[Control of colorectal cancer recurrence treated with curative surgery: results of a prospective study].

Eighty four colorectal cancer patients who underwent presumably curative surgery were considered as candidates for control recurrence study. The study protocol included physical exam, blood biochemistry, chest X-ray, and CEA every 3 months (every 6 months after the third year) as well as an opaque enema, colonoscopy, abdominal echography, and pelvic CT scan (in case of abdominoperineal resection of a distal rectum cancer) every six months (once a year after the third year). Thirteen patients (15%) refused to participate in the study or abandoned it early, thus 71 patients are finally evaluated

Single or multiple doses of metronidazole and ampicillin in elective colorectal surgery. A randomized trial.

A randomized trial including 294 patients was performed to evaluate the prophylactic effect of single vs. multiple doses of antibiotics in elective colorectal surgery. All patients received 1.5 g metronidazole and 3.0 g ampicillin peroperatively and were randomized to: no further prophylactic antibiotic treatment, or ampicillin 1 g X 3 and metronidazole 0.5 g X 3 given intravenously during the second and third postoperative days. Deep wound infection was seen in 9/149 (6 percent) receiving a single dose and in 8/145 (6 percent) receiving multiple doses. No differences were found in the two gro…

Metronidazole need not be combined with an aminoglycoside when used for prophylaxis in elective colorectal surgery.

A prospective randomized controlled study has compared prophylactic intravenous metronidazole alone with the combination of metronidazole and gentamicin in 100 patients having elective colorectal surgery. There was no significant difference in mortality, infection, dehiscence or postoperative stay between the antibiotic groups. Postoperative sepsis was almost entirely due to aerobic organisms but was associated with minimal morbidity. A significantly higher rate of anastomotic dehiscence and postoperative infection was seen where the quality of bowel preparation was judged to be poor.

Comparison of systemic prophylaxis with metronidazole-fosfomycin and metronidazole-cephalothin in elective colorectal surgery.

A comparative randomized study was designed to evaluate the prophylactic effect of two different combinations of antimicrobials, i.e. metronidazole-fosfomycin (n = 30) and metronidazole-cephalothin (n = 28), in elective colorectal surgery. The study was strictly consecutive and the treatment groups comparable. The total rate of surgical septic complications was low (10%) in both treatment groups. No anaerobic infections could be demonstrated and the clinical course was mild in all patients developing septic complications. No other antimicrobial therapy was given. Peroperative bacteriological s…

Robotics on the rise, but not everywhere: National trends in robotic approach for colorectal cancer surgery.

BACKGROUND: Minimally invasive surgical approaches, including laparoscopic and robotic techniques, have been shown to improve the outcomes in colorectal cancer resection by decreasing pain, hospital length of stay, and perioperative mortality. However, broader adoption of robotics faces several barriers. This study aimed to evaluate national trends in robotic colorectal surgery using the National Cancer Database (NCDB). METHODS: Patients with stage I to III colon and rectal adenocarcinomas who underwent resection (2010-2021) were identified. Trends in surgical approaches (open, laparoscopic, …

Effectiveness of prehabilitation for patients with colorectal surgery: a systematic review and network meta-analysis.

For colorectal surgery patients, prehabilitation can address common challenges in recovery, including muscle wasting, malnutrition, and emotional stress. This study aim to determine the impact of prehabilitation for patients with after colorectal surgery. We searched PubMed, Embase and Cochrane Library from inception up to April 2024. Two authors assessed the risk of bias by the Cochrane Risk of Bias tool-version 2. We computed mean difference (MD) on the basis of change values, with 95% credible interval (CI), for continuous outcomes (length of hospital stays (LOS and 6-min walk test (6MWT)),…

The effect of myopenia, myosteatosis and visceral obesity on postoperative complications and inflammation in colorectal surgery.

