a past history of female genital mutilation who were attend

Early Infant Male Circumcision Decisions in Zambia: Demographic and Familial Influences.

Public health initiatives in Zambia encourage the uptake of early infant male circumcision (EIMC) as an HIV prevention strategy. This study assessed EIMC parental decision-making during perinatal care in Lusaka, Zambia, focusing on the influence of sociodemographic factors, family, and friends. A longitudinal pilot perinatal intervention, Like Father Like Son (LFLS), was implemented among 300 couples attending antenatal clinics in four urban community health centers. Participants were assessed postpartum regarding subsequent EIMC decisions. Partners, religion, and marital status were associate

Postoperative analgesia in children who have genito-urinary surgery. A comparison between caudal buprenorphine and bupivacaine.

A study conducted on 40 children, aged 1-11 years, who had genito-urinary surgery compared the quality and duration of analgesia after caudal blocks in two groups of patients. Group 1 (n = 20) received caudal bupivacaine 0.25% and group 2 (n = 20) caudal buprenorphine 4 micrograms/kg; each received 0.5 ml/kg body weight. Patients were operated on under general anaesthesia. Postoperative behaviour and severity of pain were measured on a 3-point scale. The results indicate that caudal buprenorphine provides excellent postoperative analgesia in children comparable to caudal bupivacaine in the ear
