Long-acting reversible contraception in adolescents: a systematic review and meta-analysis.

BACKGROUND: Among adolescent pregnancies, 75% are unintended. Greater use of highly-effective contraception can reduce unintended pregnancy. Although multiple studies discuss adolescent contraceptive use, there is no consensus regarding the use of long-acting reversible contraception as a first-line contraception option. OBJECTIVE: We performed a systematic review of the medical literature to assess the continuation of long-acting reversible contraceptives among adolescents. STUDY DESIGN: Ovid-MEDLINE, Cochrane databases, and Embase databases were searched using key words relevant to the pro

Contraceptive Method Use and Chlamydia Positivity Among California Family Planning Clients: The Case for New Multipurpose Prevention Technologies.

BACKGROUND: Adolescent girls and young women experience high rates of sexually transmitted infection (STI) with currently available contraceptive methods, yet few studies examine the burden of chlamydial infection by contraceptive method used. MATERIALS AND METHODS: In this cross-sectional analysis, we linked July 2012-June 2013 claims from a publicly-funded family planning program in California to chlamydia laboratory test results. Female clients were classified by the most effective contraceptive method reported by providers during the year: tier 1 (high-efficacy permanent or long-acting re

Dislocated intrauterine devices: clinical presentations, diagnosis and management.

OBJECTIVE: Intrauterine devices (IUDs) are globally one of the most popular methods of contraception. Uterine perforation is one of the most significant complications of IUD use and commonly occurs at the time of IUD insertion rather than presenting as delayed migration. This paper reports a series of 13 cases of displaced IUDs requiring retrieval by laparoscopy or laparotomy. All the IUDs were copper bearing and most perforations occurred immediately after IUD insertion. CASES: In two patients with sigmoid colon injury and IUD penetration of the appendix, laparoscopic management had failed a

Combined hormonal contraceptive (CHC) use among obese women and contraceptive effectiveness: a systematic review.

OBJECTIVE: To evaluate from the literature whether combined hormonal contraception (CHC), including combined oral contraception pills (COCs), transdermal patch, vaginal ring or combined injectables, have different effectiveness or failure rates by body weight or body mass index (BMI). STUDY DESIGN: We searched PubMed and the Cochrane Library databases for all articles in all languages published between inception and February 2016, for evidence relevant to body weight or BMI, CHC use and contraceptive effectiveness. The quality of each individual study was assessed using the system for evaluat

Bacterial flora of the cervix in women using different methods of contraception.

Bacteriologic culture samples were taken from the cervix in three groups of 10 healthy, sexually active women using barrier contraception, oral contraceptives, or a levonorgestrel-releasing intrauterine contraceptive device. Culture samples for Candida albicans and Trichomonas vaginalis were taken, a cytologic vaginal smear was obtained, and an amine sniff test was performed; these were in addition to a routine gynecologic examination. Multiple bacteria were isolated from the cervix in women using oral contraceptives or an intrauterine contraceptive device, whereas lactobacilli alone dominated

A multimodality review of gynecologic devices in the pelvis.

There are a wide variety of gynecologic devices encountered on pelvic imaging which may not be the focus or primary reason for imaging. Such devices include pessaries, menstrual products, radiation therapy devices, tubal occlusion devices, and contraceptive devices, including intrauterine devices and intravaginal rings. This manuscript offers a comprehensive review of multimodality imaging appearances of gynecologic devices encountered on pelvic imaging and discusses device indications, positioning, and complications.

Pharmacodynamics and pharmacokinetics of a copper intrauterine contraceptive system releasing ulipristal acetate: A randomized proof-of-concept study.

OBJECTIVES: To assess pharmacodynamic and pharmacokinetic outcomes of a novel copper (Cu) intrauterine system (IUS) releasing ulipristal acetate (UPA) in healthy women. STUDY DESIGN: In this single-blinded, randomized proof-of-concept study, ovulatory women received one of three Cu-IUSs releasing low-dose UPA (5, 20 or 40 µg/d) for 12 weeks. The study included a baseline cycle, three 4-week treatment-cycles and 2 recovery cycles. Primary outcomes included effects of the IUS on bleeding profile, ovarian function, and the occurrence of progesterone receptor modulator associated endometrial chan

Recent developments in the clinical use of the levonorgestrel-releasing intrauterine system.