INTRODUCTION: Despite advancements in perioperative care, patients undergoing colorectal surgery remain at significant risk of developing postoperative complications. Myopenia, myosteatosis and visceral obesity may impair postoperative recovery, however the exact effects are unclear. This substudy from a prospective, randomized controlled trial investigates the effects of myopenia, myosteatosis and visceral obesity on postoperative outcomes and inflammatory markers following colorectal surgery. METHODS: Adult patients undergoing elective colorectal surgery were included from a previous random…

Impact of Delayed Surgery on Local Colorectal Neuroendocrine Tumors: Is Surveillance a Feasible Option?

BACKGROUND: Guidelines for some pancreatic neuroendocrine tumors (NETs) have shifted towards active surveillance given the indolent nature of this malignancy. We sought to assess the safety of delayed surgery on colorectal NETs as a surrogate for surveillance. METHODS: Resected, stage I, well-differentiated colorectal primary NETs included in the Surveillance, Epidemiology, and End Results Program from 2010 to 2020 were included. Demographics, interval from diagnosis to surgery, and disease-specific survival (DSS) were retrospectively analyzed. Clinical stage I patients in the National Cancer

Quantification of indocyanine green fluorescence angiography in colorectal surgery: a systematic review of the literature.

BACKGROUND: Indocyanine green fluorescence angiography (ICGFA) during colorectal surgery associates with reduced post-operative anastomotic complication rates. Because its interpretation is subjective, quantification has been proposed to address inter-user variability. This study reviews the published literature regarding ICGFA quantification during colorectal surgery with a focus on impactful clinical deployment. METHODS: A systematic review was performed of English language publications regarding clinical studies of ICGFA quantification in colorectal surgery in PubMed, Scopus, Web of Scienc…

The evaluation of cognitive workload associated with indocyanine green fluorescence angiography in colorectal surgery.

BACKGROUND: Intraoperative ICGFA use is correlated with lower rates of anastomotic leak in colorectal surgery. This study evaluated the cognitive workload (CWL) associated with the interpretation of indocyanine green fluorescence angiography in colorectal surgery, relative to other operative tasks, in both simulated and actual surgeries. METHODS: Specialty surgical trainees, fellows and consultants were recruited with consent. Heart rate variability (HRV) and functional near-infrared spectroscopy (fNIRS) were used as physiological indicators of CWL during both a simulated skills lab [sequenti…

Does the indocyanine green test reduce the risk of anastomotic dehiscence in colorectal surgery?

INTRODUCTION: In colorectal surgery, assessment of the vascularization of the intestinal segments to be anastomosed by injection of indocyanine green (ICG) is one of the intra-operative measures to decrease the risk of anastomotic leak (AL). The aim of this evidence-based review was to evaluate the effectiveness and utility of this measure. METHOD: We conducted a comprehensive literature review in electronic databases (PubMed, Scopus, Cochrane Library), coupled with a manual search. Only randomized trials and meta-analyses were included. The primary outcome measure was clinical AL (Grade B an…

[Pathogenesis and progress in diagnosis and treatment of diversion colitis after colorectal cancer surgery].

Diversion colitis (DC) is a non-specific inflammation caused by the lack of fecal flow stimulation in the distal intestine after intestinal diversion surgery. It is mainly related to factors such as intestinal flora imbalance, deficiency of short-chain fatty acid (SCFA) and immune abnormalities. The clinical manifestations of diversion colitis include abdominal pain, mucus and bloody stools, diarrhea and other symptoms, but most patients may have no obvious symptoms. Diagnosis mainly relies on endoscopic examination and pathological characteristics. Common endoscopic findings include mucosal c

[A Case of Surgery for Abdominal Wall Metastasis of Colorectal Cancer to Improve the Quality of Life].

The patient was a 47-year-old woman who underwent partial colon resection and medial segmentectomy of the liver via laparotomy for descending colon cancer with a metastatic liver tumor. Postoperative chemotherapy was administered. Six months later, she developed pelvic recurrence and subsequently underwent bilateral adnexectomy, hysterectomy, and lymph node dissection, which confirmed colorectal metastasis. Two years and 6 months after the initial surgery, a gradually enlarging abdominal wall mass was detected. As the mass rapidly increased in size and became painful, an abdominal wall metasta
