A rare complication in elective repair of an abdominal aortic aneurysm: multiple transmural colonic infarcts secondary to atheroemboli.

Numerous complications may occur during elective and emergency repair of abdominal aortic aneurysms. The following report will document a rare complication in a patient with chronic renal failure. Multiple atheroemboli were found to produce transmural infarction of the left colon after elective repair of an abdominal aortic aneurysm. The pathologic process and the proposed mechanism of injury are also discussed.

COVID-19-associated multiple colonic perforations in a 6-month-old infant: a rare case report.

BACKGROUND: While COVID-19 primarily manifests as a respiratory illness, gastrointestinal complications-including severe colitis and perforation-have been reported, particularly in adults. However, such presentations in infants remain exceedingly rare. CASE PRESENTATION: A 6-month-old infant presented with lethargy and poor feeding, initially diagnosed with sepsis. Subsequent testing confirmed COVID-19 positivity. During hospitalization, the patient developed feeding intolerance, vomiting, and abdominal distension. Imaging revealed free intraperitoneal fluid with suspected perforation. Emerge

Safety of preoperative branch embolization in patients undergoing evar.

The purpose of this systematic review is to evaluate the safety of pre-endovascular abdominal aortic aneurysm repair (EVAR) embolization of aortic side branches - the inferior mesenteric artery and lumbar arteries. Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines were followed. A search of MEDLINE and DIMENSION databases identified 9 studies published from 2011 to 2021 that satisfied the inclusion and exclusion criteria. These studies were analyzed to detect the incidence of embolization-related complications. A total of 482 patients underwent preoperative aortic s

[Enteropathy in premature newborn infants. Prospective study over one year].

BACKGROUND: In neonatal units, there is a tendency to assume that any acutely sick infant with gastro-intestinal symptoms has necrotizing enterocolitis (NEC). This prospective study was conducted to find a better definition of enteropathy in preterm neonates and their risk factors. MATERIAL AND METHODS: All the 351 preterm neonates admitted to a neonatal unit from 1 August 1988 to 31 July 1989 were included in the study. A chart including 45 items was established for each infant, with special attention to data on the pregnancy, delivery, any early ischemic and/or infectious problem, nutrition

Abdominal Surgery in Patients with a Ventricular Assist Device: A Single Center Experience in Israel.

BACKGROUND: Left ventricular assist devices (LVADs) are used more commonly in patients with advanced-stage heart failure. Some of these patients may require elective or urgent abdominal surgical procedures. OBJECTIVES: To determine the outcomes of the management of LVAD-supported patients who underwent elective and urgent abdominal surgical procedures in our institution. METHODS: A retrospective review was conducted on 93 patients who underwent LVAD implantation between August 2008 and January 2017. All abdominal surgeries in these patients were studied, and their impact on postoperative mor

A black cecum: diagnostic challenges and conservative management.

A 51-year-old man with type 1 diabetes mellitus and end-stage chronic kidney disease on hemodialysis presented with 12 hours of lower right quadrant abdominal pain and bloodless diarrhea. Examination showed tenderness without guarding. Blood tests showed leukocytosis (12200/microl) and elevated C-reactive protein (190 mg/L). Abdominal CT showed ascending colon thickening (~10 cm), mesenteric fat densification, and small locoregional ganglia, without signs of arterial/venous obstruction. Given the diagnostic doubt, colonoscopy was performed and revealed the cecum mucosa and ileocecal valve with

[Two cases of colonic obstruction after acute pancreatitis].

Several forms of colonic complications are rarely observed during the clinical course of acute pancreatitis, and potentially fatal in some cases. Colonic lesions associated with acute pancreatitis can be divided into several groups from a pathogenic point of view. Possible pathogenesis includes: 1) spread of pancreatic enzymes through the retroperitoneum to mesocolon, causing pericolitis, 2) external inflammatory compression by mesocolic mass secondary to necrosis of fatty tissue, and 3) hypotension due to shock, and thrombosis of mesenteric arteries. These might lead to colonic infarction, fi

Management of Inflammatory Bowel Disease in the Elderly.

A substantial and growing proportion of patients with inflammatory bowel disease (IBD) are elderly, and these patients require tailored treatment strategies. However, significant challenges exist in the management of this population due to the paucity of data. Establishing the initial diagnosis and assessing the etiology of future symptoms and flares can be challenging as several other prevalent diseases can masquerade as IBD, such as ischemic colitis, diverticular disease, and infectious colitis. Important pharmacologic considerations include reduced glomerular filtration rate and drug-drug i

ADH1C Down-Regulation Is a Key Hypoxia Response in Colon Epithelium.

Insights into how normal epithelial cells adapt to microenvironmental perturbations may reveal molecular vulnerabilities that become obscured later in carcinogenesis, and hypoxia is common in colorectal cancer (CRC). Although colon mucosa exists in a state of physiological hypoxia and is susceptible to ischemic injury, normal colon epithelial adaptive responses to changes in oxygenation are largely uncharacterized. In this study, human colon organoids (colonoids) were subjected to sustained hypoxia in vitro with characterization of consequent phenotypes and transcriptional changes. Hypoxia tol

Ischemic bowel disease in 2021.

