Transfusion medicine laboratories routinely perform a series of pretransfusion serological tests including: ABO grouping, Rh typing, and investigation of the recipient's serum to detect antibodies against blood group antigens (antibody screen). As a final check, most laboratories also perform a crossmatch in which the recipient's serum is incubated with the donor's red cells followed by the addition of an antiglobulin reagent (antiglobulin crossmatch). The need for the antiglobulin crossmatch when the antibody screen is negative has been questioned because there are few antibodies that are det

A 50-year-old woman with primary biliary cirrhosis developed immune hemolytic anemia and renal failure while receiving rifampicin for the treatment of refractory pruritus. Serological studies revealed the presence of rifampicin-dependent antibodies of the IgM class that, when tested against a wide panel of erythrocytes, had anti-I specificity. Subsequently, rifampicin was withdrawn and prednisone treatment instituted, this resulting in a rapid resolution of the hemolysis, whereas hemodialysis was required for recovery of the renal function. A role is suggested for the anti-I specificity of the

Delayed hemolytic transfusion reactions due to anti-U are rare, only two (2) cases having been reported in the literature. We now report a third case: a multiparous black woman without any transfusion history was admitted to hospital for severe microlytic anemia (31 g/l). The patient was group AB negative, the direct antiglobulin test was negative and an anti P1 cold allo-antibody was present in her serum. Five A, Rh negative, P2 packed red cells were cross-marched with the sample obtained at admission on January 8, 1988. She was transfused on January 8, 9, 10, 11 and 12. On the 12th of Januar

The purpose of this study is to give an overall picture of the results obtained from antibody screening in 80,509 pregnancies, the influence of irregular antibodies on the outcome of the fetus and neonate and the frequency of exchange transfusions performed in babies suffering from hemolytic disease. With regard to the strength of agglutination in 1282 positive direct anti(human)globulin tests in newborns observations are made whether or not it would be possible to draw conclusions on the causative maternal antibody. Based upon the observed number of pregnancies at risk resulting in hemolytic

A patients of blood type A1 developed brisk, but transient haemolysis after receiving a platelet transfusion derived from 4 group 0 donors. Anti-A was detected on his red cells and in his plasma. 2 of the platelet donor were found to have very high titers (1:10240) of anti-A and positive haemolysin tests. Thus, a haemolytic reaction can result from transfusion of incompatible plasma in a platelet concentrate.

As improved diagnostic reagents become available, greater diagnostic accuracy can be expected. After a thorough clinical and laboratory examination, intensive therapy must be started immediately and maintained until clinical remission is achieved. An effort must be made to determine the existence of associated disease states and to institute appropriate treatment. Prognosis is guarded when AIHA is accompanied by severe hepatic and renal diseases, especially in the older animal. Prognosis is more guarded when AIHA is associated with either AITP or SLE. Remissions are to be expected and necessit

Auto-immunohaemolytic anaemia is a very unusual complication during long-term treatment with ibuprofen. In order to detect the haemolytic antibodies involved, serum and erythrocytes from 87 patients were investigated after continuous treatment with an average daily dose of 1337 mg ibuprofen for some 6-47 months. Eight patients showed a weak or medium-positive antiglobulin test result with polyspecific anti-human serum. With monospecific anti-human serum, complement C3c and/or C3d were detected on the surface of 8 patients' erythrocytes, but none had biochemical parameters indicating haemolysis

The amount of IgG present on the surface of platelets (platelet-bound IgG, or PBIgG) was measured using a quantitative antiglobulin consumption assay developed by Dixon et al. (1975). Normal platelets or platelets from patients with thrombocytopenia due to a decreased production had less than 10 X 10(-15) g of IgG/platelet. Studied were 364 blood samples drawn from 196 patients; 173 patients had idiopathic thrombocytopenic purpura; 65 (93 %) of 70 in the acute phase of their disease had elevated PBIgG (values up to 100 times greater than the normal); 98 (76.5 %) of 128 blood samples from patie

Acute intravascular haemolysis and renal failure developed while a patient was taking triamterene. A direct antiglobulin test with a polyvalent reagent was positive. Serum caused agglutination of normal red cells in the presence of triamterene and caused an increase of partial haemolysis of both trypsin-treated erythrocytes and red cells from a patient with paroxysmal nocturnal haemoglobinuria (PNH) in the presence of complement. From the results of antibody-neutralization test and treatment with 2-mercaptoethanol, the presence of IgM antibody with lambda light chain could be demonstrated. The

These patients require admission to an In …

Fetomaternal hemorrhage (FMH) refers to the entry of fetal blood into the maternal bloodstream before or during delivery. FMH of more than 30 mL occurs with the frequency of about 1/300. Fetal outcomes may be compromised by still births, hydrops fetalis, cardiac complications, and increased rates of postpartum infant death. In most cases, the cause is not identified. Clinical manifestations of FMH depend on the volume of blood lost and the rate that it occurred. We report a case of chronic massive FMH in a newborn of an immigrant mother with a favorable outcome. Medical visits and tests during

The MRSPAH (mixed reverse solid-phase passive antiglobulin haemadherence) test for antiglobulins (Agbs) of IgA, IgG and IgM classes has been quantified and also modified to measure their C3 fixing activity. An alternative ELISA technique is also described. Levels of all Agbs and their C3 fixing activity were significantly raised in established rheumatoid arthritis (RA) patients, early RA patients and established SLE patients. Ankylosing spondylitis (AS) patients had normal levels of Agbs and C3 fixation. Patients with infectious mononucleosis (IM) had high levels of all Agbs, but these did not

Brucellosis is a zoonotic disease that mimics many other diseases, making diagnosis difficult in pediatric patients. If the clinical signs of the disease are not well known and there is a delay in diagnosis and treatment, complicated brucellosis involving different body sites can develop. This study aimed to analyze the demographics, clinical information, laboratory test results, and imaging findings of children with brucellosis. Particular attention was paid to the involvement of organs and potential complications. Pediatric patients diagnosed with brucellosis at the Sanlıurfa Training and Re

INTRODUCTION: Vitamin B12 deficiency may rarely present with hemolysis and schistocytosis, mimicking thrombotic microangiopathy (TMA). This pseudo-TMA variant is clinically significant, as delayed recognition may lead to unnecessary treatments and adverse outcomes.
