OBJECTIVE: To assess colonization and infection with methicillin-resistant Staphylococcus aureus (MRSA), high-level gentamicin-resistant enterococci (R-ENT) and gentamicin and/or ceftriaxone-resistant Gram-negative bacilli (R-GNB) and the factors that are associated with colonization and infection with these organisms.

Irritable bowel syndrome (IBS) has been reported in 10% to 22% of adults. The authors compared patients seeking treatment for dysthymia (N = 59) in an outpatient setting to an age- and sex-matched comparison group of patients (N = 54) seeking treatment in a general physician's office for other medical illnesses. The comparison group did not have any Axis I disorders. IBS was diagnosed by using the criteria established by Drossman and colleagues. Studies of rectal sensory thresholds and compliance in patients with the irritable bowel syndrome have produced conflicting results though there is persistent evidence of rectal hypersensitivity particularly in those with diarrhoea-predominant symptoms. This study examined rectal sensation and compliance in 31 patients with constipation-predominant irritable bowel syndrome (mean age 41 years, 27 female) and 17 healthy volunteers (mean age 45 years, 17 female). A rectal balloon was inflated with fluid at a constant rate and the volume and intrarectal pressure at sensory threshold was recorded.

Patterns of oesophageal motility were recorded in 17 healthy volunteers and 12 patients with the irritable bowel syndrome. Recordings were taken at rest and under stress by hyperventilation, a dichotic hearing challenge and a cold pressor test. The motility of the ileocaecal region of the gut was studied in 10 women with irritable bowel syndrome (IBS) and bloating and in 8 normal women. Bran labelled with 37 MBq 99mTc was administered after fasting, and a dynamic scan was done after a standard meal 3 h later. Time-activity curves were plotted for the ileum and caecum. Rectal biopsies from 6 patients with ulcerative colitis, 6 patients with irritable colon and 9 control patients were studied by formaldehyde-induced fluorescence after freeze-drying and Epon embedding. The mean number of enterochromaffin cells per crypt was higher in the patients with irritable colon whereas the patients with ulcerative colitis had fewer enterochromaffin cells than the controls. There was an abundance of enterochromaffin cells in 5 of the 6 cases of irritable colon. Various types of autofluorescent granules were visible in the cytoplasm of the macrophages.