Ischemic bowel disease (ISBODI) includes colon ischemia, acute mesenteric ischemia (AMI) and chronic mesenteric ischemia (CMI). Epidemiologically, colon ischemia is the most common type followed by AMI and CMI. There are various risk factors for the development of ISBODI. Abdominal pain is the common presenting symptom of each type. High clinical suspicion is essential in ordering appropriate tests. Imaging studies and colonoscopy with biopsy are the main diagnostic tests. Treatment varies from conservative measures to surgical resection and revascularization. Involvement of multidisciplinary

Vancomycin-resistant Enterococcus faecium bacteraemia as a complication of Kayexalate (sodium polystyrene sulfonate, SPS) in sorbitol-induced ischaemic colitis.

We present the case report of an 80-year-old woman with chronic kidney disease stage G5 admitted to the hospital with fluid overload and hyperkalaemia. Sodium polystyrene sulfonate (SPS, Kayexalate) in sorbitol suspension was given orally to treat her hyperkalaemia, which precipitated an episode of SPS in sorbitol-induced ischaemic colitis with the subsequent complication of vancomycin-resistant Enterococcus (VRE) bacteraemia. SPS (Kayexalate) in sorbitol suspension has been implicated in the development of intestinal necrosis. Sorbitol, which is added as a cathartic agent to decrease the chan

Severe hypercalcaemia and colon ischaemia: dehydration as an unusual cause?

Hypercalcaemia is an emergency with severe consequences. Dehydration can be an uncommon cause of hypercalcaemia, as seen in this case. A 63-year-old woman with type 2 diabetes mellitus, hypothyroidism and osteoporosis, was admitted to the emergency room with abdominal distension and vomiting for 24 h. Initial evaluation was Hg 18.5 g/dL, Htc 56.2%, creatinine 2 mg/dL, metabolic acidaemia, lactate 8.3 mmol/L, anion gap 19, total Ca(2+) 17.7 mg/dL and PO4+ 6.6 mg/dL. CT revealed colonic distension without obstruction or ischaemia. Renal replacement therapy and pamidronate were initiated. The pat

Clinical features in young adult patients with ischaemic colitis.

The clinical, radiographic and endoscopic findings in 16 patients with ischaemic colitis, all of whom were < 45 years of age, were analysed. The clinical features were characterized by an acute onset of abdominal pain and rectal bleeding, and persistent constipation prior to the onset of symptoms. Twelve of the 16 patients did not have any known predisposing factors. Barium enema examination and colonoscopy revealed longitudinal ulcers and oedema of the left side of the colon of these patients. These features were then compared with those found in patients with ischaemic colitis, who were > 70

Bladder necrosis and perforation in end-stage renal disease and recurrent urinary tract infection: A rare medical emergency.

Bladder necrosis and perforation is a rare and life-threatening medical emergency. Risk factors include trauma, malignancy, previous surgery and/or radiation therapy and diabetes mellitus. Signs, symptoms, and imaging findings are often obscure making the diagnosis difficult. Urinary tract infection is common in end-stage renal disease (ESRD) patients who have residual urine production and associated with increased complication and mortality rates. We describe the case of a 57-year-old female with a medical history of recurrent cystitis, type 2 diabetes mellitus and ESRD on hemodialysis that w

Robotic-assisted laparoscopic repair of rectovesical fistula after Hartmann's reversal procedure.

The case is of a 59-year-old male with history of severe ischemic colitis following emergent intervention for a ruptured infrarenal aortic aneurysm who subsequently underwent left hemicolectomy, partial proctectomy, and Hartmann colostomy. The patient later underwent reversal of the Hartmann colostomy with diverting ileostomy. The surgery was complicated by a right ureteral and posterior bladder injury that resulted in a large rectovesical fistula involving the right hemitrigone and right ureteral orifice. An attempt to repair the rectovesical fistula at an outside facility was unsuccessful. T

[Angiodysplasia of the colon].

The authors present a case review of a congenital abnormality of the inferior mesenteric blood vessels which resulted in ischemia of the descending colon and rectosigmoid area. The resulting colitis is only very rarely secondary to a vascular congenital abnormality. In this case, rectosigmoidoscopy, barioum study of the large bowel and a biopsy of the rectal mucosa pointed towards the ischemic nature of the lesion. This led to the selective angiographic study of the inferior mesenteric vessels with findings of a dilated inferior mesenteric artery and angiodysplasia. Surgical treatment included

Inflammatory bowel disease and cardiovascular risk: A meta-analysis.

BACKGROUND: Inflammatory bowel disease (IBD) is a chronic immune-mediated intestinal disorder characterized by recurrent inflammation. Growing research over the last 10 years has connected IBD to heightened heart-related danger, with major adverse cardiovascular events (MACE, including myocardial infarction (MI), ischemic heart disease (IHD), ischemic stroke) arising earlier in life without reliance on conventional risk elements like high blood pressure or abnormal lipid levels. METHODS: A systematic literature search was performed in Medline, Embase, and the Cochrane Library to identify obse
