Incidence and prevalence of seropositive rheumatoid arthritis among Korean women of childbearing age: a nationwide population-based study.

BACKGROUND/AIMS: Women with rheumatoid arthritis (RA) are often diagnosed with the disease during their reproductive years; however, its incidence and prevalence among women of childbearing age have not been studied. The objective of this study was to estimate the incidence and prevalence of seropositive rheumatoid arthritis (SPRA) among Korean women of childbearing age. METHODS: Women aged 20 to 44 years with SPRA were identified from National Health Insurance Service-National Health Information Database (2009 to 2016). SPRA was defined by International Classification of Diseases, 10th revis

Neuro-ophthalmology malpractice: A review of the Westlaw Database.

BACKGROUND: Neuro-ophthalmologic conditions are at a higher risk of misdiagnosis compared to other ophthalmic conditions. Increased awareness of the most common diagnostic errors in neuro-ophthalmology that lead to malpractice claims can allow ophthalmologists to further improve their diagnostic workup to reduce delays in diagnosis and management, while also mitigating the risk of litigation. METHODS: Malpractice trials in the Westlaw Legal Database that involved cases of neuro-ophthalmologic diagnostic errors or failures by ophthalmologists were included in this study. RESULTS: A total of 4

Stepwise removal compared to complete removal for deep carious lesions.

INTRODUCTION: Carious lesions have traditionally been treated by non-selective (complete or total) removal of carious tissue. However, due to its risks and adverse effects, carious tissue removal techniques have been developed, based on the preservation of tooth tissue, including the stepwise removal. The objective of this summary is to clarify the uncertainty about the effectiveness and safety of the stepwise removal technique compared to complete removal. METHODS: To answer this question we used Epistemonikos, the largest database of systematic reviews in health, which is maintained by sear

Analysis of in court malpractice litigation following pancreatic surgery.

BACKGROUND: Pancreatic operations are technically complex with a significant risk of morbidity and mortality. Analysis of litigation following surgery can disclose avoidable errors. METHODS: Two legal databases, Westlaw and Lexis Nexis, were queried for jury verdicts, settlements and appeal cases in the United States related to pancreatic surgery and malpractice between 1980 and 2020. RESULTS: Thirty-four cases were analyzed. Pancreaticoduodenectomy (n = 22, 65%) was the most common procedure litigated. Claims most commonly involved malpractice in the postoperative (n = 16) setting. The most

An ensemble framework based on Deep CNNs architecture for glaucoma classification using fundus photography.

Glaucoma is a chronic ocular degenerative disease that can cause blindness if left untreated in its early stages. Deep Convolutional Neural Networks (Deep CNNs) and its variants have provided superior performance in glaucoma classification, segmentation, and detection. In this paper, we propose a two-staged glaucoma classification scheme based on Deep CNN architectures. In stage one, four different ImageNet pre-trained Deep CNN architectures, i.e., AlexNet, InceptionV3, InceptionResNetV2, and NasNet-Large are used and it is observed that NasNet-Large architecture provides superior performance

Analysis of endotracheal intubation-related judicial precedents in South Korea.

BACKGROUND: Medical malpractice during endotracheal intubation can result in catastrophic complications. However, there are no reports on these severe complications in South Korea. We aimed to investigate the severe complications associated with endotracheal intubation occurring in South Korea, via medicolegal analysis. METHODS: We retrospectively analyzed the closed judicial precedents regarding complications related to endotracheal intubation lodged between January 1994 and June 2020, using the database of the Supreme Court of Korea. We collected clinical and judicial characteristics from t

A Critical Appraisal of Evidence- and Consensus-Based Guidelines for Actinic Keratosis.

Actinic keratoses (AK) are common lesions of the skin that can be effectively treated with several lesion- and field-directed treatments. Clinical practice guidelines assist physicians in choosing the appropriate treatment options for their patients. Here, we aimed to systematically identify and evaluate the methodological quality of currently available guidelines for AK. Guidelines published within the last 5 years were identified in a systematic search of guideline databases, Medline and Embase. Then, six independent reviewers evaluated the methodological quality using the tools "Appraisal o

Improved Otsu and Kapur approach for white blood cells segmentation based on LebTLBO optimization for the detection of Leukemia.

The diagnosis of leukemia involves the detection of the abnormal characteristics of blood cells by a trained pathologist. Currently, this is done manually by observing the morphological characteristics of white blood cells in the microscopic images. Though there are some equipment- based and chemical-based tests available, the use and adaptation of the automated computer vision-based system is still an issue. There are certain software frameworks available in the literature; however, they are still not being adopted commercially. So there is a need for an automated and software- based framewor

Design and Validation of a New Diagnostic Tool for the Differentiation of Pathological Voices in Parkinsonian Patients.

Pathological speech, in its many forms, is a symptom of numerous serious diseases affecting millions of people worldwide, including more than 10 million Parkinson patients. Here, a powerful method is proposed for detecting pathological speech, using a two-dimensional (2D) convolutional neural network (CNN). Spectrograms are extracted from voice recordings of healthy and Parkinson diagnosed patients, which are fed into the CNN architecture. The voice samples comprise a subset of the benchmark mobile Parkinson Disease (mPower) study. The proposed model achieves 98% accuracy in Parkinson detectio

Defining Urinary Tract Infections in Children With Spina Bifida: A Systematic Review.

CONTEXT: Children with spina bifida are at high risk for urinary tract infections (UTI). However, there is no standardized definition of UTI in this population, leading to variability in both clinical management and research. This was highlighted in the 2013 systematic review on the same topic. OBJECTIVE: Evaluate the frequency with which researchers are defining UTI in their studies of children with spina bifida and to determine what parameters are used. DATA SOURCES: We searched Medline and Scopus databases for articles that included pediatric patients with spina bifida and used UTI as an

Automatic data extraction from 24 hour blood pressure measurement reports of a large multicenter clinical trial.

BACKGROUND AND OBJECTIVES: Ambulatory blood pressure monitoring (ABPM) is usually reported in descriptive values such as circadian averages and standard deviations. Making use of the original, individual blood pressure measurements may be advantageous, particularly for research purposes, as this increases the flexibility of the analytical process, enables alternative statistical analyses and provide novel insights. Here we describe the development of a new multistep, hierarchical data extraction algorithm to collect raw data from .pdf reports and text files as part of a large multi-center clin

A classification-based approach to semi-supervised clustering with pairwise constraints.

In this paper, we introduce a neural network framework for semi-supervised clustering with pairwise (must-link or cannot-link) constraints. In contrast to existing approaches, we decompose semi-supervised clustering into two simpler classification tasks: the first stage uses a pair of Siamese neural networks to label the unlabeled pairs of points as must-link or cannot-link; the second stage uses the fully pairwise-labeled dataset produced by the first stage in a supervised neural-network-based clustering method. The proposed approach is motivated by the observation that binary classification