This overview focuses on recent developments in the clinical use of the levonorgestrel- releasing intrauterine system (LNG-IUS) on established indications. Recent studies confirm that the LNG-IUS is safe and equally effective for all age groups of women. All available studies showed that intrauterine contraception is more effective when compared to non-intrauterine contraceptive methods to prevent repeat abortions. Furthermore, insertion in young nulliparous women is acceptable in the majority of women and associated with high continuation rates. The effectivity of LNG-IUS appears to be simila

A comparative study of the safety and efficacy of FemCap, a new vaginal barrier contraceptive, and the Ortho All-Flex diaphragm. The FemCap Investigators' Group.

The FemCap is a new silicone rubber barrier contraceptive shaped like a sailor's hat, with a dome that covers the cervix, a rim that fits into the fornices, and a brim that conforms to the vaginal walls around the cervix. It was designed to result in fewer dislodgments and less pressure on the urethra than the cervical cap and diaphragm, respectively, and to require less clinician time for fitting. This was a phase II/III, multicenter, randomized, open-label, parallel group study of 841 women at risk for pregnancy. A subset of 42 women at one site underwent colposcopy. Women were randomized to

Diaphragm insertion increases human vaginal oxygen tension.

The normal insertion of a contraceptive diaphragm into the vagina of five young women induced a dramatic increase in the vaginal oxygen tension (131 +/- 13 mm Hg, mean +/- SD, n = 5), which slowly fell at an initial rate of 0.9 +/- 0.2 mm Hg/min. Withdrawal of the diaphragm immediately restored vaginal oxygen tension to the normal hypoxic state (11 +/- 12 mm Hg). The change in gaseous equilibrium created by the trapped air of the diaphragm could have significant actions on the microbial activity and balance of the vagina.

[Fertility and contraception in women older than 40 years].

Women in their later reproductive years constitute a large proportion of contraceptive users. Although increasing age is accompanied by a general decline in fecundity, each individual needs to know her own risk or possibility of becoming pregnant and when it is safe to stop using contraceptives. Women over 40 need special consideration since pregnancy and childbirth is sometimes associated with higher health risks both for mother and infant. Women over 40 are more likely than younger women to have medical problems, menstrual disorders and previous experience that may affect their choice and us

Levonorgestrel intrauterine system: do the contraceptive benefits outweigh the risks in light of new FSRH guidance?

BACKGROUND: The levonorgestrel-releasing intrauterine system (LNG-IUS) is a long-acting reversible contraceptive. In 2023, the Faculty of Sexual and Reproductive Healthcare (FSRH) extended its recommended duration of use from 5 to 8 years. This change underlines the need to re-examine the balance of efficacy and risks associated with LNG-IUS. AIM: To systematically review evidence on contraceptive effectiveness, safety, and acceptability of LNG-IUS, and evaluate its alignment with updated FSRH guidance. METHOD: A systematic search of MEDLINE, Embase, PsycINFO, CINAHL, and Scopus (April 2025)

Contraception, venous thrombosis and biological plausability.

Exogenous use of hormones leads to different impact on coagulation. Usually estrogen leads to an activation of coagulation, while use of progestogens alone do not. Combined oral contraceptives (COC) differs significantly regarding VTE risk depending on amount of estrogen and type of progestagen: COC containing desogestrol, gestoden or drospirenone in combination with ethinyl-estradiol (EE) (so called 3rd or 4th generation COC) are associated with a higher VTE risk than COC with EE and levonorgestrel or norethisterone (so called 2nd generation COC). The VTE risk for transdermal COC like vaginal

Levonorgestrel Intrauterine Device Placement in Uterine Anomalies: A Case Series.

BACKGROUND: Levonorgestrel intrauterine devices (IUDs) provide effective contraception and medical benefits. They are safe for even severely medically complex patients. The CDC Medical Eligibility Criteria (MEC) states that IUDs are category 4 for people with uterine cavity distortions; however, there are previous case reports of their successful use in this population. CASES: We present 5 cases of IUD use in uterine anomalies utilizing ultrasound or hysteroscopy for intra-cavitary placement confirmation. With up to 4.5 years of follow-up, our expulsion rate is 40%. In those whose IUDs were e
