Gonococcal Conjunctivitis Despite Successful Treatment of Male Urethritis Syndrome.

We report a case of progressive, cephalosporin-susceptible, Neisseria gonorrhoeae conjunctivitis despite successful treatment of male urethritis syndrome. We hypothesize that conjunctival infection progressed due to insufficient penetration of cefixime and azithromycin and point out that extragenital infection and male urethritis may not be cured simultaneously in settings where the syndromic approach is used.

Biomarker Detection and Validation for Corneal Involvement in Patients With Acute Infectious Conjunctivitis.

IMPORTANCE: Infectious conjunctivitis can lead to corneal involvement and result in ocular morbidity. The identification of biomarkers associated with corneal involvement has the potential to improve patient care. OBJECTIVE: To identify biomarkers in patients with acute infectious conjunctivitis. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study took place from December 2016 to March 2024. Analyses were performed in 3 phases. First, logistic regression and machine learning algorithms were used to predict the probability of demonstrating corneal involvement in patients with presum

The safety of metronidazole in pregnancy.

Medication use during pregnancy carries risks of teratogenicity, preterm birth, and spontaneous abortion. CDC's guidelines advocate for the use of metronidazole for the treatment of bacterial vaginosis (BV) in pregnant women. A literature review assessing the safety of metronidazole during pregnancy was conducted. Metronidazole was found to be effective in preventing preterm births when used in conjunction with other antibiotics. Its use did not predict birth defects or congenital abnormalities. It was however associated with a 70% increased risk of spontaneous abortion. This risk should be in

Bilateral simultaneous acute angle closure glaucoma following anticholinergic therapy for vestibular neuritis.

Attacks of bilateral simultaneous acute angle closure glaucoma are rare. A 59-year-old man developed vertigo and vomiting, with nystagmus in primary gaze, left-beating nystagmus in left gaze and a positive head impulse test. He was diagnosed with acute vestibular neuritis and received standard therapy, using four drugs that have anticholinergic effects. He subsequently developed bilaterally red eyes, initially diagnosed as bacterial conjunctivitis, and then blurred vision with ocular pain. An ophthalmological assessment identified mid-dilated, non-reactive pupils with corneal oedema and marked

Refractory Kawasaki Disease With a Coronary Artery Aneurysm in a Three-Month-Old Infant: Diagnostic Challenges and Rescue With Infliximab.

Kawasaki disease (KD) is an acute systemic vasculitis and the leading cause of acquired heart disease in children. Diagnosis is particularly challenging in early infancy due to incomplete presentations. We describe a three-month-old infant with persistent fever, irritability, and lymphadenopathy who was initially treated for bacterial infection. By Day 7, the patient developed conjunctivitis, cracked lips, and pharyngeal congestion, meeting criteria for incomplete Kawasaki disease. Laboratory findings included leukocytosis, anemia, thrombocytosis, elevated C-reactive protein, and hypoalbuminem

Case Report: Challenges in diagnosing Kawasaki disease in children under 3 months of age: a case series report.

Infants younger than 3 months old often present with incomplete Kawasaki Disease (KD), where not all the classical features are present. This makes the diagnosis challenging, as KD may be easily confused with other common pediatric conditions, such as viral infections, bacterial sepsis, or toxic shock syndrome. Moreover, the risk of developing coronary artery abnormalities is reportedly higher in younger infants, making timely diagnosis and treatment critical. Here we reported three cases of KD in infants under three months, each illustrating the challenges in diagnosis due to the absence of t

Otorrhea bacterial profile, epidemiology before widespread use of the third-generation pneumococcal conjugate vaccine in French children, a prospective study from 2015 to 2023.

OBJECTIVES: The aim of this study was to describe the bacterial profile of middle ear fluid from spontaneous perforation of the tympanic membrane (SPTM) prior to widespread utilization of third- generation pneumococcal conjugate vaccines (PCVs). PATIENTS AND METHODS: From October 2015 to January 2023, children with SPTM were prospectively enrolled by pediatricians. RESULTS: Among the 852 children with SPTM, 73.2% were less than 3 years old; more frequently than older children, they were and suffering from complex acute otitis media (AOM) (27.9%) and conjunctivitis (13.1%). In children under

Virological, pathological, and glycovirological investigations of an Ezo red fox and a tanuki naturally infected with H5N1 high pathogenicity avian influenza viruses in Hokkaido, Japan.

In winter/spring 2021-2022, high pathogenicity avian influenza viruses (HPAIVs) that are genetically closely related to each other were detected worldwide. In a public garden in Sapporo, Hokkaido, Japan, a crow die-off by HPAIV infection occurred from March 29 to May 18, 2022. During the event, H5N1 HPAIVs were isolated from an Ezo red fox (Vulpes vulpes schrencki) and a tanuki (Nyctereutes procyonoides albus) found in the same garden. The fox showed viral meningoencephalitis and moderate virus replication in the upper respiratory tract, whereas the tanuki showed viral conjunctivitis and secon

Chlamydia buteonis, a new Chlamydia species isolated from a red-shouldered hawk.

Chlamydiaceae are obligate intracellular bacterial pathogens for humans and animals. A recent study highlighted that a Chlamydiaceae intermediary between C. psittaci and C. abortus can infect hawks. Here, an isolate was obtained upon passage of cloacal and conjunctival sac material collected from a female hatch-year red-shouldered hawk (Buteo lineatus) in cultured cells. The diseased bird, one of 12 birds housed in a rehabilitation center, developed conjunctivitis and later died. Swabs from both sites tested positive for Chlamydia using the QuickVue Chlamydia test. The isolate, named RSHA, tes

Acute allergic reaction caused by topical azithromycin eye drops: A report of two cases.

Azithromycin 1.5% ophthalmic solution (Azyter®, Thea Pharmaceuticals, Newcastle, UK) is an effective and well-tolerated option for the treatment of bacterial conjunctivitis and blepharitis, and has the advantage of a shorter treatment duration than other topical antibiotics. No acute allergic reaction has yet been reported in response to topical azithromycin eye drops. Here, we report two cases with unusual acute-type allergic reaction to topical azithromycin eye drops. A 63-year-old female patient and 67-year-old male patient treated for blepharitis with topical azithromycin 1.5% eye drops pr

Zika Virus Infection and Solid Organ Transplantation: A New Challenge.

Public health concerns exist surrounding the epidemic of the Zika virus (ZIKV) and the rapid growth of transplantation in developing countries, including endemic zones of active arbovirus transmission, as well as travel to such regions by potential organ donors and recipients. Few data exist regarding the clinical characteristics of ZIKV infection in immunocompromised hosts. Laboratory screening protocols for transplantation to differentiate ZIKV infections from other endemic viral diseases and for the detection of possible donor-derived infection have not been stated. The diagnosis of ZIKV in
